SERVICES
SCOPE of WORK
SEMI-ANNUAL ROOF INSPECTION
Basset Army Community Hospital
Fort Wainwright, ALASKA
17 FEBRUARY 2020
1.1. Project Summary: Provide Semi-Annual Roof Inspection, Maintenance Services and Reporting on various buildings under the responsibility of MEDDAC-AK as required in accordance with this SOW, and the attachments.
1.2. Facility: Bassett Army Community Hospital (BACH) is a 313,202 square foot combined, Patient Care and VA Clinic facility. The “Essential Facility” designation requires the hospital to withstand a 1000 year event (10.5 earth quake) and be capable of operating without any commercial utility support for 96 hours. Bach Facility Management Headquartered at Ft Wainwright (Ft WW) is also responsible for (5) other Facilities on the Ft WW Post; (1) Facility at Ft Greely Post and (4) Facilities at the Joint Base Elmendorf / Ft Richardson (JBER)
1.3. Purpose: Reroof of the BACH Facility Bldg 4076 was Completed AUG 2019. The new Carlisle Syntec Roof System carries a 20 Year Full System Warranty. The Terms of the Warranty Require a Semi-Annual Roof Inspection and Maintenance (2 Inspections per year) including Documentation of the Inspection and any Maintenance.
In addition the following list of roofs under the MEDDAC-AK Facilities are in various stages of warranty with equally important Inspection and maintenance requirements to insure the integrity and longevity of the facility roof systems.
1.3.1. Ft. WW: Bldgs. 4076, 4077, 3406, 3567, 3597, 4066
Ft Greely: Bldg. 655
JBER Bldgs. 631, 786, (47815 & 47810)
2.0 Execution: The Contractor shall provide all supervision, labor, materials, transportation, equipment ( Ladder’s, Scaffold, Manlift’s, Fall Protection) and storage as required to perform the service in accordance with the attached specifications, sound commercial practices, industry and facility standards, and applicable Manufacturers’ requirements. All work shall be accomplished in accordance with applicable federal, state, and local regulations and safety requirements.
2.1. The Contractor shall Inspect each building roof and provide detailed separate reports for each building on the form provided. Digital photos of any defects, and a minimum of 1 photo per deck area shall be provided with the report.
2.2. Initial Inspections shall occur immediately after contract award then twice annually: One as early as practical in the spring, once roof surfaces are free of all ice and snow, and not later than May 15th; and a second at the end of summer not sooner than August 30th or later than September 30th.
2.3. Contractor shall use qualified, experienced personnel to perform the inspections and maintenance. Minimum 3 Years experience in each type of Roof Installation.
2.4. Contractor shall be certified by the manufacturer of any warranted roofs, and utilize only Manufacturers approved materials for repairs.
2.5. The Contractor shall perform all service activities so as to avoid interference with the operation of the facility and the work of others. Work shall take place during regular business hours during weekdays Monday – Friday 06:30-16:30. Hours outside those parameters may be granted on prior notification and approval. Under no circumstances shall the work interfere with or delay patient care. Any work affecting Life Safety Code requirements and NFPA will require ILSMs to be implemented to mitigate these deviations during any maintenance operations.
2.6. Upon award of the contract, Contractor shall meet with the Facility Management staff / COR for a Pre-Con meeting, before beginning work.
2.7. All trash will be removed from the site on a daily basis. Upon completion of the project, all equipment, materials and any other items associated with the project will be removed from the project site. The site will be left clean.
2.8. Anything mentioned in the Scope of Work and not shown on the drawings, or shown on the drawings and not mentioned in the Scope of Work, shall be of like effect as if shown or mentioned in both.
2.9. The Contractor shall be responsible to verify all measurements and dimensions.
Contractor shall clarify with the Project Manager any discrepancies in measurements shown on the drawings. Conflict and/or clarity issues in reference to items, descriptions or drawings within and of this scope of work will be resolved with the writer of this scope.
2.10. Contractor will be required to provide documentation of Inspection and maintenance to Facilities Management (example included in attachments)
3.0 Site Security: The Contractor’s employees are required by the Anti-Terrorism – Operational Security form to receive access and general protection policy and procedures which will be provided when obtaining a permit to access post through the front gate. The Contractor’s employees and subcontractor employees requiring access to Army installations, facilities, and controlled access areas shall complete Anti- Terrorism Level I awareness training which can be accessed online at the following address: http://jko.jten.mil/courses/atl1/launch.html. The Contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee as a submittal. The Contractor’s employees and subcontractor employees requiring access to Army installations, facilities, and controlled access areas shall complete initial Operations Security (OPSEC) training which can be accessed online at http://jko.jten.mil/courses/atl1/launch.html the following address: https://securityawareness.usalearning.gov/opsec/. The Contractor shall provide certificates of completion for each affected contractor employee and subcontractor employee as a submittal. The Contractor’s personnel are also required to complete the iWATCH training which can be accessed online at the following address:
http://www.myarmyonesource.com/familyprogramsandservices/iwatchprogram/default.a spx. Have employees sign attendance roster for completing the iWATCH training and provide as a submittal. The Contractor’s employees are not to divulge anything they may see or hear with regards to the activities and operations of units within the workplace or anywhere else on and off the installation. Employees are not to discuss anything they are doing on the installation to anyone that does not need to know.
4.0 Payments: Contractor shall use WAWF and Input Invoices as a 2 in1 Invoice.
5.0 Contractor Manpower Reporting: The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://contractormanpower.army.pentagon.mil
6.0 The required information includes:
(1) Contracting Office, Contracting Officer, Contracting Officer’s Technical Representative;
(2) Contract number, including task and delivery order number;
(3) Beginning and ending dates covered by reporting period;
(4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data;
(5) Estimated direct labor hours (including sub-contractor);
(6) Estimated direct labor dollars paid this reporting period (including sub-contractor);
(7) Total payments (including subcontractor);
(8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different);
(9) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information);
(10) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest City, Country, when in an overseas location, using standardized nomenclature provided on website);
(11) Presence of deployment or contingency contract language, and,
(12) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide https://securityawareness.usalearning.gov/opsec/ http://www.myarmyonesource.com/familyprogramsandservices/iwatchprogram/default.aspx http://www.myarmyonesource.com/familyprogramsandservices/iwatchprogram/default.aspx the estimated total cost (if any) incurred to comply with this reporting requirement.
Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year.

