STATEMENT OF WORK
HEATED SEALED MATTRESS
AND BLUE NYLON SEWN MATTRESS
Delivery, Removal, Installation Joint Base San Antonio (JBSA) Fort Sam Houston (FSH) Dorm Mattresses
1. PROJECT DESCRIPTION:
Procure 3560 mattresses for Joint Base San Antonio (JBSA) Fort Sam Houston. The contractor shall ensure the mattresses are properly delivered based on industry standards. Contractor shall deliver to the locations shown below, which is located within secured military installation.

Twin Size, Heat Sealed Mattress, Blue 38”x 80”x 10.5” ±1 inch variance 12-14 gauge tempered steel spring wire 275-285 coil count for twin size.
Shall have minimally 2 pieces of 1” thick thermally bonded fiber batting (Fire barrier)

1.1. SCOPE: Contractor shall provide all management, tools, supplies, equipment, labor and transportation necessary to perform this requirement in accordance with the statement of work. The Contractor should be able to make adjustments without impact to the user with government critical mission training.
2. PROJECT MANAGEMENT:
Contractor shall provide the government a 15-day notification prior to delivery and a 24-hour notification prior to truck arrival. Furthermore, the Contractor shall also provide email notification as to the date and time the trucks will be arriving to the Technical Representatives. The Technical representatives (TR) for this project are:

3. DELIVER/STAGE/SETUP
4.1. NOTE: The buildings are currently occupied.
4.2. The mattresses needs to be deliver and installed into the noted buildings stated in the table above.
4.3. The installation date shall be coordinated with the Technical Representatives. This project requires delivery and installation of the mattresses to multiple buildings listed above on JBSA Fort Sam Houston, TX 78234. There are elevators in BLDG 3312 and 3314 that can be utilized. For BLDG. 1350 there is no elevators, has 3 floors and 1stairwell to access floors building has 5 wings. All materials and accessories shall be installed in accordance with industry standards and manufacturer’s recommendations.
4.4. The contractor shall clean up and dispose of waste materials or rubbish in accordance with this paragraph and all applicable federal, state and local regulations, standards, codes and laws.
4.5. Contractor shall unload and install all new mattresses, remove all old mattresses that are currently in place and transport the old mattress to the contractor provided/supplied dumpster. The contractor will remove 1400 mattresses from bldg 3312,1400 mattresses from bldg 3314, and 500 from bldg 1350. The contractor is responsible for providing any roll-off dumpsters required and responsible for removing as necessary to complete this project; the Government will not provide nor supply any dumpsters or Government sites for this project. The Contractor shall not dispose of or discard recyclables, packing materials or trash in any recycling or trash containers or dumpsters located on the installation.
4.6. STORAGE AND PARKING: No contractor storage will be provided for this project. Parking will be in the vicinity of the job site. There is no loading dock.
5. HOURS OF OPERATION:
5.1. Normal Hours. Normal Hours. Performance of work shall be during normal duty hours between 7:30 am - 4:30 pm, Monday through Friday unless otherwise requested in writing and approved by the Contracting Officer.
Recognized Holidays. The contractor is not required to provide scheduled service on the following holidays:
· New Year’s Day, January 1st
· Martin Luther King’s Birthday, 3rd Monday in January
· President's Day, 3rd Monday in February
· Memorial Day, Last Monday in May
· Independence Day, July 4th
· Labor Day, 1st Monday in September
· Columbus Day, 2nd Monday in October
· Veterans Day, November 11th
· Thanksgiving Day, 4th Thursday in November
· Christmas Day, December 25th
Note: Any of the above holidays falling on a Saturday will be observed by the government on the preceding Friday. Holidays falling on a Sunday will be observed by the government on the following Monday.
6. WORKMANSHIP: The Contractor shall perform all operations in a manner that will result in a professional and expeditiously completed project. The work shall be in strict accordance with the prevailing industry standards.
7. SUBMITTALS: Provide product data on specified products.
8. QUALITY ASSURANCE: The government shall inspect all work performed by the contractor. Work performance shall be deemed satisfactory and acceptable to the government when the 502nd CES/CEIH declares the project satisfactory.
9. PROTECTION: The contractor shall take precautions to prevent damage to government property. The contractor shall repair or replace damaged items at no additional cost to the government.
10. COMPLETION OF WORK: The contractor shall ensure that work for this project is performed in accordance with the criteria herein, and that all relevant materials, finishes, equipment, and systems shall be fully operational at the completion of work for this project.
11. ENVIRONMENTAL REQUIREMENTS: The contractor shall perform all work in accordance with federal, state, and local environmental regulations.
12. BASE ACCESSES: Contractor shall contact Government point of contact for base access at minimum of 10 days prior to delivery in order to submit for base pass access/request. Contractor shall provide the proper documentation necessary (name of individual requiring base access, state driver’s license number, date of birth, etc.) for any contractor employee requiring access. All contractor vehicles must enter and depart through JBSA Fort Sam Houston via the Jadwin Gate Access Control Point which can be accessed via Interstate 35.
