Performance Work Statement (PWS)
Janitorial Services, Forest Service Missoula, Montana
1. General Requirement
1.1. Scope of Work
The contractor shall furnish all labor, materials, tools, equipment, permits, transportation, and insurance required by this contract. The contractor shall at all times provide adequate supervision of employees ensuring all work is accomplished in accordance with the terms and conditions of the contract in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. Appendix A details the frequency at which the services are to be performed.
1.2. Personnel
1.2.1 Project Manager. The contractor shall designate, in writing to the Contracting Officer (CO), a full time Project Manager who shall be available to address any Government concerns. This individual shall be responsible for the overall management and coordination of work required under this contract and act as a central point of contact with the Government. The contractor shall also include in the designation letter any limitations on the Project Manager's ability to bind the contractor to changes/adjustments. This individual and the quality control personnel cannot be the same. At no time shall the Project Manager perform any custodial duties. The Project Manager shall have two full years of housekeeping experience within the last six years and demonstrate an active knowledge of aseptic requirements and cleaning techniques.
1.2.2. Personnel List. The contractor shall, within five work days after the start of the contract, furnish the CO a complete list of the name, address, social security number, and date of birth of each employee engaged in the performance of this contract. The Government may require additional information to establish the identity and legal employment status of each employee having access to Government buildings or facilities. In the event of a change in employees, the contractor shall inform the CO promptly of the change, normally within five calendar days after new employee hire or employee dismissal.
1.2.3. Employee Identification. The Government shall issue picture identification badges, containing the Government’s name and employee's name and picture at the Government’s expense. The identification badges shall be worn in a conspicuous place on the exterior clothing whenever work is being performed under this contract.
1.3. Contractor Quality Control
1.3.1. General Requirement. The contractor is responsible for quality control and shall establish and maintain an effective quality control system. The quality control system shall consist of plans, procedures, and organization necessary to produce services that comply with the contract requirements. The system shall cover all custodial operations. The contractor shall be responsible for the quality of work on the job and is subject to termination for non-compliance with quality requirements specified in the contract.
1.3.2. Quality Control System. The contractor shall implement and maintain a documented quality control system, including a Quality Control Plan (QCP), that identifies and results in correction of potential and actual problem areas throughout the entire scope of the contract. Three copies of the QCP shall be provided to the Contracting Officer no later than five (5) working days prior to commencement of work under this contract. The Government will consider an interim plan for the first 30 days of operation. The QCP or interim plan shall be implemented on the first day of contract performance. The contractor's Quality Control system shall include, as a minimum, the following to cover all custodial operations, including work by subcontractors:
1.3.2.1. Be structured to assure the individual(s) responsible for Quality Control is independent from any other parts of the contractor's organization.
1.3.2.2. Assure the Quality Control Manager has direct accountability to the contractor's top management.
1.3.2.3. Contain procedures for written and verbal communication with the Government regarding performance of the contract.
1.3.2.4 Contain procedures for handling corrective action without dependence upon Government direction.
1.3.2.5. Contain, as a minimum, specific surveillance procedures for each contract service identified in the Performance Requirements Summary. These surveillance procedures shall identify who shall perform the surveillance, the frequency, the method, listing of items under surveillance, and corrective action that will be taken to correct contractor identified deficiencies.
1.3.2.6. Include a customer complaint system. At a minimum, the customer complaint system shall contain procedures for the customer to file complaints with the COR/CO, form to be used by customers, procedures for investigation of complaint, and feedback to the customer and the Government on the results and actions, if any, taken on the complaint.
1.3.2.7. Maintain records of all contractor quality control checks and corrective action. These files shall be maintained by the contractor throughout the term of this contract and shall be made available to the Contracting Officer or designated representative during the term of this contract. The contractor shall retain its quality control files for not less than one year after the completion or termination of the contract.
