C - SECTION C - Description/Specifications/Statement of Work
C.1 DESCRIPTION OF WORK
Repair split stringer on Lower Beasore Creek Bridge. The work consists of simultaneously jacking up the west end of the bridge and temporarily shore. Repair girder by attaching new treated timbers on both sides of split with specified hardware, place new blocking through-out bridge, place bridge back on to abutment while maintaining traffic control.
C.2 APPLICABLE SPECIFICATIONS AND DRAWINGS
In addition to the specifications which are part of the base contract, the following specifications and drawings apply:
a. Special Project Specification (SPS) 557 Timber Structures
b. Project Plans, drawing sheets 1-3.
c. Sierra NF Fire Plan dated August 2012.
A detailed “Work Schedule Plan” and “Traffic Plan” will be required prior to start of work.
C.3 PROJECT LOCATIONS
The project is located on the Bass Lake Ranger District of the Sierra National Forest. From Fresno, CA. proceed north on state highway 41 approximately 40 miles through the community of Oakhurst, CA. to the intersection of County Road 274, then proceed southeast on County Road 274 approximately 5 miles to the intersection of Forest Road 5S07 (Beasore Road), then proceed north on Forest Road 5S07 approximately 20 miles to the intersection of 5S15, proceed on 5S15 approximately 1000 feet to the Lower Beasore Bridge and project site.
C.4 CONTRACT TIME
Estimated start work date is August, 2015. The contract time will be 60 days. Contract time includes delays for weather; locating and transporting supplies and materials; and final site cleanup.
C.5 PROJECT SITE CLEAN UP
Project site cleanup includes the removal and proper disposal of all construction trash, debris, and residue off of National Forest Land.
All spills of petroleum products including leak spots from parked vehicles shall be removed and properly disposed according to all Local, State, and Federal rules, regulations, and laws governing such disposal.
During the Work the Contractor is required to furnish dumpsters or other trash receptacles and to empty such containers either as they become full or weekly, whichever is more frequent. The Contractor shall not place materials of any kind into private or Government trash bins or dumpsters.
C.6 PRICE RANGE
The price range of this project is less than $25,000.
C.7 SPECIAL CONDITIONS AFFECTING THE WORK
a. Physical Conditions. The indications of physical conditions on the drawings and in the specifications are the result of site investigations and surveys performed by the Government.
b. Weather. Low temperatures at the project sites range from temperatures of less than 20 ºF in December and January to lows of 65 ºF in July and August. High temperatures range from 45 ºF in December and January to over 90 ºF in July and August. The annual precipitation at the project sites varies from less than 12 inches per year including snow fall in the valley to several feet of snow in the mountains. Winter snow storms and freezing temperatures will occur during the winter months that result in a winter project shutdown at higher elevations. Icy conditions on local roads and highways can occur during the fall, winter and spring. Highway curves in shaded areas will remain covered with ice or frost for weeks.
c. Work Hours. Work hours are from 7:00 am to 5:00 pm Monday through Friday.
Weekend equipment work is not allowed at any time during the Work. Dogs and pets are not allowed on the project site at any time.
d. Safety, Emergency Communications and Evacuation. Contractor is responsible for the safety of the Contractor’s employees including Subcontractors employees, Government employees associated with the project, and any visitors to the site both Government and private. The Contractor shall submit a project safety plan to the Contracting Officer for review prior to commencement of Work. The project safety plan shall comply with the appropriate California OSHA and Federal OSHA rules and regulations and must include a medical evacuation and emergency communications plan.
e. Highway Permits. Contractor is responsible for securing any overload or over width limit permits required for the transport of materials or equipment to the project site. The Contractor is responsible for conducting a thorough reconnaissance of all routes leading to the site.
f. Toilets. No toilet or shower facilities are available on site. Contractor shall provide toilet facilities for its employees.
g. Camping. Contractor may use a self-contained travel trailer or recreational vehicle to camp on National Forest System lands near the project sites subject to the approval of the Contracting Officer. All waste material must be transported off-site to a station equipped to handle such waste.
h. Security. Contractor is responsible for the Contractor’s equipment and materials supplied to the site. All equipment not equipped with a key lock system shall be secured in such a manner as to make it inoperable to the average person at the end of each working day. All hand tools and other equipment shall be secured at the end of each working day. On Fridays all construction debris, trash, and refuse shall be properly stored in dumpsters or trash bins or removed from the site.
i. Burning Permits. Contractor is subject to all rules and regulations governing the on site burning of slash and other debris. Contractor shall obtain a burning permit from the jurisdiction having control over burning. The Contractor shall comply with Fire Plan for Construction and Service Contracts included in the Contract.
j. Protection of Resources. Contractor shall thoroughly clean all equipment prior to delivery to the project site to remove weeds, mud, grease and oil buildup from the tires, wheels, tracks, engines and engine compartments, and other surfaces. Spills of petroleum products and other hazardous substances shall be reported, contained, and removed in accordance with all applicable rules, regulations and laws. Contractor shall submit a project spill response plan at the pre-work meeting for review by the Contracting Officer.
C.8 EQUIPMENT CLEANING
Unless the entire project area is already infected with noxious weeds, the contractor shall ensure that prior to moving on to another part of the project area all off-road equipment, which last operated in areas known by Forest Service to be infected with noxious weeds, is free of soil, seeds, vegetative matter, or other debris that could contain or hold seeds. Contractor shall certify in writing that off-road equipment is free of noxious weeds prior to each start-up of operations and for subsequent moves of equipment to other project areas. Measures taken to ensure that off-road equipment is free of noxious weeds will be identified. “Off-road equipment” includes all equipment operating off of paved roadways. A Current list of noxious weeds of concern to Forest Service is available at the Forest Supervisor’s office. Contractor shall clean off-road equipment prior to moving between project areas with known infestations with noxious weeds and other project areas, if any, which are free of such weeds. Project area maps show areas known by Forest Service that contain specific noxious weeds species of concern. Contractor shall employ whatever cleaning methods are necessary to ensure that off-road equipment is free of noxious weeds. Equipment shall be considered free of soil, seeds, and other such debris when a visual inspection does not disclose such material. Disassembly of equipment components or specialized inspection tools is not required. Contractor shall notify Forest Service at least 24 hours prior to beginning of work in area designated as a noxious weed area, unless otherwise agreed.
Notification shall include the location of the equipment’s most recent operations. If the prior location of the off-road equipment cannot be identified, forest service may assume that it was infested with noxious weed seeds. Upon request of Forest Service, Contractor must arrange for forest Service to inspect each piece of off-road equipment prior to it being placed into service. If Contractor desires to clean off-road equipment on national forest land, such as at the end of a project or prior to moving to a new unit that is free from noxious weeds, Contractor and Forest Service shall agree on methods of cleaning, locations for the cleaning, and control of off-site impacts, if any. New infestations of noxious weeds, of concern to Forest Service and identified by either Contractor or Forest Service, within the project area or on the haul route, shall be promptly reported to the other party.

