NOTE TO OFFEROR/PRICE SCHEDULE/STATEMENT OF WORK

1. NOTE TO OFFEROR:

In order for an offeror to have a complete quotation package, the following must submitted to the Contracting Officer to be considered responsive:

□ A copy of this document with line item prices and quote validity period completed.

□ Copy of a completed provision 52.212-3, Offeror Representations and

Certifications—Commercial Items. An offeror shall complete only paragraph

(l) of this provision in Attachment 1 if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs

(b) through (k) of this provision.

3. STATEMENT OF WORK - BLOOD SAMPLE COURIER

Scope of Work:

Transport blood samples from XL Four Star Beef, Nampa, ID to the Animal Health Lab, Boise, ID for Brucellosis testing.

Work Requirements:

Provide a daily pick up of blood samples from XL Four Star Beef, 3611 E Amity Avenue, Nampa, Idaho and deliver to the Animal Health Lab, 2230 Old Penitentiary Road, Boise, Idaho. Pick up samples at approximately 4:00 pm and deliver to the Animal Health Lab by 5:00 pm the same day. The courier will enter the plant, locate and load the samples. The samples are stored and transported in barrels weighing approximately 80lbs. The samples are contained in one barrel, but there may be rare instances when the delivery would be two barrels. The courier must be willing to wait for the blood samples if they are not ready at the time of pick up. Wait times can vary from 15 – 45 minutes. If delivery is delayed past 5:00 pm the courier must coordinate with the Animal Health Lab staff to ensure timely same day delivery. The courier will collect empty barrels and supplies from the lab and deliver to XL Four Star Beef the following day. Proper pick up, treatment and delivery of these blood samples is critical to the viability and testing of the samples for Brucellosis. Anticipated time to accomplish pickup and delivery is approximately 2.5 – 3 hours per day. Work is performed Monday through Friday, every day the plant is open. However, occasional Saturday work is also required. The requirement for Saturday work will be determined by Wednesday of each week, and a telephone number is available for the courier to find out the Saturday operational status.
