STATEMENT OF WORK (SOW)-Air Compressor System

2. Contract Title. Air Compressor Maintenance
3. Background.
Contract is required to ensure that the Great Lakes Consolidated Mail Outpatient Pharmacy (GL- CMOP) processes high volume, outpatient mail prescription workloads using compressed air to the Production Equipment and Conveyor System.
4. Scope.
The Veterans Affairs, (GL-CMOP) intends to solicit and award a contract to provide preventative maintenance service for the Air Compressor System which consists of Two(2) Model Number RK-60VSD-115B Compressors , Two(2) RDF-375-NC-216 Compressed Air Dryers, and One(1) OWS-725 Oil and Water Separator located in the GL-CMOP, at Building 37 West Hines, Illinois 60141. Services shall be in accordance with the terms, conditions, provisions and schedule of the contract. Services shall be for the period of _1 September, 2022__ through _30 August, 2027__, with four (4) pre-priced one (1)-year options for renewal.
5.1 Specific Tasks-Deliverable Product.
5.1.1 Contractor shall provide all labor, materials, equipment, transportation, lodging and supervision necessary to perform preventative maintenance for both Model Number RK-60VSD-115B Compressors , both RDF-375-NC-216 Compressed Air Dryers, and the OWS-725 Oil and Water Separator Air Compressor System located in the GL-CMOP, at Building 37 West Hines, Illinois 60141. Any issues with equipment will be documented in the inspection checklist provided at the bottom of the SOW and the COR shall be notified immediately of any critical issues with equipment immediately upon discovery during the inspection. Critical issue is defined as an issue in the technicians opinion can cause immediate failure of the equipment if not addressed immediately, see section 5.1.3 for further details.
5.1.2 The annual and first semiannual inspection will be completed within 90 days of contract award and exercising of option years. The second semiannual inspection will not be scheduled and completed within 90 days of the first inspections but must be completed 90 days before the end of period of performance for the given contract year. Any deviations to this schedule must be cleared with the CO with COR concurrence for scheduling.
5.1.3 If a critical issue is discovered during any inspection the COR will be immediately notified of the issue to include the scope and severity the issue poses to the proper functioning of the identified equipment. The vendor can submit a quote for repairs to include materials, labor hours and timelines for completion to be funded as needed on the miscillanuose repairs CLIN. All proposals for repair must be approved by the CO before any repairs are started.
5.2 Service Performed Under Maintenance Plan

5.3 Hours of Operation
All work for this contract is to be done on During normal hours of operation (Monday thru Friday 6:30AM to 4:00PM. The facility will observe the following Federal Holidays dates changed per the appropriate calander year coorisponding to the contract period of performance.

6. Performance Monitoring Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the performance monitoring of this contract.
6.1 Direct Observation-(Can be performed periodically or through 100% surveillance.) The COR does not have the authority to modify or approve any alterations to the SOW or contract. Any changes or issues must be addressed through the Contracting Officer.
a. ACCEPTABLE PERFORMANCE
The Government shall document positive performance. Any report may become a part of the supporting documentation for any contractual action.
b. UNACCEPTABLE PERFORMANCE
When unacceptable performance occurs, the COR shall inform the contracting Offcier. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file.
When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor's program manager for review before submission to the Contracting Officer.
The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR and Contracting Officer. The Government shall review the contractor's corrective action plan to determine acceptability.
Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the CO.
7. Security Requirements
Contractor will not have access to VA Sensitive Information, patient records, or data. Contractor will not have access to government computer systems.
For repairs or services performed during normal work hours, 6:30 a.m. to 4:00 p.m. at the GL- CMOP , contractor’s service technician shall report to Door 27 Adminstration, Building 37.
Contractors shall sign-in and obtain a badge and return the badge at the end of the day or when required service is completed. After all work is complete, the contractor’s service technician shall again report in person to the location listed above, submit a written service report outlining the services performed and completed to include location and parts used.
For services performed outside normal working hours, the contractor’s service technician must report to Door 27 Adminstration at GL-CMOP and call Facility Management Officer,Carren Christianson, work Cellular phone (708) 657-8593. After all work is complete the contractor’s service technician must report to Administration Office C1037 where the service technician must submit the report required above.
8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).
Vendor to provide all labor, material, equipment, transportation, lodging and supervision necessary to perform preventative maintenance for Two(2) Model Number RK-60VSD-115B Compressors , Two(2) RDF-375-NC-216 Compressed Air Dryers, and One(1) OWS-725 Oil and Water Separator located in the GL-CMOP, at Building 37 West Hines, Illinois 60141.
The GL-CMOP will not furnish parts and/or test equipment for the performance of this contract.
It is the responsibility of the contractor to bring the appropriate equipment; and/or supplies necessary to complete the work as required within.
9. Other Pertinent Information or Special Considerations.
All work must be performed by personnel qualified to work on the specific equipment.
Contractor will remove and properly dispose of any oils resulting from servicing the air compressors covered in this agreement.
10. Place of Performance.
The Two(2) Model Number RK-60VSD-115B Compressors , Two(2) RDF-375-NC-216 Compressed Air Dryers, and One(1) OWS-725 Oil and Water Separator located in the GL- CMOP, at Building 37 West Hines, Illinois 60141.