1.3.3 Acceptance of Plan. Government acceptance of the QCP is conditional and will be predicated on satisfactory performance during the custodial services. The Government reserves the right to require the contractor to make changes in its QCP and operations including removal of personnel, as necessary, to obtain the quality specified. After acceptance of the QCP, the contractor shall notify the Contracting Officer in writing of any proposed change. Proposed changes are subject to acceptance by the Contracting Officer.
1.4. Safety. The contractor shall conduct its operations in strict conformance with all United States Department of Agriculture, United States Forest Service, and all applicable provisions of 29 CFR 1910 and 1926 of the Occupational Safety and Health Act. The contractor shall implement a suitable safety program for employees performing work under this contract. The safety program shall be in writing and a copy shall be furnished to the CO within thirty (30) calendar days after commencement of work under the contract. Immediately after a job-connected injury, the contractor shall prepare a report of the injury using its own accident report form or other documentation and send the report to the CO who will forward it to the Rocky Mountain Research Station Safety Officer. Any technical advice and assistance necessary for reporting and investigating accidents may be requested from the Safety Officer through the CO.
1.5. Security. All outside doors shall remain locked while contractor performs work and the contractor shall assure that all interior and exterior doors in the main building and outlying buildings are locked prior to departure. Unnecessary lights/faucets shall be turned off and, windows closed upon completion of duties. Do not adjust mechanical equipment controls. Contractor shall immediately report fires and hazardous conditions to the COR, turn in lost and found articles to the COR, and notify the COR when an unauthorized or suspicious person is seen on the premises. Report any items in need of repair to the COR. Contractor shall immediately notify the COR if any problems are encountered either with the alarm or if any keys are lost.
2. –Not Used-
3. Government Furnished Property, Facilities and Services
3.1. Facilities. The Government will furnish the contractor’s an area to use for storage of equipment and supplies. The area occupied by the contractor may change at any time at the Government's discretion. The contractor shall not construct any new building facilities or structures on Government property nor make any structural changes without written approval of the Program Manger through the CO. Structural repairs required during the term of the contract shall be reported to the COR for appropriate action. The contractor shall reimburse the Government for repairs not attributable to fair wear and tear.
3.1.1. Contractor shall not change the locks at any time.
3.1.2. Within five (5) working days after contract start date, the contractor and COR will conduct a joint building condition and installed equipment condition/repair inventory inspection. Building and installed equipment condition, damage, and repair requirements will be recorded and the record signed by the contractor and the COR.
3.1.3. Within fifteen (15) working days prior to the end of a contract period, the contractor and the COR shall conduct a joint building and condition inventory of all building facilities and installed equipment furnished to the contractor. All damage (other than fair wear and tear) which was nonexistent at the time of contract start date and/or building possession date will be documented and signed by the contractor and the COR. Any damage noted at this time, which resulted through other than fair wear and tear, shall be corrected or repaired at the contractor's expense.
3.2. Supplies. A Government ladder will be provided for cleaning high areas and windows.
3.3 Utilities. The Government will furnish electrical power, sewer service, and water at no cost to the contractor during performance of this contract. The contractor shall conserve all furnished utilities. The Government will not furnish telephone service.
3.4. Trash Disposal. The Government will furnish pick up and disposal of trash generated in or associated with the performance of this contract. The contractor shall be responsible for the placement of trash in the nearest dumpster. Trash shall not be placed in dumpsters/roll-a-ways belonging to other contractors. If such an event occurs and results in a claim against the Government for disposal of the trash, the contractor will be charged for the amount of the negotiated claim.
3.5. Key Control. The contractor will be responsible to conduct a background check on their employees. This clearance will be sent to the CO/COR. The contractor will complete any required Government forms necessary to issue security door access cards. Keys and Contractor Security Door Access Cards will be provided by the Forest Service. The contractor shall be liable for all loss or damage of such delivered Government-furnished property until completion of and final acceptance of work required under this contract. The Government, at the contractor's expense, shall replace any keys lost by the contractor's employees. In the event a master key is lost, the Government, at the contractor's expense, shall replace keys on that master system. If a standard (non-master) key is lost, the lock for that key will be replaced by the Government and the cost deducted from the monthly payment due the contractor. Lost keys shall be reported to the COR immediately. If the COR is not available, the loss shall be reported to the CO.
