I. CONTINUATION OF BLOCKS 19 THROUGH 24 (Schedule of Services):

1. BASICSERVICES:SeethePerformanceWorkStatement(PWS).Provideallmanagement,supervision,
labor,supplies,materials,andequipment(exceptasotherwisespecifiedherein),andplan,schedule,
andcoordinateasnecessarytoensureeffectiveperformanceofON‐SITEDentalservicesasspecified

herein.

TheDentalserviceprovider(s)musthaveanactive,unrestrictedlicensetopracticeasan
independentpractitionerintheStatewhereservicesareprovided.Allservicesprovidedmust
beinaccordancewithstateregulations.
AllcandidatesrequireapprovalbytheContractingOfficePRIORtoprovingservicesatthe
Centerbeforeandafteraward
ContractorsareonlyresponsibleforprovidingforbasicdentalcareasdetailedinthePWSin

2. LOCATION: The Contractor shall be fully responsible for providing Dental Health services as indicated herein for the location listed below:

3. PERFORMANCE: Contractor personnel will perform all work required in the PWS for the amounts

agreedtoasstatedbelowandinaccordancewiththeapplicablewagedetermination,ifapplicable..
Thecenterwillbeclosedforone,two(2)weekperiodinthewinterandforallFederalholidays.The
breakgenerallyoccursthelasttwoweeksinDecember;however,theexactdatesaresubjecttochange.
TheContractorwillbepaidbasedupontheactualworkperformedpursuanttothePWS.Performance
schedulesshallbeagreedtothroughmutualagreementwithCenterpersonnel.

4. SUBSTITUTE SERVICES: When Contractor personnel are on vacation, sick leave, or any other leave,

qualifiedsubstitute(s)shallbeprovidedbytheContractorinaccordancewiththePWS.TheContractor
shallensurethatthecurrentlicense(s)foreachsubstitutepersonnelissubmittedtotheWO‐AQMoffice
forapprovalPRIORtoprovidingservices.

FAR 52.217‐8 Option to Extend Services (NOV 1999)

TheGovernmentmayrequirecontinuedperformanceofanyserviceswithinthelimitsandat
theratesspecifiedinthecontract.Theseratesmaybeadjustedonlyasaresultofrevisions
toprevailinglaborratesprovidedbytheSecretaryofLabor.Theoptionprovisionmaybe
exercisedmorethanonce,butthetotalextensionofperformancehereundershallnotexceed
6months.TheContractingOfficermayexercisetheoptionbywrittennoticetothe
Contractorwithin30days.

52.217‐9 Option to Extend the Term of the Contract (MAR 2000)

(a)TheGovernmentmayextendthetermofthiscontractbywrittennoticetothe
Contractorwithin30daysofexpirationofthecurrentcontract;providedthatthe
GovernmentgivestheContractorapreliminarywrittennoticeofitsintenttoextendatleast
60daysbeforethecontractexpires.Thepreliminarynoticedoesnotcommitthe
Governmenttoanextension.

and procedures established by the Department of Agriculture (USDA) Directives 4620‐002 series.

(b)ShouldtheresultsofthePIVprocessrequiretheexclusionofacontractor's
employee;thecontractingofficerwillnotifythecontractorinwriting.
(c)Thecontractormustappointarepresentativetomanagecompliancewiththe
PIVpoliciesestablishedbytheUSDADirectives4620‐002seriesandtomaintainalistof
employeeseligibleforaUSDALincPassrequiredforperformanceofthework.
(d)Theresponsibilityofmaintainingasufficientworkforceremainswiththe
contractor.ContractoremployeesmaybebarredbytheGovernmentfromperformanceof
workshouldtheybefoundineligibleortohavelosteligibilityforaUSDALincPass.Failureto
maintainasufficientworkforceofemployeeseligibleforaUSDALincPassmaybegrounds
forterminationofthecontract.
(e)Thecontractorshallinsertthisclauseinallsubcontractswhenthesubcontractor
isrequiredtohaveroutineunaccompaniedphysicalaccesstoaFederally‐controlledfacility
and/orroutineunaccompaniedaccesstoaFederally‐controlledinformationsystem.
(f)ThePIVSponsorforthiscontractisadesignatedprogrampointofcontact,which
inmostcasesistheCOR\COTR,unlessotherwisespecifiedinthiscontract.ThePIVSponsor
willbeavailabletoreceivecontractoridentityinformationfromMondaytoFridayduring
businesshoursattheJobCorpsCenter.TheGovernmentwillnotifythecontractorifthereis
achangeinthePIVSponsor,theofficeaddress,ortheofficehoursforregistration;however,
itisthecontractor'sresponsibilitytomeetallaspectsofparagraphs(c),(d),and(e).

