Amendment 3 AHRQ-12-10009 Evidence-based Practice Centers (EPCs) IV
Amendment 3:
Solicitation AHRQ-12-10009 is amended as set forth below:
Page 79 Section L.7 General Instructions (b) II. is revised to read:
II. PAST PERFORMANCE INFORMATION: See Past Performance
Information Instructions for format (L.9). Submit Original, (clearly marked) , 2 copies and 2 CDs.
Page 81 Section L.8 Technical Proposal Instructions opening paragraph is revised to read:
The technical proposal shall be comprised of 2 Sections. Section 1 for the EPC IV program described below shall be limited to 25 pages not including resumes/CVs and Letters of Intent. The 25 page limit includes all other portions of the technical proposal Section 2, for Task Order 1 described below shall be limited to 15 pages. (…the remainder of the paragraph remains unchanged.)
Page 86 Section L.9 Past Performance first paragraph and (a) are revised to read:
Offerors shall submit the following information as part of their proposal for both the offeror and proposed major subcontractors:
(a) A list of five (5) contracts and subcontracts completed during the past three years or currently in process that are similar in nature to the solicitation workscope. (…the remainder of the paragraph remains unchanged.)
Page 87 Section L.9 Past Performance (b) is revised to read:
The offeror may provide information on problems encountered on the contracts and subcontracts and corrective actions taken to resolve those problems. Offerors should not provide general information on their performance on the identified contracts.
General performance information will be obtained from the references.
Page 87 the last paragraph on is revised to read:
The attached Past Performance Questionnaire and Contractor Performance Form shall be completed by the offeror’s customers and forwarded directly to the following:
Page 93. Under “Technical Approach”, the criteria “Development of a sound approach to managing conflicts of interest, for their own institution as well as for clinical expertise or external stakeholders” should be deleted.
Page 94. Under “Key Personnel and Core Team,” the criteria “Demonstrated ability to adapt to changing needs of the science, literature, Partner and/or AHRQ; or to adapt to other unexpected events” should be deleted.
Page 94. Criteria “Development of a sound approach to managing conflicts of interest, for their own institution as well as for clinical expertise or external stakeholders” should be added under “Key Personnel and Core Team.”

Page 12 prior to the “Examples of EPC products” section, this paragraph may be inserted:
The term ―technology assessment‖ specifically refers to reports conducted under the Technology Assessment Program. Technical briefs are a specific type of report conducted under the Effective Health Care Program as described in the preface of several examples provided via the website –
A Technical Brief provides an overview of key issues related to a clinical intervention or healthcare service—for example, current indications for the intervention, relevant patient population and subgroups of interest, outcomes measured, and contextual factors that may affect decisions regarding the intervention. Technical Briefs generally focus on interventions for which there are limited published data and too few completed protocol-driven studies to support definitive conclusions. The emphasis, therefore, is on providing an early objective description of the state of science, a potential framework for assessing the applications and implications of the new interventions, a summary of ongoing research, and information on future research needs.
Page 12: Headings under “Examples of EPC Products” should read:
– CERs, systematic reviews and technical briefs
– Future research needs
– Advanced Systematic Reviews for USPSTF
– Abbreviated, Focused Reviews (Technology Assessments) for TA Program:
Attachments:
Attachment with the header: “Attachment 4 Sample formant for cost proposal”, the footer is revised to read Attachment 4.
Attachment 6/Task Order 1 Section 4.C.3. of Attachment 6 is revised to read:
Travel to conferences to present findings from AHRQ supported EPC reports, with prior approval from Task Order Officer (TOO). The conference title, time, place, agenda, and sponsor must be provided to the TOO for approval prior to submission of abstracts. Approval of travel will be contingent on match of the conference to program needs.
