SECTION 1
DESCRIPTION OF SERVICES
1. OBJECTIVE:
The Contractor shall provide management and labor to of the Kadena Air Base Airman and
Family Readiness Center (A&FRC) as set forth in this Performance Work Statement (PWS). The purpose is to conduct and maintain data entry and client services for the Transition Assistance
Program (TAP). These duties are to be performed in the Airman and Family Readiness Center
(A&FRC), or any location the A&FRC conducts transition services.
NON-PERSONAL SERVICE. All services requested under this PWS are non-personal in nature. The contractor is solely liable for the actions of their employee. Hence, the contractor employees performing the service under this contract shall be controlled, directed, and supervised by management personnel of the contractor. Contractor employees will perform their duties independent of, and without supervision of, any Government official. The tasks, duties, and responsibilities set forth in the contract may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal Policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal service to any Federal employee. The Government will control access to the facility and will perform the inspection and acceptance of the completed work.
SERVICES DESCRIPTION:
1.1. Contractor shall receive telephone calls, greet visitors, ascertain the nature of the call or visit, direct clients to the appropriate program manager in the A&FRC, schedule clients for
A&FRC classes and provide complete and clear information that addresses most routine or procedural issues.
1.1.1. Administers the timely and accurate input of congressionally mandated documentation for the Transition Assistance Program. Responsible for the accurate collection, input, inventory, maintenance and tracking of all forms within the Transition Assistance Program
1.1.2. In coordination with the TAP Core Compliance Expert, the contractor reviews the attendance registration for transition assistance workshops, creates and disseminates pertinent information and inquiries to program participants for the collection and entry of data for statistical reports and transition workshop registration. Accurately inputs customer information into the Air Force Family Integrated Reporting and Statistical Tracking (AFFIRST) system.
1.1.3. The Contractor shall maintain the appearance of the Front Desk/TAP Resource Center.
The Contractor will align chairs, keyboards, materials, publications, and resources on a daily basis. The Contractor shall report inoperable equipment to A&FRC management.
1.1.4. Provide technical assistance to users of the TAP Resource Center, assisting in the operation of computer-based programs, as specified in requirements below, and other programs available to transitioning personnel. Serve as initial point of contact for all transition related software, hardware, and user problems to include network issues. If the contractor cannot resolve the problem he/she should contact the Work Group Manager (WGM) for assistance.
1.1.5. Ensure workstations and network software remains virus free. Review and submit locally developed software to the WGM. Will not install or modify any equipment or software without prior approval from WGM. This includes any software downloaded from the World Wide Web
(www) or vendor bulletin board systems (BBS).
1.1.5. Maintain network integrity. Permits only authorized users to access the A&FRC Customer
Wi-Fi and Internet Service Provider (ISP) accounts.
1.1.6. Maintain system documentation (maintains memory and application configuration;
maintains network schema, checklists, and systems operations logbook.
1.1.7. Control communication access to the wide area network. Perform routine system maintenance and weekly system backups.
1.1.8. Ensure maintenance of accountability statistics on resources used by clients; develop charts, logs, and statistical summaries as directed by the A&FRC management.
1.1.9. Ensure all clients completely fill out sign-in log and generate a monthly client usage report in the format established by A&FRC management not later than the fifth working day of the next month.
1.1.9. Maintain computer security at all times to include the following:
a) TAP Resource Center clients only access authorized programs using the main menu.
b) Access on-line programs under the direct supervision of the contractor or TAP.
c) Document /report all computer software/hardware malfunctions or abuse to the WGM.
1.2. CONTROLS OVER WORK: Oversight to be provided by Chief, Airman & Family
Readiness Center or designee.
1.3. OTHER SIGNIFICANT FACTORS/QUALIFICATIONS: Contractor must have a background in, and working knowledge of:
1.3.1. Must be proficient in applying and troubleshooting Microsoft Office Suite applications to include MS Windows, MS Word, MS excel, MS PowerPoint and must quickly become proficient on other available resource software in order to maintain integrated reports using Microsoft
Word with Excel spreadsheet and data analysis.
1.3.2. Basic communication skills appropriate in the professional environment of the A&FRC.
Therefore, a minimum of 12 college credit hours in social work or social services is required; or a minimum of one-year experience working in an Air Force A&FRC or a similar helping service agency environment will be a suitable substitute.
1.3.3. Basic understanding of military protocol, support group organizations, understanding of family systems in a military setting, ability to communicate, and work effectively with diverse groups.
1.3.4. Speak, write and understand the English language fluently. Provide certificate to confirm
English proficiency level.
1.3.5. Gain competency of duties within 30 calendar days
1.3.6. Contractor shall dress in appropriate business attire while in performance of contract.
Inappropriate attire includes but is not limited to jeans, leggings, sweats, skirts shorter than 2” above the knee, exposed midriff, spaghetti straps, sheer clothing, low cut tops, shorts, flip flops and thong style footwear, slippers, sleep wear, bib overalls, short shorts, tops with potentially offensive words, terms, logo’s, pictures, cartoons or slogans, halter tops, tops with bare shoulders, t-shirts unless worn under another blouse. Jewelry should be in good taste with limited visible body piercings. Make-up, cologne and perfume should be in good taste and not over powering. Many customers are allergic to the chemicals in perfumes and make-up so these substances should be worn with restraint. Hats are not appropriate attire however head covers that are required for religious purposes or to honor cultural traditional are allowed.