REPORTING ALLEGED CHILD ABUSE

Pursuant to the spirit and requirements of P.L. 101‐647, Section 225 and 226, the following applies:

AnyemployeeorrepresentativeoftheContractorwhobecomesawareof
allegationsthatachildintheprogramcoveredbythiscontractis(a)avictimofacrimeof
physical,verbal,oremotionalabuse,sexabuse,orexploitation;or(b)awitnesstoacrime
committedagainstanotherperson;shallmakeareportofthesuspectedabusetothe
ContractingOfficerandtheContractingOfficer’sRepresentative(COR)within24hours.The
initialreportshallbemadebyphoneandshallbefollowed‐upinwriting,providingsalient
pointsoftheallegations.
Whensuchallegationsaboutacontractoremployeeoccurascoveredbythisclause,
theContractoremployeewhoisthesubjectoftheallegationswillimmediatelyberequired
tovacatetheworksiteandwillbereplacedbyanothercontactoremployee.Thesubject
employeewillremainawayfromtheworksiteuntilaninvestigationiscompleted.The
GovernmentwillnotincuradditionalcostsbytheContractorwhomustprovidea
replacementemployee.Ifthecontractorcannotprovideasubstituteemployeeduringthe
investigation,thecontractmaybeterminatedforperformancerelatedissues.
TheContractorshalllimitdisclosureofdocumentsconcerningallegedchildabuseto
thepersonswhobyreasonoftheirparticipationintheproceedinghavereasontoknowsuch

information.

Anyconvictionofasexcrime,anoffenseinvolvingachildvictimordrugfelony,an
assaultfelony,oranyotherviolentcrime,willbegroundsfordenyingemploymentorfor
dismissalofanemployeeinanyposition‐servingchildren.Convictionofacrimeotherthana
sexcrimemaybeconsideredifitbearsonanindividual’sfitnesstohaveresponsibilityfor
thesafetyandwell‐beingofchildrenasdeterminedbytheContractingOfficer.The
considerationofanothercrimebearingonanindividual’sfitnesswillbemutuallyagreed
uponbetweentheContractingOfficerandagencyinvolved.

PERFORMANCE WORK STATEMENT

III. 1. INTRODUCTION:

TheUSDAForestServiceJobCorpsCivilianConservationCenters(JCCCC)provideacademic,vocational,
andlife‐learningskillstoatriskyouthandyoungadultsat28JobCorpsCentersacrosstheUnitedStates
(U.S.).Aspartofthisprogram,youthandyoungadultsattheCentersareprovidedhealthcareservicesas
follows:medical,dental,mentalhealth,anddrug&alcoholintervention.EachForestServiceJCCCChasa
needforbasichealthservicesprovidedbylicensedmedicalprofessionalsthatincludesexaminationsand
assessments,treatment,issuanceofprescriptionsandimmunizations,monitoringhealthconditions,
recommendinghospitalization,follow‐upcare,andpreventionplanningandintervention.