4 Contractor Furnished Items
4.1. Supplies. The contractor shall furnish soap, recycled, paper hand towels, 2-ply toilet paper, ladder, and other necessary cleaning supplies. The contractor shall provide all nontoxic, no fragrance cleaning products, vacuums, carpet cleaner, shampoo, rags, and any other cleaning supplies and equipment necessary to adequately meet the cleaning specifications. No flammable or explosive liquids such as gasoline will be stored. The contractor shall submit a list of cleaning products to be used to the Contracting Officer’s Representative (COR), for approval at the pre-performance conference. If a piece of equipment needs repair or is determined unsafe by the COR, the contractor shall immediately stop using the equipment. Defective equipment shall be repaired or replaced within 72 hours. All supplies, unless otherwise specified, shall be of the highest quality and most suitable type or grade for the type of work specified. Materials and equipment shall be of suitable size and type for the work or service being performed.
4.1.1. All containers of cleaning chemicals, hazardous materials, and similar products shall be conspicuously marked with a factory label to identify contents. Hazardous materials shall be labeled with appropriate Department of Transportation (DOT) or Interstate Commerce Commission (ICC) hazard labels (flammable, corrosive, acid, etc.), as specified on the Material Safety Data Sheet.
4.1.2. The Government reserves the right to require samples of supplies to be used by the contractor to determine suitability and standards compliance. The contractor, at the contractor's expense, shall replace item(s) failing to meet required standards.
4.1.3. The contractor shall use products made with recycled materials to the greatest extent possible. Preference shall be given to concentrated cleaning chemicals which use ecologically sound packaging and are phosphate-free, non-corrosive, non-flammable and fully biodegradable.
4.1.4. The use of caustics (acid-based cleansers) will not be approved. Materials or supplies shall not be used in performance under this contract (or placed or stored on Government property) until the applicable Material Safety Data Sheets for all chemicals and hazardous materials (products containing CAUTION or WARNING labels) have been furnished to the Government and approval has been received from the COR.
4.2. Material Safety Data Sheets (MSDS). The contractor shall submit MSDS sheets obtained from the manufacturer or dealer concerning all hazardous materials (i.e., chemicals and cleaning solutions) to be used under this contract through the COR to the Rocky Mountain Research Station Safety Office within three (3) days after commencing work. Reference: 29 CFR 1910-1200. MSDS sheets shall be submitted and products approved prior to the use of any products.
4.3. Contractor’s Equipment. The Government may inspect Contractor's equipment at any time. Contractor's equipment shall be subject to approval by the COR/Contracting Officer. The contractor shall service or replace at no cost to the Government any equipment found to be defective, unsafe, or unsuitable.
4.3.1. For protection of walls and doors, all wheeled and movable equipment shall be equipped with protective non-marking wheels and rubber bumpers or guards around the entire perimeter. No part of the equipment (except fixed handles) shall protrude beyond the rubber bumpers.
4.3.2. The contractor shall establish a communication paging and/or cell phone system whereby key personnel (Project Manager, Assistant Project Manager, Quality Control Inspector, Day and Night Supervisors, etc.) may be contacted by the CO/COR.
4.4. Trash Receptacle Liners. The contractor shall provide at a minimum 1mm thick liners in trash receptacles.
4.5. Removal of Property Upon Expiration of the Contract. The contractor shall, within five (5) days after the expiration of this contract, remove from Government premises all contractor owned vehicles, equipment, tools, supplies, materials, or other items. If the contractor does not remove said items within the stated period, the items will be delivered to the designated property office and disposition will be made in accordance with Government regulations.
5. Specific Tasks
5.1. Basic cleaning services. The contractor shall perform the required custodial services of this contract between the hours of 5:00 p.m. and 11:00 p.m. Monday through Friday and in accordance with the terms of this Performance Work Statement (PWS). The tasks and frequencies of the work is listed in the Task and Frequency Charts.
