STATEMENT OF WORK FOR
Delivery, Installation and Disposal of Technical Training Replacement Furnishings For Dormitory 10540 1435 Metzger St, JBSA Lackland, Texas 78236
1. GENERAL INFORMATION. The Contractor shall provide all transportation, supplies, materials, tools, equipment, and labor necessary to complete the delivery and installation of new furniture and for the disposal of all existing wooden furniture only being replaced for the Technical Training Dormitory located in building 10540 on JBSA Lackland, TX. Installation, delivery and removal is required for 56 rooms located on three levels within the facility. The facility is occupied and contractor shall be expected to work with the Government for the best method on mitigating disturbance to the occupants in this facility.
2. DESCRIPTION OF SERVICES. The contractor shall furnish all labor, supervision, materials, incidentals, equipment, storage, assembly, expertise, and transportation necessary to perform the installation of new furniture and for the disposal of the existing furniture being replaced as contained in this performance work statement and solicitation documents. Contractor is required to provide dumpsters for the disposal of the existing furniture. The Government shall not provide any services or materials, tools or equipment necessary for the completion of this project. The contractor upon completion of install/work shall ensure 100% operation of all items with 502 CES.
3. SALIENT CHARACTERISTICS.
a. Twin Wood Beds - Stackable bed, 39 to 42 inch Width x 80 to 85 inch Length x 33 to 35 inch Height, Steel spring deck with head and foot boards Oak hardwood and oak finish for head and foot boards.
b. XL Twin Mattresses - Dimensions for all twin mattresses shall be 38 inch Width x 80 inch Length and 9 to 10 inch Depth and made of Nylon Material and dark blue in color. Mattress shall fit Twin Wood Bed appropriately.
c. Student Pedestal Desks - Length: 42 to 46 inch, Depth: 22 to 24 inch, Height: 30 to 33 inch. Description: Single pedestal desk has one pencil drawer, one box and box file pedestal. Oak hardwood with natural oak finish with recessed pulls and NO HARDWARE.
d. Wood Two Position Desk Chairs - 18 inch w x 22-1/2 inch D, x 32 inch H Sled base, armless, light colored upholstered/fabric seat and back material with oak wood frame.
e. Nightstands - Width 23 inch, Depth: 16 to 23 inch, Height: 22 to 24 inch. Oak hardwood and oak finish with recessed Pulls NO HARDWARE.
f. Cook N Cool Microwave and Refrigerator Combination
- Refrigerator- Black, 10.3 cu. Ft. Total Capacity with 7.9 cu. Ft Refrigerator and 2.4 cu. ft. Freezer with frost free operation; Refrigerator Freezer sealed system and hermetic compressor. Shall include Standard Manufacturer Warranty on all refrigerator and freezer mechanical components with two year Warranty, ENERGY STAR qualified performance saves on yearly running costs designed to meet ADA guidelines. Slim 24 inch width offers over 10 cu. ft. of interior storage. 2 inch variance on dimensions is acceptable
- Microwave with Internal Power Allocator- Black with Capacity/Cooking Watts 0.9 cu. ft. 900 Watts, Control type Electronic Touch Pads, Turntable with 11 inch Diameter Glass, Depth to Edge 7 inch to Front of Refrigerator. ½ inch variance on dimensions is acceptable
g. Three Drawer Chest- Solid wood with natural oak finish and or High Pressure Laminate tops, oak edges, plywood drawer fronts with solid oak overlay drawer pulls, oak veneer core plywood construction. Recessed or routed in handles pulls with NO HARDWARE METAL HANDLES. 32 inch Width x 24 inch Depth x 30 inch Height. 2 inch variance on dimensions is acceptable
3. DELIVERY/INSTALLATION INSTRUCTIONS. Building 10540 is a three story dormitory facility with a single external balcony to individual rooms on each floor. All furnishings shall be delivered and installed only in sixteen rooms located on the first level, twenty rooms located on the second level, and twenty rooms located on the third floor. Installation of furnishings will be completed one level at a time, with no less than two days before starting install on level two after completion of level one, and no less than two days before starting install on level three after completing install on level two, to accommodate moving of Airmen from occupied rooms from level two and three of the building. Installation will start with sixteen rooms on level one of building 10540 first, after complete install of furnishings on level one, contractor will then proceed to complete install of furnishings in twenty rooms on level two, with final install of furnishings to twenty rooms on level three. Please note that contractor shall complete level one prior to starting on level two, and then shall complete level two prior to starting installation on level three. The primary government point of contact shall ensure all rooms prior to delivery are free of any old or previously stored furnishings or internal debris before delivery of any new furnishing on the day of delivery. Note: Access to building 10540 second and third level is by stairwell only and DO NOT HAVE ELEVATOR ACCESS.