III. 2. BACKGROUND:

JobCorpsisaU.S.DepartmentofLabor(DOL),EmploymentandTrainingAdministration(ETA)program.
JobCorpsisdesignedtoassistyoungpeoplewhoneedandcanbenefitfromthewide‐rangeofservices
providedataJobCorpscenter.Servicesincludethefollowing:basiceducation,HighSchoolDiploma(HSD)
program;GeneralEducationalDevelopment(GED);vocationalskillstraining;residentiallivingprogram;
healthcare;andrelatedsupportservices.Thecomprehensivecombinationoftrainingandsupportservices
isdesignedtohelpeachstudentbecomearesponsibleandproductivecitizen.Studentpopulationsrange
from100toover300enrollmentsdependingontheCenter’sOnBoardStrength(OBS).JobCorpsisaself‐
pacedprogram:therefore,astudent’slengthofenrollmentvaries.Studentsmayremainintheprogramfor
uptotwoyears,buttheaveragelengthofenrollmentisapproximatelyeightmonths.

III. 3. CONTRACT EFFORT REQUIRED:

TheContractorDentist,DentalHygienistandDentalAssistantshallprovideON‐SITEdentalcareservices
forallstudentsonCenter,inaccordancewithapplicableFederallaws/regulations,OSHAandHIPAA
requirements,stateandlocallaws,andguidelinesoftheAmericanDentalAssociation.Dentalservicesare
requiredonaweeklybasistoassiststudentsinattainingandmaintainingoptimaloralhealth,withthe
servicesbeingprovidedattheCenter.TheoralhealthcareservicesmustcomplywithJobCorpspolicies,
requirements,andguidelinesandmustalsoincludeanalysisanduseofthemostefficientandcost‐effective
practiceofcontemporary,evidence‐basedhealthcarepossiblegivenavailabletime,resources,and

personnel.

TheseserviceswillinvolvecoordinatingandintegratingtheDentalHealthCareservicesprogramwith
otherH&Wcomponents/activitiesonCenterandprovidingoralhealthcareservicestoenrolleesand
establishedstudentsonCenter.Provisionofdentalservicesrequires,butisnotlimitedto:

the ability to work both independently and as a member of a healthcare team to provide optimal

servicestoapopulationoffinanciallydisadvantagedyouth,rangingfrom16to24yearsinage;
provisionofallproceduresauthorizedbytheStateBoardofDentistry,withageneralemphasisonearly
detection,diagnosisoforalhealthproblems,basicoralhealthcare,dentalhygiene,and
prevention/education(e.g.oralhygieneinstructions,riskassessments,relationshipwithemployability,
oralhealthandwellness);
anelectiveoralexamination,includingbitewingx‐rays,priorityclassification,andtreatmentplan,shall
becompletedandrecordedontheJobCorpsapprovedoralexaminationform,uponstudentrequest

performingdentalprocedurestotreatoraldiseaseandcorrectoralhealthconditionsthatmay
representemployabilitybarriers,toinclude:restorations,extractionofpathologicalteeth,rootcanal
therapyonanterior/otherstrategicteeth,replacementofmissingupperanteriorteethwitha
removableprosthesis,anddentalhygienetreatmentforperiodontaldisease,uponapprovalfrom
centerpersonnel;
complyingwithJobCorpsinfectioncontrolpoliciesandprocedures;
providingwrittendiagnosisandtreatmentplanforeachstudentwithtwoweeksaftercompletionof
thedentalreadinessinspectionandobtainingagreementforvoluntarycarebeforeproceedingwith
suchtreatment;
providingstudentservicesinsuchamannertoobtainpositivestudentsatisfactionsurveyresultsas
determinedbyJobCorps;and

Additional services required in support of the Job Corps Center’s Health and Wellness Program include:

ensuringpatientprivacyforallassessments,exams,andprocedures;maintaining,safeguarding,and
ensuringtheconfidentialityofallrequiredstudenthealthrecordswhentheyareinthehealthclinicor
inthepossessionofhealthandwellnessteammembers,accordingtotheHealthInsurancePortability
andAccountabilityAct(HIPAA);
notengaginginanyresearchprojectsand/orpublicationsrelatingtoJobCorpshealthactivities
withoutpriorwrittenapprovalfromtheJobCorpsNationalOfficeinGolden,CO;
advisingandassistingthehealtheducationinstructorinthedevelopmentandimplementationofthe
DentalHealthEducationProgram;
ensuringdentalequipmentisproperlycleanedandscheduleformaintenance;
reportingadversestudentbehaviorincidentstotheHSMorCOR;and
understandingthatdentalservicesshallbesubjecttomonitoringandreviewbyJobCorpsassessment

teams, and cooperation with such teams is required.

