SCOPEOFWORK

for

Refuge Collection/Disposal Service

Location Services will be provided at the following Residential locations: Shiprock Agency (upper quarters), Aneth School, Beclabito School, Cove Day School, Nanahnezad Community

School, Red Rock Day School, T'iis Nazbas Community School, and Toadlena Community

School (T ohaali) BIA Quarters.

Notifications/Coordination

• The Contractor shall accept directions only from the Contracting Officer (CO) or the duly authorized Contracting Officer Representative (COR).

• Request made directly by end users or site personnel must be referred immediately to the

CO and/or COR.

• Any changes, scheduling, or special coordination shall be communicated in writing through the CO or COR.

Period of Performance The period of performance for this contract is 360 calendar days from the issuance of the Notice to Proceed, with services performed weekly.

Service and Hours of Operations Requirements The contractor shall provide waste container bins, remove waste container bins and transport to an environment approved landfill. Waste shall be removed on a weekly basis between the hours of 8:00 a.m. to 5:00 p.m. Monday through Friday.

SCOPE OF WORK

1. Contractor shall provide new or well-maintained waste containers free of graffiti. Waste containers delivered with graffiti shall be removed within 24 hours.

2. Contractor shall inform drivers ofbin(s) that are damaged and/or with graffiti.

3. Contractors shall replace within two (2) business days any damaged bin(s) caused by the contractor's drivers at no cost to the Government. The damaged bin(s) shall be replaced/or replace the damaged part within two (2) business days of written notification by the CO/COR.