5.1.1. Basic restroom cleaning services. The contractor shall accomplish all cleaning tasks to meet the requirements of this PWS and the SDS. The minimum cleaning frequencies are established in the Task and Frequency Summary, Appendix A.
5.1.2. Periodic Cleaning Services (monthly/quarterly/annually). The contractor shall accomplish all cleaning tasks to meet the requirements of this PWS and the SDS. The minimum cleaning frequencies are established in the Task and Frequency Summary, Appendix A.
1.5.3. Cleaning of the carpets, waxing, and windows can be done on weekends from 5:00 a.m. to 11:00 p.m. with prior approval from the COR.
6. Service Delivery Summary. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement, Appendix B.
7. Performance Requirements Summary. The purpose of Appendix C is to:
· List contract and work requirements for satisfactory contract performance;
· List contract requirement paragraph number in the statement of work;
· Provide maximum allowable defect rate for each work requirement; and
· Identify the Method of Quality Assurance
8. General Information
8.1. Government Responsibilities. The Government will not be responsible for damage occasioned by fire, theft, accident, or otherwise to the contractor's supplies, materials, or equipment in storage, or the contractor's and their employee's personal belongings brought into the building.
8.2. Contractor Responsibilities. The contractor is responsible for loss, theft, damage, or destruction of any Government or personal items belonging or assigned to the occupant or user in any office or other rooms of the facility when such acts can be proven, beyond a reasonable doubt, to be the fault of the contractor or one of their employees. No one under the age of 16 shall be permitted in the building or on the premises during the period of 5:00 p.m. to 11:00 p.m. The contractor shall ensure that no one but their employees have knowledge of the fire alarm security code to the main building alarm system. The contractor will supply written background checks for security purposes of its employees to the COR. The contractor shall be responsible for ensuring their employees do not disturb papers on desks, open desk drawers or cabinets, or use Government telephones or equipment, unless authorized by the COR. Contractor must establish and implement methods of ensuring that all keys issued by the Government are not lost, misplaced, or used by unauthorized persons.
8.3. Energy Conservation. Contractor shall turn on lights in rooms only during the time needed to perform the required work and ensure that all lights are turned off in each room as work in that room is completed, except those required to remain on.
8.4. Federal, State, and Local Codes and Laws. All chemicals used will be those commercially available which meet Federal, State, and local codes and which have been approved by the COR/CO. Information can be obtained from Federal, State, and local agencies concerning safe chemical cleaning materials. The contractor shall use products made with recycled materials to the greatest extent possible. Preference should be given to concentrated cleaning chemicals which use ecologically sound packaging and are phosphate‑free, non‑corrosive, non‑flammable, and fully biodegradable. The use of caustics (acid‑based cleansers) will not be approved. Materials or supplies shall not be used in performance under this contract (or placed or stored on Government property) until the applicable Material Safety Data Sheets have been furnished to the COR/CO. No item containing a CAUTION or WARNING label will be added to the contractor's products list until the COR/CO has given verbal approval for its addition. The contractor shall not use any material which the COR/CO determines would be unsuitable for the purpose or harmful to the surfaces to which applied. Costs for correcting damage caused by misused materials will be borne by the contractor. The COR/CO reserves the right for final approval on all materials and supplies utilized by the contractor.
8.5. Occupational Safety and Health Act (OSHA). Full compliance with Federal Law and USDA Regulations requiring safety is applicable.
8.6. Environmental Protection Agency (EPA). Full compliance with environmental and hazardous waste management laws, rules, and regulations is applicable.
8.7. Fire Prevention. Fire prevention shall be exercised at all times by the contractor and employees. All combustible material shall be used and stored in a safe manner. Contractor shall report to the COR/CO any fire hazards.