III. 4. SCHEDULE AND AVAILABILITY:

Dentalservicesarerequiredonaweeklybasis,duringdaysoftheweekandhoursofthedayestablishedin
coordinationwithCenterpersonnel.Thedaysandtimesofservicesshallbeuponmutualagreement
betweentheContractorandcenterpersonnelandmustbeconsideredreasonable.Areasonableschedule
wouldbeconsideredprovidingservicesone(1)ortwo(2)daysaweek,M‐F,8am–5pm.
CenterOBSandthePRHmaychangeatanytime;consequently,thequantityofrequiredDentalservices
hoursmayvary,dependingontheneedsofthestudentpopulation.However,anychangestotherequired
hourswillbeexecutedviaabilateralmodificationtothecontract
KeyPersonnelmustprovideacurrentcontactnumbertotheCenterHSMorCOR.WhenKeyPersonnelare
unavailable,theContractorwillprovideanacceptablesubstitutetoprovideservices.

III. 5. QUALIFICATIONS OF CONTRACTOR PERSONNEL:

A. Contractor personnel:

must be registered/licensed with the state in which Dental services will be provided; all licensing

requirements/certification/accreditationofthecontractor’sstaffmustbemaintainedduringthe
entireperformanceperiodwithproofbeingsubmittedattherequestoftheGovernment;
thedentistmusthaveanactive,unrestrictedlicensetopracticeasanindependentpractitioner
shallprovideacopyoftheDrugEnforcementAdministration(DEA)Certification,ifapplicable.

maybesubjecttoacriminalbackgroundcheckanddrugtestingasprerequisitesforcontract
performance.Informationobtainedthroughthebackgroundcheckmaybeusedindetermining
contractor’squalificationforthiscontract.Informationregardingaperson’srace,nationalorigin,
color,sex,religion,disability,geneticinformation(includingfamilymedicalhistory),oragewillnot
beevaluatedorhaveanyimpactonthedeterminationofqualificationsforthiscontract.
Governmentwillterminatethecontractforconvenienceifthebackgroundcheckisdeterminedto
beunacceptableorifanyaspectwouldjeopardizethehealthandsafetyofthestudents.

III. 6. MATERIALS, EQUIPMENT, TRAINING, AND UTILITIES:

A. The Government shall furnish the following:

Adequateworkspace,equipment,supplies,andmaterialsasneededtoaccomplishthework
requirementsoutlinedherein,includingexaminationroomsandfurniture,assessmentequipment,
telephone,computer,afaxmachine,prophylaxissupplies,gloves,masks,andsimilaritems(unless
servicesareprovidedatthephysician’soffice).Prescriptionandover‐the‐countermedicationswillbe
disbursedattheCenter.Thesesuppliesandpropertyshallbeutilizedduringtheperformanceofthis
contractonlywhileprovidingstudentcare.Thecontractorwillprovideanitemizedlistofitems
neededtotheHWMatleast30daysinadvanceofthedesireddeliverydatetoallowforadequatetime
fororderingandreceiving.

B. The Contractor shall furnish the following:

Alllabor,services,permits,licenses,transportationofContractorpersonnel(asnecessary),and
incidentals(otherthanthoseprovidedbythefederalGovernment)asneededtofulfillthework
requirementsherein,forthehoursrequiredbasedontheCenter’sOnBoardStrength(OBS).In
addition,theContractorshallinsurethatallContractorpersonnelandtheiragentsarecoveredby
professionalliability(malpractice)insuranceandgeneralliabilityinsuranceduringtheentire
performanceperiodandagreestoindemnifyandholdharmlesstheJobCorpsCenterfromanyactions
takenby,for,oragainsttheContractorortheiragentsthatcouldresultinadditionalexpenses,fees,or

lawsuits.

C. Security:

Thespace,equipment,andsuppliesusedbycontractorpersonnelMUSTbesecuredatalltimes,per
Centerrequirements.TheContractorshallreviewsecurityprocedureswiththeCenterHWMandCOR.