4. REMOVAL and DISPOSAL INSTRUCTIONS. Building 10540 requires the disposal of existing furniture. As noted in paragraph 3 above, the building is three levels which are only accessed by stairwell (there are NO elevators). The contractor shall provide all transportation, supplies, materials, tools, equipment, dumpsters, labor and another other cost associated for the removal and disposal of the items listed below:

5. GENERAL SERVICES For the contractor coordination and management of all new product, equipment, and personnel required to deliver, and install new furniture at JBSA-Lackland, TX, Dormitory 10540.
5.1 The contractor shall be responsible for all personnel, equipment, tools, materials, supervision, storage, assembly, transportation, incidentals, and all other items and services necessary to provide and install the furniture. Contractor shall be responsible for all personnel, equipment, tools, materials, supervision, transportation and any incidentals to dispose of existing furniture (including dumpsters).
5.2 The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, codes, permits, DoD, Air Force, and installation requirements and instructions required for the performance of the duties in this Statement of Work (SOW).
5.3 The contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental risks. These policies shall be in compliance with Federal and installation policies and procedures.
5.4 The contract shall follow CDC and JBSA COVID-19 guidelines that require social distancing, wearing personal masks, and sanitization, to include both personal hygiene and surface disinfection.
5.5 The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate or alternates who shall act for the contractor when the manager is absent shall be designated in writing to the Government point of contact within ten (10) business days after contract award. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to the daily operations of this contract on the installation.
5.6 All contractors shall submit a complete list of personnel in accordance with installation policy, including deliverymen, who will be expected to work on the installation. This list will contain Privacy Act information as required, as well as the expectation that each individual will be able to present a current and valid state identification, to meet local Air Force security requirements.
5.7 The contractor shall not employ persons for work on this contract if such employee is identified to the contractor or by the CO as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily recognized as a contractor employees.
5.8 The contractor shall manage ordering, shipment, delivery and installation of all new furniture from manufacturer to final acceptance by the designated Government POC.
6. QUALITY CONTROL. The Contractor shall develop and maintain a quality control program to ensure installation and services are performed in accordance with the Technical Order and the manufacturer’s instructions. The contractor shall develop and implement procedures to identify and prevent defective services from reoccurring.
7. QUALITY ASSURANCE. If any discrepancies are identified by the installers or by the 502 CES it will be reported to the 502d Contracting Specialist and the Contracting Officer.
8. PROJECT MANAGEMENT. The contractor shall identify and provide a single point of contact (POC) to provide overall management and supervision during the term of this project. The single POC shall serve as representative for the customer representatives to ensure the delivery of all furnishing awarded. It is necessary to ensure all furnishings are successfully delivered on time with active communications with the assigned points of contact throughout due to personnel living throughout the dormitory in building 10540. All pertinent communications should be written and or confirmed in writing to ensure 100 percent compliance.

9. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (Oct 2018), for contractor’s failure to correct nonconforming services.
10. HOURS OF OPERATION. The contractor shall perform all work during the hours of 7:30 A.M. to 4:30 P. M., Monday through Friday and weekends from 7:30 A. M. to 4:30 P.M. The contractor is not required to perform work during federal holidays. If any work needs to be done after hours the contractor needs to notify contracting official in charge. Changes to the schedule shall be immediately identified to the Contracting Official and 502 CES.
