Questions received as of July 29, 2015
1. Is a bid guarantee required for this submittal? No bid guarantee is required for this solicitation.
2. On the bid schedules it calls for % per hour over DB wage, the Contractor Representative and Mine Safety Qualified Person are normally "exempt" positions as salaried employees could not find a rate for them on the DB wage rate pages. How would you like to see these hourly rates listed? The Schedule of Items for Labor has been edited to reflect the changes caused by this question and is a part of this amendment.
3. The SF 1442 form it states Sealed Proposals, so no electronic submittals as in the past correct? Electronic submittals are encouraged. Please email them to: rcole01@fs.fed.us .
4. On the bid schedules for the Polyurethane Foam per Cubic Yard, this is for the cost of the foam only, not an installed cost.
Correct – for foam only. Labor to install is under LABOR category in the Schedule of Items.
5. Please provide wage decision(s) number to be used for pricing the work included in the solicitation. These will vary within each state. The web address for DOL is www.wdol.gov.
6. Since this project covers both Arizona and New Mexico, it would be advisable to have separate bid schedules for each state. This would be based upon potential differences in the wage decision(s). Thank you for this suggestion but we are maintaining one bid schedule for the region.
7. Is a day considered 8 hours?
For equipment a day is considered 24 hours.
8. Is a week considered 40 hours?
For equipment purposes a week is considered to be calendar 7 days.
9. For mobilization/demobilization of backhoe or dozer, what is typical distance from a major city (i.e.
Phoenix, Flagstaff, Tucson, Albuquerque, Las Cruces)? Since project areas will vary the offeror must make their best estimate. Locality maps, and exact location of projects sites will be included in every task order.
We believe the average to be around 100 miles or less to a major city however, any miles over 100 miles are the responsibility of the contractor”
10. Will the unit prices quoted be the maximum allowed by the individual task order? Or is this being used only for contractor evaluation? This is the maximum allowed.
11. Please clarify how you would like contractors to mark up (i.e., show percentage increases) to the Davis Bacon wage determination rates for the specified positions. Wage determinations vary widely depending upon location and, in the case of heavy equipment operators, the type of equipment being operated. Percentage is requested by the solicitation.

APPLICABLE WAGE RATE DECISION
The Secretary of Labor has made a determination of the minimum prevailing wage rates which are to be paid to the classes of labor to be employed on this project. The decision number and date with minimum rates for this project will be identified within the RFQ of each Task Order.