The Contractor shall furnish the necessary personnel, material, equipment, services and facilities (except as otherwise specified), to perform the Statement of Work/Specifications referenced in Section J.

SECTION F - Deliveries or Performance
F.1 52.211-10 Commencement, Prosecution, and Completion of Work. (APR 1984)
The Contractor shall be required to (a) commence work under this contract within 7 calendar days after the date the Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than 60 calendar days from the effective date on the Notice to Proceed.
The time stated for completion shall include final cleanup of the premises.
F.2 FAR 52.242-14 Suspension of Work. (APR 1984)
(a) The Contracting Officer may order the Contractor, in writing, to suspend, delay, or interrupt all or any part of the work of this contract for the period of time that the Contracting Officer determines appropriate for the convenience of the Government.
(b) If the performance of all or any part of the work is, for an unreasonable period of time, suspended, delayed, or interrupted (1) by an act of the Contracting Officer in the administration of this contract, or (2) by the Contracting Officer's failure to act within the time specified in this contract (or within a reasonable time if not specified), an adjustment shall be made for any increase in the cost of performance of this contract (excluding profit) necessarily caused by the unreasonable suspension, delay, or interruption, and the contract modified in writing accordingly.
However, no adjustment shall be made under this clause for any suspension, delay, or interruption to the extent that performance would have been so suspended, delayed, or interrupted by any other cause, including the fault or negligence of the Contractor, or for which an equitable adjustment is provided for or excluded under any other term or condition of this contract.
(c) A claim under this clause shall not be allowed -
(1) For any costs incurred more than 20 days before the Contractor shall have notified the Contracting Officer in writing of the act or failure to act involved (but this requirement shall not apply as to a claim resulting from a suspension order); and
(2) Unless the claim, in an amount stated, is asserted in writing as soon as practicable after the termination of the suspension, delay, or interruption, but not later than the date of final payment under the contract.

(a) Contractor's Responsibility for Fire Fighting. (1) The Contractor, under the provisions of FAR clause 52.236-9, PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND IMPROVEMENTS, shall immediately extinguish all fires on the work site other than those fires in use as a part of the work. (2) The Contractor may be held liable for all damages and for all costs incurred by the Government for labor, subsistence, equipment, supplies, and transportation deemed necessary to control or suppress a fire set or caused by the Contractor or the Contractor's agents or employees.
(b) Contractor's Responsibility for Notification in Case of Fire. The Contractor shall immediately notify the Government of any fires sighted on or in the vicinity of the work site.
(c) Contractor's Responsibility for Responding to Emergencies. When directed by the Contracting Officer, the Contractor shall allow the Government to temporarily use employees and equipment from the work site for emergency work (anticipated to be restricted to fire fighting). An equitableadjustment for the temporary use of employees and equipment will be made under the CHANGES clause, FAR 52.243-4.

