About this file

This is an Indian Economic Enterprise (IEE) Representation Form issued by the Indian Health Service (IHS), Office of Management Services, Division of Acquisition Policy for solicitation #75H71525Q00008 regarding SFC Laptops.

The form requires offerors to self-certify that they meet the definition of an "Indian Economic Enterprise" under the Buy Indian Act (25 U.S.C. 47) throughout three key periods: at time of offer submission, at contract award, and during the full contract performance period. Offerors must indicate whether they meet the definition of an IEE with "Indian Ownership" by checking the appropriate box and providing details including the name of the Federally Recognized Tribal Entity, the 51% owner of the IEE firm, business name, and DUNS number. The form warns that false certifications are punishable under various U.S. codes and requires registration in the System for Award Management (SAM).
