PERFORMANCE WORK STATEMENT

1.INTRODUCTION:TheUSDAForestServiceJobCorpsCivilianConservationCenters(JCCCC)provide
academic,vocational,andlife‐learningskillstoatriskyouthandyoungadultsat28JobCorpsCentersacrossthe
UnitedStates(U.S.).Aspartofthisprogram,youthandyoungadultsattheCentersareprovidedhealthcare
servicesasfollows:medical,dental,mentalhealth,anddrug&alcoholintervention.EachForestServiceJCCCChas
aneedforbasichealthservicesprovidedbylicensedmedicalprofessionalsthatincludesexaminationsand
assessments,treatment,issuanceofprescriptionsandimmunizations,monitoringhealthconditions,
recommendinghospitalization,follow‐upcare,andpreventionplanningandintervention.
2.BACKGROUND:JobCorpsisaU.S.DepartmentofLabor(DOL),EmploymentandTrainingAdministration
(ETA)program.JobCorpsisdesignedtoassistyoungpeoplewhoneedandcanbenefitfromthewide‐rangeof
servicesprovidedataJobCorpscenter.Servicesincludethefollowing:basiceducation,HighSchoolDiploma
(HSD)program;GeneralEducationalDevelopment(GED);vocationalskillstraining;residentiallivingprogram;
healthcare;andrelatedsupportservices.Thecomprehensivecombinationoftrainingandsupportservicesis
designedtohelpeachstudentbecomearesponsibleandproductivecitizen.Studentpopulationsrangefrom100
toover300enrollmentsdependingontheCenter’sOnBoardStrength(OBS).JobCorpsisaself‐pacedprogram:
therefore,astudent’slengthofenrollmentvaries.Studentsmayremainintheprogramforuptotwoyears,butthe
averagelengthofenrollmentisapproximatelyeightmonths.

3. CONTRACT EFFORT REQUIRED:

a. Position:PhysicianServicesarerequiredonaweeklybasistoassiststudentsinattainingandmaintaining
optimalhealthwiththeservicesbeingprovidedattheCenterorphysician’sofficeasdeterminedbythe
ContractingOfficer(CO),ContractingOfficer’sRepresentative(COR),andContractor.Physicianservices
providedmustcomplywithJobCorpspolicies,requirements,andguidelines,andmustalsoincludeanalysis
anduseofthemostefficientandcost‐effectivepracticeofcontemporary,evidence‐basedhealthcarepossible
givenavailabletime,resources,andpersonnel.TheseserviceswillinvolvethemonitoringoftheHealthand
Wellness(H&W)programtoensurethatstudentsacquirethehealth‐relatedskills,knowledge,andattitudes
thathelpmakethememployable;coordinatingandintegratingtheBasicH&WprogramwithotherH&W
components/activitiesonCenter;andprovidingbasichealthcaretoenrolleesandestablishedstudentson
Center.Provisionofphysicianservicesrequires,butisnotlimitedto:

theabilitytoworkbothindependentlyandasamemberofahealthcareteamtoprovideoptimal
servicestoapopulationoffinanciallydisadvantagedyouth,rangingfrom16to24yearsinage;

ensuring patient privacy for all examinations, procedures, and consultations as mandated by

HIPPA;

providingwrittendiagnosesandtreatmentplansaspartofaChronicCareManagementPlanfor
studentswithchronicconditions/disabilities
providingon‐centerimmunizationofenrolleesforpreventionandcontrolofcommunicabledisease
asspecifiedinPRH‐6,Appendix601andTAGB,ImmunizationandCommunicableDiseaseControl;
providingmaintenanceofchronicconditions;treatmentofacuteconditions;promotionand
maintenanceofafamilyplanningprogramforstudents(includingprescriptionbirthcontrol

medications);

establishinganappropriatereferralsystemtoensurethereceiptofspecialtycarebystudents,as
definedintheDOLJobCorpsPolicyandRequirementsHandbookandwithinbudgetconstraints;
ensuringavailabilityfor24‐houremergencycarebyphoneconsultation;
wherefeasible,providingphysicianservicestothosestudentswhorequirehospitalization;and
providingstudentservicesinsuchamannerastoobtainpositiveStudentSatisfactionSurvey(SSS)

results as determined by Job Corps.

