PERFORMANCE WORK STATEMENT (PWS)

FOR

AFPET/PTPLE

FLOOR CLEANING & MAINTENANCE SERVICES

BLDG. 6670

26 July 2021

VANDENBERG SPACE FORCE BASE, CA

1. Objectives.

635 SCOW/AFPET/PTLE requires weekly vacuuming of carpeted areas and weekly sweeping and mopping of tiled surface floors in the hallways and common areas, excluding the laboratories, in Bldg. 6670 for a period of 12 months. AFPET requires one (1) time annual waxing of all hard-surface floors in Bldg. 6670, to include the laboratories, to be performed after the normal duty day when the facility is unoccupied by facility personnel.

AFPET also requires one (1) time annual shampooing of all carpeted areas in Bldg. 6670 to be performed after the normal duty day when the facility is unoccupied by facility personnel.

2. Scope.

The Contractor shall provide all labor, supervision, materials, tools, equipment, transportation, and other items necessary to provide floor cleaning and maintenance services for Bldg. 6670 on Vandenberg Space Force Base (VSFB), CA. The Contractor shall vacuum, sweep, and mop all non-laboratory areas every week to maintain floor finishes.

The Contractor shall strip and wax all tiled floors including laboratories in Bldg. 6670 one

(1) time during the 12 month performance period. The Contractor shall also shampoo and thoroughly dry all carpeted areas in Bldg. 6670 one (1) time during the 12 month performance period.

3. Description of Work.

3.1. Carpet Cleaning. The Contractor shall provide weekly vacuuming of carpeted areas in

Bldg. 6670. Work shall be performed during the normal duty day. Bldg. 6670 has approximately 136 sq. yd. (1,224 sq. ft.) of carpet that requires weekly vacuuming.

3.2. Carpet Shampooing. The Contractor shall provide one (1) time annual shampooing of carpeted areas within Bldg. 6670. Work shall be performed after the normal duty day.

3.3. Sweeping & Mopping. The Contractor shall provide weekly sweeping and mopping of all tiled surface floors in the hallways and common areas of Bldg. 6670. Work excludes the laboratories and shall be performed during the normal duty day. Bldg.

6670 has approximately 280 sq. yd. (2,520 sq. ft.) of tile that will require weekly sweeping & mopping services.

3.4. Floor Tile Strip & Wax. The Contractor shall provide one (1) time annual stripping and waxing of tiled surface floors in Bldg. 6670. Work includes the laboratories and shall be performed after the normal duty day. Bldg. 6670 has approximately 1,433 sq.

yd. (12,897 sq. ft.) of tile that will require annual strip & wax services.

Additional details are below:

3.4.1. Floors must be vacuumed to remove all small debris before stripping.

3.4.2. Equipment/furniture on wheels that are not plugged into a wall shall be moved for stripping and waxing underneath and replaced when finished.

3.4.3. Vacuum up stripper. Do not use floor drains.

3.4.4. Clean baseboards and door kick plates from stripping splatter.

3.4.5. A high quality traffic wax should be applied in four (4) coats.

4. Hours of Operation

4.1. Normal Hours for weekly cleaning. The Contractor is responsible for conducting weekly cleaning business outside normal hours of operation. Hours of operation are 6:00 A.M.- 4:00 P.M. Pacific Standard Time, Monday through Friday excluding recognized Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor is required to safely perform services outside normal hours of operation (standard hours) unless provided specific written authorization by the Contracting Officer to work Special Hours. The Contractor must submit a request and justification for working during lab operations to the Contracting Officer at least three

(3) working days in advance. Inclement weather, operational delays, and security related delays might affect the contractor’s work periods. The Contractor may be required to temporarily stop work for operational requirements, special events, installation exercises, real world activities, or while Distinguished Visitor (DV) familiarization visits/tours are conducted. Notification to the Contractor shall be provided by the Contracting Officer as required. The Contractor must make allowances for these events and resulting lost time will not be reimbursable.