I.2 FAR 52.243-5 Changes and Changed Conditions. (APR 1984)
(a) The Contracting Officer may, in writing, order changes in the drawings and specifications within the general scope of the contract.
(b) The Contractor shall promptly notify the Contracting Officer, in writing, of subsurface or latent physical conditions differing materially from those indicated in this contract or unknown unusual physical conditions at the site before proceeding with the work.
(c) If changes under paragraph (a) or conditions under paragraph (b) increase or decrease the cost of, or time required for performing the work, the Contracting Officer shall make an equitable adjustment (see paragraph (d)) upon submittal of a "proposal for adjustment" (hereafter referred to as proposal) by the Contractor before final payment under the contract.
(d) The Contracting Officer shall not make an equitable adjustment under paragraph (b) unless -
(1) The Contractor has submitted and the Contracting Officer has received the required written notice; or
(2) The Contracting Officer waives the requirement for the written notice.
(e) Failure to agree to any adjustment shall be a dispute under the Disputes clause.

SECTION M - Evaluation Factors for Award
M.1 AWARD DETERMINATION
Award will be made to the offeror whose acceptable offer provides the best value to the government, considering price and the non-price factors listed below. Non-price evaluation factors, when combined, are approximately equal to cost or price.
The selection official will base the award decision on a tradeoff between price and non-price factors, comparing the relative risk to the government of poor or non-performance posed by each of the offerors, and making a judgment as to whether or not reduced risk of performance is worth the additional cost. In some cases this will result in award to a lower-ranked but lower-priced offer; in other cases the result will be award to a higher-ranked but higher-priced offer.
NOTE: Offeror shall submit a minimum of two copies of their technical proposal and cost proposal.
Failure to provide a technical proposal in sufficient detail will be considered non-responsive and will result in disqualification.
M.2 EVALUATION FACTORS
Acceptability of the Offer The Government will determine the acceptability of each offer on a pass or fail basis. An offer is acceptable when it manifests the offeror's assent, without exception, to the terms and conditions of the RFP, including attachments. If an offeror takes an exception to any of the terms and conditions of the RFP, then the Government will consider its offer to be unacceptable.
Capability of the Offeror The Government will assess the capability of each offeror on the basis of the following criteria which are of approximately equal value: (1) Past Performance; and (2) Experience of Project Personnel. The Government will not assess technical proposals on a pass/fail basis, but will use its assessments of proposals as a basis in determining best value.
Criteria (01) - Past Performance
Offeror shall submit similar projects in which the offeror was a prime contractor within the past 5 years as part of the proposal response for the contractor (and proposed major subcontractors). Submit no more than five (5) past performance information.
(a) Offeror’s proposal shall provide the general background, experience and qualifications of their organization. If large portions of the contract will be accomplished by subcontractors, this information shall also be provided for these companies. Address specific performance issues and how you mitigated the cause of those issues. Provide details regarding timeliness of performance or lack thereof. Discuss business performance statement that describes relationships and cooperation with customers. Identify the projects you have successfully completed that you consider evidence that proves your firm has the experience required to complete this project. Include any investigation of alleged or actual violations of labor or immigration laws committed by your firm within the past three years and the disposition of those cases.
(b) LIKE contracts (contracts that perform work as defined in this solicitation), and/or grants should be included that shall contain the name of the customer, contract number, dollar amount, time of performance, and the names and telephone numbers of the project officer and contract/grants officer. Like work may receive preferential consideration.
(c) Similar or related contracts, and/or grants should be included that shall contain the name of the customer, contract number, dollar amount, time of performance, and the names and telephone numbers of the project officer and contract/grants officer.
(d) The proposal shall include a listing of all previous projects within the last five (5) years with telephone numbers of key personnel on these projects. Each project shall also clearly identify as to what type of project it is; LIKE, SIMILAR or RELATED project.
Criteria (02) - Experience of Project Personnel
(a) Describe experience performing similar requirements during the past 5 years. Include accurate information that will allow the evaluators to contact contract administrators involved with the referenced work. Names, phone numbers, dates, locations, type of work, etc. are critical. Specifically identify a project manager and construction superintendent that will provide for the performance of this contract as key personnel. Provide detail information about his/her experience. Describe what work you will directly perform and what work will be performed by a subcontractor. The approximate percentage of time each individual will be available for this project must be included.
M.3 PRICE PROPOSALS
The Offeror shall submit prices, which reflect the costs for the project, considering all related costs and profits.
Price will be evaluated after the non-price factor evaluations are completed. The award decision will be made in accordance with the award statement in Section M.1.
M.4 REQUIRED SUBMISSIONS