Additional services required in support of the Job Corps Center’s Health and Wellness Program follow:

Maintain,safeguard,andensuretheconfidentialityofallrequiredstudenthealthrecordswhenthey
areinthehealthclinicorinthepossessionofhealthandwellnessteammembersaccordingtothe
federalHealthInsurancePortabilityandAccountabilityAct(HIPAA);
Develop,support,andensurecompliancewithinfectioncontrolpoliciesandprocedures,per
OccupationalSafetyandHealthAdministration(OSHA)andotherJobCorpssafetyguidelines;
ParticipateintheReasonableAccommodationCommittee(RAC)meetingsandfileapplicantreview
teammeetingsasrequested;
ReviewtheJobCorpsHealthandWellnesswebsite(http://jobcorpshealth.com)andattendJob
Corps’physicianconferencecalls,webinars,andDOLphysicians’trainingasmayberequiredthat
doesnotinvolveactualtravel.
Ensurethatregularpreventivemaintenanceisperformedonthemedicalequipmentthroughthe
HealthandWellnessManager(HWM);
Donotengageinanyresearchprojectsand/orpublicationsrelatingtoJobCorpshealthactivities
withoutpriorwrittenapprovalfromtheJobCorpsNationalOfficeinGolden,CO;
Adviseandassistthehealtheducationinstructorinthedevelopmentandimplementationofthe
healtheducationprogram;
ReportadversestudentbehaviorincidentstotheHWMorContractingOfficer’sRepresentative;
understandthatmedicalservicesshallbesubjecttomonitoringandreviewbyJobCorps

assessment teams and that cooperation with such teams is required.

b. ScheduleandAvailability:Physicianservicesarerequiredonaweeklybasis,duringdaysoftheweekand
hoursofthedayestablishedincoordinationwithCenterpersonnel.BasedontheCenter’scurrentOBS,the
ContractormustprovideatleasttheMinimumStaffingRequirementsspecifiedintheProgram
RequirementsHandbook(PRH).CenterOBSandthePRHmaychangeatanytime;consequently,the
quantityofrequiredphysicianserviceshoursmayvary,dependingontheneedsofthestudentpopulation.
Additionalhoursmaybeneededonanoccasionalbasis,andwillberequestedinwritingbytheContracting
Officer(CO)inadvance.

NOTE:ThePRHcurrentlyrequiresthathalfthephysicianservicesprovidedbeprovidedbyalicensed
physician,andtheotherhalfbeprovidedbyaphysicianassistantornursepractitioner.
TheContractorisrequiredtoprovide24hoursadvancednoticetocancelscheduledservices.WhenKey
Personnelareunavailable,theContractorwillprovideanacceptablesubstitutetoprovideservices.
SubstitutepersonnelmustbeapprovedinadvancebytheCO.
KeyPersonnelmustbeavailablebytelephone24hoursperday,7daysperweektoprovideemergency
services.KeyPersonnelmustprovideacurrentemergencycontactnumbertotheCenterHSM.WhenKey
Personnelis/areunavailable,theContractorwillprovidetotheCenterHSMacurrentemergencycontact
numberofanacceptablesubstitutetoprovideemergencyservices.
PhysicianserviceswillnotbescheduledonfederalGovernmentholidaysandarenotrequiredduringthe
periodofCenter‐widestudentleave(approximatelytwoweeks,twiceyearly.Inaddition,physician
servicesmaybelimitedduringtheonetotwoweeksimmediatelyprecedingandfollowingeachofthese

breaks.

4. QUALIFICATIONS OF CONTRACTOR PERSONNEL:

Contractor personnel:

mustberegistered/licensedwiththestateinwhichphysicianserviceswillbeprovided;alllicensing
requirements/certification/accreditationofthecontractor’sstaffmustbemaintainedduringtheentire
performanceperiodwithproofbeingsubmittedattherequestoftheGovernment;

are responsible for maintaining their own malpractice/professional liability insurance and general liability insurance;

mustprovideacurrentDrugEnforcementAdministration(DEA)registrationnumbertotheCOandHWM
foruseinprocuringnecessarysuppliesinsupportoftheworkrequirements;
ifpossible,musthaveadmittingrightstoahospitalwithinareasonabletravelingdistancefromthecenter
toensureabilitytoprovideservicestohospitalizedcenterstudents;

may be subject to a criminal background check and drug testing as prerequisites for contract performance.