4.2. Weekends/holidays for waxing and shampooing. The Contractor is responsible for stripping and waxing floors and shampooing carpets during weekend hours of non-operation between the hours of 4:00 P.M. Friday and 5:00 A.M. Monday Pacific Standard Time (PST), or including recognized Federal holidays. The Contractor may be required to temporarily stop work for operational requirements, special events, installation exercises, real world activities, or while Distinguished Visitor (DV) familiarization visits/tours are conducted. Notification to the Contractor shall be provided by the Contracting Officer as required. The Contractor must make allowances for these events and resulting lost time will not be reimbursable.

4.3. Recognized Federal Holidays. The Contractor is not required to perform services on recognized Federal holidays unless provided specific written authorization by the Contracting Officer to work Special Hours. The following is a list of recognized Federal holidays:

5. Environmental Requirements.

5.1. The Contractor shall adhere to all environmental requirements in addition to any other environmental coordination and laws applicable.

5.2. All materials to be removed and/or disposed of by the Contractor shall remain the property of the Government.

5.2.1. The Contractor shall make every effort to recycle materials generated by this effort to include but not limited to woods and metals. The Contractor shall contact the Vandenberg Recycling Center (805-605- 1143/0102) to coordinate disposal of Government Office Furniture, Wood, and Scrap Metal.

5.2.2. The Contractors shall coordinate with DLA Disposition Services

(previously “DRMO”) at 805-606-9852 for proper disposal of items.

6. Hazardous Material Requirements

6.1. The contractor shall obtain approval from the Government's Hazardous Material

Supply System (known as HAZMART), by processing an Air Force Form 3952, Chemical/Hazardous Material Request Authorization, for all of their hazardous materials with the HAZMART, 30 CES/CEIEC Environmental Support (Bldg. 5500 Bay C Rm 10), (805) 605-3870/ 7573, prior to using any hazardous materials on VSFB.

6.2. Contractor shall report their monthly hazardous materials usage for their activities on Vandenberg Space Force Base to the HAZMART. By reporting all of the monthly usage through the HAZMART, the contractor shall meet VSFBs reporting requirements for air emissions, EPCRA Toxic Release Inventory (TRI), solvent usage, and hazardous materials.

7. Government Furnished Items & Services

7.1. Equipment/Material. The Contractor shall provide all equipment, materials, and tools required to perform work.

7.2. Utilities. Electrical outlets are available in most areas. The facility has a janitor’s closet with a water faucet and sink available for use. Additionally, the facility has water faucets located on the exterior of the facility available for use (contact Facility Manager listed below for the key). The Contractor shall not change or modify any utility system or component; or connect any Government property, equipment or system without prior Contracting Officer review and written approval authorizing change/modification. The Contractor shall practice energy/resource conservation in its use of Government provided utilities.

7.3. Facilities. The Contractor shall provide its own portable toilet and sanitary facilities.

7.4. Security, Fire, and Medical Response Services. The Government shall provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis. Emergency services may be obtained by calling 9-1-1 from a base phone or 805- 606-3911 on a cellular phone. The Government will provide initial medical treatment for life-threatening emergencies. It shall be the responsibility of the Contractor or Contractor personnel to seek continued medical treatment if needed. The Contractor or employee shall reimburse the Government for all Government-provided medical services.

8. Applicable Documents/Permits. The Contractor shall identify and comply with all applicable Federal, State, and local statutes; Air Force/ Military instructions, manuals, handbooks, regulations, guidance, and policy letters; Executive Orders; American Petroleum Institute Codes; National Association of Corrosion Engineers; National Fire Protection; Steel Structures and Painting Council; National Electrical Code; Uniform Fire Code; Occupational Safety and Health Administration (OSHA) regulations; Environmental Protection Agency regulations; and International Building Code, including all changes and amendments in effect on the date of issuance of each contract. It is the Contractor’s responsibility to identify and comply with all applicable and relevant requirements. It is the responsibility of the Contractor to identify all permits, clearances and other documents required for this project.

