Quality Assurance Surveillance Plan (QASP)
Project Name:
Ojo Forest Restoration Cultural Survey
Introduction
This Quality Assurance Surveillance Plan (QASP) has been developed pursuant to the requirements of the Statement of Work (SOW). This plan sets forth procedures and guidelines that the Forest will use in evaluating the technical performance of the contractor.
A. Purpose of the QASP
1. The QASP is intended to accomplish the following:
a. Define the roles and responsibilities of the participating government officials.
b. Define the types of work to be performed, i.e., work orders, preventive maintenance, telephone trouble calls, etc.
c. Describe the evaluation methods that will be employed by the government in assessing the contractor’s performance.
d. Provide copies of the quality assurance monitoring forms that will be used by the government in documenting and evaluating the contractor’s performance.
e. Describe the process of performance documentation.
2. The contractor has developed a Quality Control Plan (QCP), which sets forth procedures and responsibilities for controlling high quality work. The contractor has designated its employee, _________________________________, to be responsible for implementation of the QCP.
B. Roles and Responsibilities of Government Officials
The following government officials will participate in assessing the quality of the contractor’s performance. Their roles and responsibilities are described as follows:
1. Jana Comstock will serve as the Contracting Officer’s Technical Representative (COR) and be responsible for monitoring, assessing, recording and reporting on the technical performance of the contractor on a day-to-day basis. The COR(s) will have primary responsibility for completing a contact daily diary which will be used to document the inspection and evaluation of the contractor’s work performance.
2. District and Zone Archaeologists (Jana Comstock and Anne Baldwin) will serve as delegated project inspectors on behalf of the COR responsible for monitoring, assessing, recording and reporting on the technical performance of the contractor on a day-to-day basis. The contractor will meet with the District and / or Zone Archaeologists prior to beginning field work to assure that the contractor receives all relevant information such as project area access, or specific archaeological, historical, logistical, or other information for the project area that may not be available through pre-field records searches. The project inspectors will complete contact daily diaries which will be used to document the inspection and evaluation of the contractor’s work performance and be provided to the COR on a weekly basis.
3. will serve as the Contracting Officer (CO) and have overall responsibility for overseeing the contractor’s performance. The CO will also be responsible for the day-to-day monitoring of the contractor’s performance in the areas of contract compliance, contract administration, cost control, and property control; reviewing the COR’s assessment of the contractor’s performance; and resolving all differences between the COR’s version and the contractor’s version.
C. Types of Work to be Performed
Perform an intensive 100% pedestrian archaeological survey; record all newly discovered sites and all previously recorded sites located within the project area; document all Isolated Occurrence encountered during the survey; produce a report of professional quality.
D. Methods of Surveillance
Even though the government, through its COR and delegated technical inspectors, will be monitoring the contractor’s performance on a continuous basis, the sheer volume of tasks performed by the contractor make 100% technical inspections impractical. Accordingly, the government will use the following methodologies to monitor the contractor’s performance under this contract.
· Intensive Survey - Random field inspections on a maximum of 10% of the survey area by the Government and the review of the contractor’s records
· Site Recording - A maximum of 10% of recorded sites and/or a minimum of four sites shall be inspected.
· Recording Non-Site Cultural Remains (I.O.’s) - Random field inspections by the Government and verify contractor’s records, data and reports and/or witness contractor’s inspection.
· Report - 100% review and verify contractor’s records data and report.
E. Quality Assurance Forms and Reports
1. The COR will use the contract daily diary to document and evaluate the contractor’s performance under the contract. The form when completed will document the COR’s understanding of what the contractor was supposed to do, what was actually done, and the impact or consequences of what was done.
2. The COR will judge each event in accordance with the performance standards and performance requirements stated in the Performance Requirements Summary (PRS).
3. The COR must substantiate all tasks which they judge to be indicative of “unacceptable” performance. Performance at the “acceptable” level is expected from the contractor and need not be substantiated.
4. The COR will forward copies of the completed “unacceptable performance” contract daily diary to the CO and contractor by close of business on the day the forms were prepared. The contractor is required to respond in writing to any negative contract daily diary form(s) within 5 working days after receipt of the form(s).
F. Analysis of Surveillance Results
1. The CO will review each contract daily diary form prepared by the COR. When appropriate, the CO may investigate the event further to determine if all facts and circumstances surrounding the event were considered in the COR’s opinions outlined on the form. The CO will immediately discuss every event receiving a substantiated rating with the contractor to assure that corrective action is promptly initiated.
2. At the end of each month, the COR will prepare a written report for the CO summarizing the overall results of their surveillance of the contractor’s performance during the previous month. This report will become part of the formal Quality Assurance documentation.