Drainage Inspect the roof surface to ensure free drainage. Clean accumulated dirt and debris on the roof and particularly at drain sumps and scuppers.
Insure Drain Screens are properly seated to filter debris.

Kitchen Exhaust Inspect the roof surface to ensure there is no evidence of expelled kitchen wastes, animal fats or vegetable oils present on the membrane.

Inspect Curb Flashings, Outside Corners, Membrane Seams, Metal Cap Flashings and Mounted Attachments and Penetrations.
Membrane, Seams and Flashings
Inspect membrane surface, membrane seams and membrane flashings for Deterioration, fish-mouths, and adhesion Integrity. Special attention paid to Inside / Outside corners.
Foot Traffic
Inspect adhesion and placement of walk pads.
Pipe and Conduit Penetration Flashings Inspect Pipe Boots, Pourable Sealer Pockets and Pipe Wraps for fishmouths, deterioration, sealants, and clamps.
Miscellaneous Installed Items on Roof
Inspect Installed Items: Antenna, Curbs, Stairs, Ladders or Other Equipment and associated flashings for watertightness and integrity of adjacent membrane.
Metal Work
Inspect for deterioration, rust and watertightness of Metal Copings, Counter Flashings, Termination Bars, Metal Curbs and associated fasteners and sealants.
Leaks
Inspect roof surface to locate any reported active leaks and determine source. Make Temporary Repairs
Temporary Repairs
Make Temporary repairs as necessary. Use Manufacturers White or Black Lap Sealant as appropriate.
Special Items
Inspect areas from previous inspections regarding adhesion of cover boards and insulation for any increase in size.
Report Inspection Results on the Form and locate issues on the associated Roof Plan.
Provide Electronic Photos Referenced on plan of any defects and subsequent repairs.