9. Security Requirements

9.1. At all times, while on VSFB premises, Contractor personnel shall comply with VSFB security requirements imposed by the Installation Commander. The Contractor shall obtain all necessary passes, decals, badges, or other items required for access to perform contract services at VSFB. The Contractor shall be responsible for accountability of all such media issued to Contractor personnel and these media shall be surrendered to the 30th Security Forces Squadron (30 SFS) Pass and Registration Section upon completion or termination of the contract, or upon termination of an individual’s employment with the Contractor. The Contractor shall comply with all VSFB policies regarding entry to the installation, rules of the road, contingency operations compliance, and photography prohibitions. Contractor personnel are authorized entry to the installation only for the purpose of work under this contract, and are not authorized in other facilities or areas without prior Government authorization. Due to safety/security requirements, Contractor personnel shall be required to fluently speak, understand, read, and write English.

9.2. Main Base Access. Entry requirements are subject to change, without notice due to installation and world situations. For base access all contractor personnel are subject to a background check through the Federal Bureau of Investigation. Current security access restrictions at the Santa Maria Gate (Main Base Entry Gate) require all Contractor vehicles (which are the size of a standard bread delivery truck or larger) to enter only through the Lompoc Gate (located at intersection of Santa Lucia Road and Pine Canyon Road). There is a vehicle check pull-off area at that gate and the Contractor will be required to submit to a vehicle search prior to gaining entry to the base. Contractor should plan and allow for delays as a result of in-place security procedures.

9.2.1. The Contractor shall coordinate access to facilities with the

AFPET/PTPLE POCs.

9.2.1.1. The Contractor shall brief all employees that they shall immediately leave any area upon notification by the Contracting Officer, constituted authority, or the Security Forces, when directed as occasional operational requirements, real world activities, or emergencies dictate.

9.2.1.2. The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the 30th Security Forces Squadron (30 SFS) as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. Due to safety/security, all Contractor personnel shall be able to fluently speak, read, and write English.

9.2.2. Contractor Vehicles. Each Contractor vehicle shall have the name of the

Contractor exhibited on each side of the vehicle. The Contractor shall provide company identification tags and vehicle identification at no additional cost to the Government.

9.2.3. Physical Security. The Contractor shall be responsible for safeguarding all Government equipment, information and property provided for Contractor use.

10. Contractor Personnel.

10.1. Contractor personnel shall present a neat appearance and be easily recognizable. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee name.

10.2. Contractor personnel shall comply, and ensure that Sub-Contractor personnel comply, with current Federal, state, local, Department of Defense (DoD), and Air Force regulations, plans, and policies including all applicable parts of the Air Force Occupational Safety and Health Standard (AFOSH) and United States Department of Labor Occupational Safety and Health Administration (OSHA).

10.3. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with (IAW) DoD 5500.7-R, "Joint Ethics Regulation (JER)", "Standards of Conduct," Dated August1993.

11. Contractor Manpower Reporting

11.1. The Contractor shall report ALL Contractor labor hours (including Subcontractor labor hours) required for performance of services provided under this via a secure data collection site.

11.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 01 October through 30 September.

While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year.

12. Invoicing. The Contractor shall invoice electronically to the Defense Finance Accounting

Service (DFAS) utilizing the Wide Area Work Flow (WAWF) application on the Procurement Integrated Enterprise Environment website (www.wawf.eb.mil) within five (5) working days of completion of work. The Contractor shall notify the AFPET/PTPLE POC identified in the WAWF clause of the contract upon submittal of invoice for processing. The Contractor shall provide supplemental documentation with the invoice. Supplemental files shall include work orders issued and other documentation that was generated for each task (inspection records). Supplemental files are used to validate invoice pricing.
