Offerors are required to submit proposals in 3 volumes and shall be in the format provided in this addendum, failure to submit proposals in the required format may render the proposal “non-responsive” and may not be considered for award. Each Volume shall be separated within the proposal package and should not be a continuation of the previous Volume. Proposals can be issued on offeror’s letterhead and must include the solicitation number. Proposals that exceed page restrictions may not be considered for award; a cover page and summary may be included and do not count against page restrictions.

a. Volume I shall contain 2 sections; Section A – Technical Approach and Section B – Quality Control

1. Equipment: Describe your equipment, including the make, model, year of manufacture, capabilities, accessories, and specifications. Include type of cutting head and how it works on brush (does it cut brush clean off and can it chip it). Describe any down time your equipment has experienced and the measures you take to get the equipment operational again.

2. Method of Performance: Briefly describe your method(s) of meeting the road bushing requirements.

3. Safety: Outline the precautions taken that will be utilized for the safety of the operator(s) and other users of the roadway and describe the safety features on the brushing equipment.

c. Volume I - Section B, Quality Control Shall contain the following information:

Quality Control: Describe your method of self-inspection and means of insuring that all contract specifications are met without repeated rework and without reliance upon Government inspections.

CONTRACTOR QUALITY CONTROL PLAN. In accordance with FAR 52.246-4 Inspection of Services -- Fixed-Price (b), the Contractor shall identify the quality control inspection system it will use to ensure that contract performance specifications will be achieved. This Quality Control Plan shall be used by the contractor to monitor its performance and at no time shall the contractor rely upon Government inspections to provide notification of unsatisfactory performance. The Contracting Officer may observe the Contractor's inspection at any time and shall otherwise have unlimited access to the inspection data.

QUALITY ASSURANCE PLAN/REQUIREMENTS and DISINCENTIVES: The Contractor shall provide a Quality Control Plan that will ensure that the work is being performed in a manner that will meet the objectives of the contract. The Government will, at its convenience, make periodic inspections of the work to assure itself of contract compliance.

(a) - Contractor Quality Control Inspection. The Contractor shall ensure that performance meets contract specifications in accordance with the Quality Control Plan, prior to requesting the Government to inspect for payment or acceptance.

(b) - Request for Government Inspection: Upon completion of a road, or segment of road not less than 5 miles in length, the Contractor shall contact the Contracting Officer's Representative (COR) to set a mutually agreed upon date for inspection of completed work.

2. Volume II, Past Performance (15 Page maximum)

Past performance information should demonstrate a history of quality work, timely completion, and the firm’s and proposed operator’s commitment to customer satisfaction.

3. Volume II, Price (1 page maximum)

a. Volume III shall contain your proposed price per mile. The USFS intends to award a single firm-fixed price requirements contract for roadside brushing based upon an established price per mile. Offerors shall propose a set price per mile which will provide the basis for the price evaluation in determining the offeror whose proposal demonstrates the over best value. Upon contract award, individual orders for roadside brushing will be issued at the fixed price proposed by the winning offer.

b. Method of Measurement: Roadside brush cutting will be measured in miles and nearest tenths of miles along the road centerline of the roadway, and shall include only those sections of the roads which are included in the contract and completed in accordance with the specifications. Parking areas, turnouts, and approach road connections are considered as adjuncts to the main roads and shall not be measured separately. Mileage includes brush cutting both sides of the road.

c. The ordering period for the resulting contract will be from the date of award through 30 September 2010. It is anticipated that brushing will occur between the months of April and November and each individual order will provide a period of performance of one (1) working day for every four (4) brushing miles as defined in part b above.