5. MATERIALS, EQUIPMENT, TRAINING, AND UTILITIES:

A.TheGovernmentshallfurnishthefollowing:Adequateworkspace,equipment,supplies,and
materialsasneededtoaccomplishtheworkrequirementsoutlinedherein,includingexaminationrooms
andfurniture,assessmentequipment,telephone,computer,afaxmachine,prophylaxissupplies,gloves,
masks,andsimilaritems(unlessservicesareprovidedatthephysician’soffice).Prescriptionandover‐the‐
countermedicationswillbeprovidedattheCenter.Thesesuppliesandpropertyshallbeutilizedduring
theperformanceofthiscontractonlywhileprovidingstudentcare.Thecontractorwillprovidean
itemizedlistofitemsneededtotheHWMatleast30daysinadvanceofthedesireddeliverydatetoallow
foradequatetimefororderingandreceiving.
B.TheContractorshallfurnishthefollowing:Alllabor,services,permits,licenses,transportationof
Contractorpersonnel(asnecessary),andincidentals(otherthanthoseprovidedbythefederal
Government)asneededtofulfilltheworkrequirementsherein,forthehoursrequiredbasedonthe
Center’sOnBoardStrength(OBS).Inaddition,theContractorshallinsurethatallContractorpersonneland
theiragentsarecoveredbyprofessionalliability(malpractice)insuranceandgeneralliabilityinsurance

duringtheentireperformanceperiodandagreestoindemnifyandholdharmlesstheJobCorpsCenter
fromanyactionstakenby,for,oragainsttheContractorortheiragentsthatcouldresultinadditional
expenses,fees,orlawsuits.
C.Security:Thespace,equipment,andsuppliesusedbycontractorpersonnelMUSTbesecuredatall
times,perCenterrequirements.TheContractorshallreviewsecurityprocedureswiththeCenterHWM
andCOR.

6. RECORDS, REPORTS, AND REQUIRED SUBMITTALS:

The following submittals are required during the period of performance of this contract:

Documentationofallexaminations,tests,treatments,medications,referrals,etc.onapprovedJobCorps
HealthandWellnessProgramformstobemaintainedinStudents’HealthRecords(SHR);
Reviewofandauthorizationofstandingordersforhealthcareannually,inaccordancewithTechnical
AssistanceGuideM:HealthCareGuidelines;and
ReportstotheCenterDirector(CD)andHealthandWellnessManager(HWM)onallhealth‐related
mattersincludingstudentmedicalseparations,justificationsforandcostsofspecialservices,Healthand
Non‐HealthCareGuidelines,equipmentrepairandreplacementneeds,depletionofmedicalsuppliesand
medications,andotherareasdeemedappropriate.
ThefederalGovernmentreservestherighttohavetheJobCorpsRegionalHealthCoordinatorandother
authorized/qualifiedpersonnelconductauditsofContractorservicesandstudentmedicalrecordstoverifythat
qualityservicesarebeingperformed.AcopyofanyauditreportshallbeprovidedtotheContractorand
ContractingOfficer(CO)withintwoweeksoftheaudit.Withintwoweeksafterreceiptoftheauditreport,the
ContractorshallsubmitaperformanceplanforimprovingservicestotheContractingOfficer(CO)forapproval.
TheContractorshallimplementtheapprovedperformanceplanimmediately.

7. THE ROLE OF GOVERNMENT PERSONNEL AND RESPONSIBILITY FOR CONTRACT ADMINISTRATION:

A.ContractingOfficer:TheContractingOfficer(CO)hastheoverallresponsibilityforthe
administrationofthiscontract.TheCOalone,withoutdelegation,isauthorizedto:
Amend,modify,ordeviatefromthecontract;
RequestremovalofunsuitableContractorpersonnel;
Orderre‐performanceofunacceptableworkorperformancebyothermeans.

Other responsibilities may be delegated to authorized representatives.

B.ContractingOfficer'sRepresentative(COR):TheCORwillbedesignatedinwritingatthetimeof
awardtoassisttheCOintheday‐to‐dayon‐siteadministrationofthecontractrequirements.The
responsibilitiesoftheCORinclude,butarenotlimitedtothefollowing:
DeterminingthecomplianceandadequacyofperformancebytheContractor;
EnsuringadequacyofscheduleandQualityAssurancePlan(QAP),aswellascoordinationof

changes to the schedule and QAP if necessary.

8. GOVERNING REGULATIONS, HANDBOOKS AND REFERENCES:

9. QUALITY ASSURANCE SURVEILLANCE MATRIX:

InaccordancewithFAR37.401,theGovernmentwillevaluatethequalityofprofessionalandadministrative
servicesprovided,butretainsnocontroloverthemedical,professionalaspectsofservicesrendered(e.g.
professionaljudgments,diagnosisforspecificmedicaltreatment,etc.)