III. 7. RECORDS, REPORTS, AND REQUIRED SUBMITTALS:

A. The following submittals are required during the period of performance of this contract:

Documentation of all examinations, tests, treatments, medications, referrals, etc. on approved Job

CorpsHealthandWellnessProgramforms,suppliedbythecenter,tobemaintainedinStudents’
HealthRecords(SHR);
Reviewofandauthorizationofstandingordersforhealthcareannually,inaccordancewith
TechnicalAssistanceGuideM:HealthCareGuidelines;and
ReportstotheCenterDirector(CD)andHealthandWellnessManager(HWM)onallhealth‐related
mattersincludingstudentmedicalseparations,justificationsforandcostsofspecialservices,
HealthandNon‐HealthCareGuidelines,equipmentrepairandreplacementneeds,depletionof
medicalsuppliesandmedications,andotherareasdeemedappropriate.

ThefederalGovernmentreservestherighttohavetheJobCorpsRegionalHealthCoordinatorand
otherauthorized/qualifiedpersonnelconductauditsofContractorservicesandstudentmedical
recordstoverifythatqualityservicesarebeingperformed.Acopyofanyauditreportshallbe
providedtotheContractorandContractingOfficer(CO)withintwoweeksoftheaudit.Withintwo
weeksafterreceiptoftheauditreport,theContractorshallsubmitaperformanceplanfor
improvingservicestotheContractingOfficer(CO)forapproval.TheContractorshallimplement
theapprovedperformanceplanimmediately.

III. 8. THE ROLE OF GOVERNMENT PERSONNEL AND RESPONSIBILITY FOR CONTRACT

ADMINISTRATION:

A.ContractingOfficer:TheContractingOfficerhastheoverallresponsibilityfortheadministrationof
thiscontract.TheCOalone,withoutdelegationisauthorizedto:
Amend,modify,ordeviatefromthecontract;
RequestremovalofunsuitableContractorpersonnel;
Orderre‐performanceofunacceptableworkorperformancebyothermeans.
Otherresponsibilitiesmaybedelegatedtoauthorizedrepresentatives.

B. Contracting Officer’s Representative (COR): The COR will be designated in writing at the time of

awardtoassisttheCOintheday‐to‐dayon‐siteadministrativeofthecontractrequirements.The
responsibilitiesoftheCORinclude,butarenotlimitedtothefollowing:
DeterminingthecomplianceandadequacyofperformancebytheContractor;
EnsuringadequacyofscheduleandQualityAssuranceSurveillancePlan(QASP),aswellas

coordination of changes to the schedule and QAP if necessary.