SECTION IV
GENERAL INFORMATION
4. HOURS OF WORK:
4.1. Services will be performed Monday through Friday, except US holidays from 0800-1700 hrs. Lunch will be no more than one hour and coordinated with appropriate personnel, or as otherwise determined by supervisor, in the A&FRC, or other assigned location. When contractor is required to work beyond an 8 hour day/40 hour week, due to fulfilling requirements such as evening classes or evening hours, time will be compensated as determined by appropriate supervisor.
Contractor shall notify the Chief of A&FRC and the Contracting Officers Representative (COR) at least two weeks in advance for any planned absence(s). Hours may be adjusted with prior approval from designated representative in advance of work performed at which time there will be an adjustment to the duty day.
In the event that the absence is greater than fourteen (14) calendar days the contractor shall provide a suitable substitute of equal qualifications at no additional cost to the government.
Approved vacations shall not exceed more than fourteen (14) consecutive calendar days at one time. All requests for absence shall be approved by the Chief of A&FRC.
The Contractor shall submit a detailed and itemized monthly invoice reflecting all hours worked to the COR or designated representative. In the event of absence, the invoiced amount shall reflect the absence accordingly.
4.2. FEDERAL HOLIDAYS: The contractor is not required to perform service on Federal
Holidays. When the holiday falls on a Saturday, the preceding Friday will be the federal holiday.
When the holiday falls on a Sunday, the following Monday will be the federal holiday. Asterisk
(*) indicates the day of the week change each year. The Wing Commander sometimes declares
“family days” or other days for base personnel to take a holiday. These holidays are not chargeable to the government if the schedule is not worked.

4.3. SECURITY REQUIREMENTS:
4.3.1. Security Training. All contractor employees shall receive initial and recurring security education training from the sponsoring agency’s security manager. Training must be conducted in accordance with DoD 5200.1-R, Information Security Program, and AFI 31-401, Information
Security Program Management.
4.3.2. E-Mail Access. In accordance with AFI 33-119, Electronic Mail (E-mail) Management and Use, paragraph 9.2.5., DoD military, civilians, or contractor personnel using unclassified automated information systems that have access to sensitive information must possess, at a minimum, a National Agency Check or Entrance National Agency Check in accordance with
DoD 5200.2-R, Personnel Security Program, or screening in accordance with AFI 31-501, Personnel Security Program Management. Those personnel requiring access to classified systems are subject to the appropriate investigation.
4.3.3. Physical Security. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.
4.3.4. Pass And Identification Items. The contractor shall ensure the pass and identification items required for the contract performance are obtained.
4.3.5. Retrieving Identification Media. The contractor shall return all identification media prior to contract expiration or termination of service.
4.3.6. Traffic Laws. The contractor shall comply with base traffic regulations.
4.3.7. Weapons Firearms and Ammunitions. Contract employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned vehicle or privately owned vehicle while on Kadena Air Base (AB).
4.3.8. Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.
4.3.8.1. The contractor shall immediately report to the Contracting Officer’s Representative
(COR) or Program Manager any occurrences of lost or duplicated keys.
4.3.8.2. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, as its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.
4.4. CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING
CRISIS IAW DoDI 3020.37.
IAW AFI 63-124, para 2.7.1, and DODI 3020.37 this requirement is not considered mission essential.
4.5. CONTRACTOR PERSONNEL: The contractor shall have knowledge of the A&FRC
Resource Center functions, and Microsoft Office Professional Suite.
4.6. JAPANESE ROAD LAWS
4.6.1. The contractor shall comply with Japanese Road Laws while on US Military installations to specifically include: Article 43 The Road Law (Japan) (prohibited acts relative to roads). No person shall commit the following described acts:
4.6.1.1. Damaging the road, causing roads to be littered with debris, polluting the roads with debris or fall out from motor vehicles or otherwise defacing roads unreasonably or without due cause.
4.6.1.2. Article 43-2 (Measures to be taken to prevent cargo or things loaded on motor vehicles from falling out)
4.6.2. The agency managing a road may when there is reasonable cause for him to feel that items or cargo being carried in or on a motor vehicle may fall out and damage, pollute, or otherwise deface the road in such manner as to hinder or obstruct the traffic, order the operator of the vehicle in question to take steps or measures necessary to prevent occurrence of such hindrance by stopping operation of the vehicle, or correcting the method of loading or traveling or operation of the vehicle.
4.7 REQUIRED INSURANCE (In addition to that required by Japanese Law)
4.7.1 The Contractor shall procure and maintain during the entire period of his performance under this contract the following minimum insurance on each vehicle used by the Contractor at the work site.
4.7.1.1 Type: Automobile Property Damage Insurance Amount: ¥3,000,000 or Dollar equivalent Bodily Injury Insurance Amount: ¥30,000,000 or Dollar equivalent
4.7.1.2 All vehicles must be properly inspected/insured in accordance with Japanese
Compulsory Insurance (JCI) requirements.
4.7.2 Prior to the commencement of work hereunder, the Contractor shall furnish to the
Contracting Officer a certificate or written statement of the above required insurance. The policies evidencing required insurance shall contain an endorsement to the effect that cancellation or any material change in the policies adversely affecting the interests of the U.S.
Government in such insurance shall not be effective for such period as may be prescribed by the laws of the state in which this contract is to be performed and in no event less than thirty (30) days after written notice thereof to the Contracting Officer.
4.8 SUBMISSION OF INVOICES
4.8.1. Submission of Invoices. The contractor shall submit invoices through Wide Area
Workflow (WAWF) IAW FAR clause 252.232-7003 Electronic Submission of Payment
Requests and Receiving Reports.