8.8 Tobacco Control. The contractor and its employees shall observe the Tobacco Control requirements contained in DR4400-006, 5 U.S.C. 301; 5 U.S.C. 7901; 29 U.S.C. 668; 41 CFR 101-20.105-3; and 41 CFR 101-39.300(d) whenever performing services in or visiting Government facilities/buildings.
Appendix A
Task and Frequency Summary
Daily (all offices)
· Vacuuming. All carpeting, rugs, under desks, filing cabinets, bookcases, around computer terminals, between areas such as space between filing cabinets and walls, desks and window, and under heat registers.
· Dusting. All rooms, entryways and corridor spaces including furniture and windowsills.
· Waste Containers. All wastebaskets and other trash/cigarette receptacles inside and outside must be emptied, place in proper container and deposited in outside trash dumpster.
· All toilet fixtures, seats, and urinals, washbasins shall be clean and sanitized. Scale removers, germicides, and deodorant shall be utilized where needed. Mirrors and fixtures must be clean and streak free. Floors must be swept and damp mopped with a disinfecting cleaner in restrooms and break room. Fill toilet paper, paper hand towels, paper bags, soap, and toilet seat dispensers.
· Windows and glass areas in entry must be spot cleaned and free of streaks.
· Drinking fountains clean and sanitized.
Weekly
· Doors, doorframes, baseboards, and wall switches must be spot cleaned.
· Disinfect and clean all bathroom stalls.
· Wash and sanitize sink and counter area.
· Mop back entryway, labs and dirty room.
· Sweep and wet mop lab floors when doors are left open.
· Clean work center conference room and restroom.
Monthly
· Damp wipe all trash receptacles inside and out.
· Spot clean all carpeting.
· Spot clean and remove marks on all wall surfaces.
· Clean and remove all marks on lab floors, vestibule and entrance floors.
· Clean wall tiles in restrooms.
· Scrub mop sink and clean janitor closet. If mop is to be left overnight, must be sanitized, disinfected, with no odor.
Quarterly
· High cleaning: dust, vacuum, or damp wipe all surfaces and objects 70+ inches above floor including, but not limited to, door ledges, moldings, tops of modular furniture, partitions, lockers, file cabinets, bookcases, clocks, pictures, plaques, heat registers, cold air grills, ceiling areas, etc.
· Clean and sanitize trashcans.
· Removal of all cobwebs and bugs from inside ceiling light fixtures.
· Sweep and mop all mechanical room concrete floors.
Semi-Annually
· Wash all inside and outside windows, frames, and casings, all sills and inside blinds, and outside screens. Wipe down to remove all water so spotting will not occur.
· Lab floors to be stripped waxed & re-waxed to include the Combustion Lab floor. Coordinate with COR one week in advance to ensure that employees will not be working in Labs.
· Clean/Shampoo Carpets. All carpets shall be cleaned in accordance with standard commercial practices. A heavy-duty spot remover may be required in heavily soiled areas. After shampooing, the carpeted area shall be uniform in appearance and free of stains and discoloration. All cleaning solutions shall be removed from baseboards, furniture, trash receptacles, chairs and other similar items. Chairs, trash receptacles, and other items shall be moved to clean carpets underneath, and returned to their original location. Schedule carpet cleaning/shampooing with the COR shall be one week prior to cleaning/shampooing carpet.
· Polish wood wall paneling in Fire Sciences Lab main building and Forestry Sciences Biology building.

Basic Restroom Cleaning Services.
Restrooms are disinfected and free of dirt, deposits, streaks and odors. Shower is disinfected and free of soap films, scum and other deposits. Toilets, urinals and toilet stalls are disinfected and free of scale, stains, scum and other deposits. Supplies are adequate until next service. Floors shall be clean and free of dirt, dust, debris, water streaks, mop marks, etc.

Periodic Cleaning Services.
Floors have a glossy uniform appearance free of scuffmarks, heel marks, wax build-up, and other stains and discoloration. Windows and Venetian Blinds are free of film, dirt, smudges, water, and other foreign matter. Carpeted area uniform in appearance and free of stains and discoloration.