11. HOLIDAYS OBSERVED. Recognized US Federal Holidays are as follows: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas. If the holiday falls on a Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
12. ENVIRONMENTAL COMPLIANCE. The contractor shall comply with all applicable federal, state, and local environmental laws and regulations. The 902 CES/CEF shall not use any ozone depleting substances or products.
13. CLEAN-UP. The contractor shall clean up and dispose of waste materials or rubbish in accordance with this paragraph and adhere to all applicable federal, state & local regulations, standards, codes & laws. Unless directed otherwise, the contractor shall remove all packing materials, debris, and cartons from the project site to include any defective product or material that has been delivered to the site in error. Do not dispose of or discard recyclables, packing materials or trash in any recycling or trash in containers or dumpsters located on/throughout the military installation. However, the contractor is permitted to bring their own dumpster on base with proper access granted prior to award for disposal of any trash as long as the dumpster is removed upon completion of the delivery and or completed delivery and installation project.
14. PROTECTION OF BUILDING & PROPERTY. Where necessary, the contractor shall provide all material to protect all floors, carpeted areas, walls, carpeting from damage and soil during the moving and installation. The protective materials shall not be bolted, glued, or stapled to floors, carpeted areas, walls, ceilings, or doors but instead shall either be free standing or braced so as not to cause any damage. Proper measures (protective runners, pads, etc.) shall be provided and utilized by the contractor to protect the building, stairs, stairwells, walls, floors, etc. The contractor shall establish and maintain this protection and maintain it throughout the full term of the purchase order. The contractor shall furnish furniture padding or other acceptable protective materials for the interior of the building, including, door frames, thresholds, and corners to which the property shall be moved under this purchase order. Any damage caused by the contractor shall be the responsibility of the contractor to repair or replace immediately.at no additional cost to the Government.
15. SECURITY REQUIREMENTS.
15.1. The Contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted 3 days prior to delivery to the below POC’s.

16. CONTRACTOR/EMPLOYEE BASE PASS
16.1. As prescribed by the AFFARS 5352.242-9000, Contractor Access to Air Force Installations the contractor shall comply with the following requirements:
16.2. The contractor shall obtain base identification and vehicle passes for all contractor personnel who make frequent visits to or perform work on the Joint Base installation(s) cited in the contract.
16.3. No later than three (3) days prior to start of contract work (can take up to ten (10) days to obtain a base pass), the contractor shall submit a written request on company letterhead (or e-mail correspondence) to the CO with copy to contract administrator listing the following: project title, project number, contract number, location of work site, contract start and stop dates, work schedule, and names of contractor employees needing access to the base, including full legal name, date of birth, state issued I.D. Card/Driver’s License #, and name of sub-contractor if not employed directly by contractor (Government will provide template to be used in submitting visitor pass request). The authorized CO will endorse the request and forward it to the issuing base pass and registration office or security forces for processing. Contractors will present government (state or federal) issued ID, and INS Form 9 (I9), before being issued a pass to enter the installation. A Wants and Warrants check is conducted for every person before a pass is issued. Personnel employed by the contractor must get a pass for their privately owned vehicles with proof of following:
a. Liability Insurance
b. Current License Plates
c. Current State Inspection Sticker (If Required)
d. Valid State Driver License
e. A phone number for sponsor on base
16.4. Vehicles owned by the contractor with the company name permanently printed on them are not required to obtain a pass as long as a current work order is presented at the time of entry. However, current liability insurance, state inspection sticker, and registration is required. The person driving the vehicle must have a valid operator license for the type of vehicle.
16.5. The contractor is responsible for ensuring employees present their Form I-9 (Employment Eligibility Verification).
16.6. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to contractor employees are returned to the issuing office. If a contractor employee has been terminated, the pass will need to be retrieved and returned to Security Forces (SF) so that employee does not have base access. If the pass is not retrieved then SF will need to be notified so base access is not allowed. Upon completion of the contract return all issued installation access passes to the issuing authority.
16.7. Failure to comply with these requirements may result in withholding of final payment.
17. SPECIAL CRITERIA. The Contractor must conduct a final inspection and acceptance of all work performed, reports, and other deliverables. The contractor shall coordinate with 502 CES on final inspection and acceptance. Payment will not be made until requirement is completed and accepted by the government.