III. 9. GOVERNING REGULATIONS, HANDBOOKS AND REFERENCES:

A. The Contractor shall provide Health and Wellness (H&W) services as specified in the following documents:

III. 11. CONTRACTORS PERFORMANCE ASSESSMENT REPORTING SYSTEM (CPARS)

InaccordancewithFAR42.1502–Pastperformanceevaluationshallbepreparedatleastannuallyandat
thetimetheworkunderacontractororderiscompletedforeachcontractand/ororderthatexceedsthe
simplifiedacquisitionthreshold(SAT).Thisincludesanycontractand/ororderthatisbelowtheSATand
ismodified,whichresultsinthecontractexceedingtheSAT.
(a)FAR42.1502directsallFederalagenciestocollectpastperformanceinformationon
contracts.TheDepartmentofAgriculture(USDA)hasimplementedtheContractorPerformance
AssessmentReportingSystem(CPARS)tocomplywiththisregulation.Oneormorepastperformance
evaluationswillbeconductedinordertorecordyourcontractperformanceasrequiredbyFAR42.15.
(b)ThepastperformanceevaluationprocessisatotallypaperlessprocessusingCPARS.CPARSis
aweb‐basedsystemthatallowsforelectronicprocessingoftheperformanceevaluationreport.Oncethe
reportisprocessed,itisavailableinthePastPerformanceInformationRetrievalSystem(PPIRS)for
Governmentuseinevaluatingpastperformanceaspartofasourceselectionaction.
(c)WerequestthatyoufurnishtheContractingOfficerwiththename,positiontitle,phone
number,andemailaddressforeachpersondesignatedtohaveaccesstoyourfirm’spastperformance
evaluation(s)forthecontractnolaterthan30daysafteraward.Eachpersongrantedaccesswillhavethe
abilitytoprovidecommentsintheContractorportionofthereportandstatewhetherornottheContractor
agreeswiththeevaluation,beforereturningthereporttotheAssessingOfficial.Thereportinformation
mustbeprotectedassourceselectionsensitiveinformationnotreleasabletothepublic.
(d)WhenyourContractorRepresentative(s)(PastPerformancePointsofContact)areregisteredin
CPARS,theywillreceiveanautomatically‐generatedemailwithdetailedlogininstructions.Furtherdetails,
systemsrequirements,andtraininginformationforCPARSisavailableat
http://www.cpars.csd.disa.mil/.TheCPARSUserManual,registrationforOnLineTrainingforContractor
Representatives,andapracticeapplicationmaybefoundatthissite.
(e)Within60daysaftertheendofaperformanceperiod,theContractingOfficerwillcompletean
interimorfinalpastperformanceevaluation,andthereportwillbeaccessibleat
http://www.cpars.csd.disa.mil/.ContractorRepresentativesmaythenprovidecommentsinresponseto
theevaluation,orreturntheevaluationwithoutcomment.Commentsarelimitedtothespaceprovidedin
Block22.YourcommentsshouldfocusonobjectivefactsintheAssessingOfficial’snarrativeandshould
provideyourviewsonthecausesandramificationsoftheassessedperformance.Inadditiontotheratings
andsupportingnarratives,blocks1–17shouldbereviewedforaccuracy,astheseincludekeyfieldsthat
willbeusedbytheGovernmenttoidentifyyourfirminfuturesourceselectionactions.Ifyouelectnotto
providecomments,pleaseacknowledgereceiptoftheevaluationbyindicating“Nocomment”inBlock22,
andthensigninganddatingBlock23oftheform.WithoutastatementinBlock22,youwillbeunableto
signandsubmittheevaluationbacktotheGovernment.IfyoudonotsignandsubmittheCPARwithin60
days,itwillautomaticallybereturnedtotheGovernmentandwillbeannotated:“Thereportwas
delivered/receivedbythecontractoron(date).Thecontractorneithersignednorofferedcommentin
responsetothisassessment.”Yourresponseisduewithin60calendardaysafterreceiptoftheCPAR.

(f) The following guidelines apply concerning your use of the past performance evaluation:

(1)Protecttheevaluationas“sourceselectioninformation.”Afterreview,transmitthe
evaluationby

completingandsubmittingtheformthroughCPARS.Ifforsomereasonyouareunableto
viewand/orsubmittheformthroughCPARS,contacttheContractingOfficerfor

instructions.

(2)Strictlycontrolaccesstotheevaluationwithinyourorganization.Ensurethe
evaluationisneverreleasedtopersonsorentitiesoutsideofyourcontrol.
(3)Prohibittheuseoforreferencetoevaluationdataforadvertising,promotionalmaterial,
pre‐awardsurveys,responsibilitydeterminations,productionreadinessreviews,orother
similarpurposes.
(g)Ifyouwishtodiscussapastperformanceevaluation,youshouldrequestameetingin
writingtotheContractingOfficernolaterthansevendaysfollowingyourreceiptofthe
evaluation.Themeetingwillbeheldinpersonorviatelephoneorothermeansduringyour
60‐dayreviewperiod.

(h) A copy of the completed past performance evaluation will be available in CPARS for your viewing and for Government use supporting source selection actions after it has been finalized.

III. 12. QUALITY ASSURANCE SURVEILLANCE MATRIX:

A. In accordance with FAR 37.401, the Government will evaluate the quality of professional and

administrativeservicesprovided,butretainsnocontroloverthemedical,professionalaspectsof
servicesrendered(e.g.professionaljudgments,diagnosisforspecificmedicaltreatment,etc.)
