Statement of Work
HIA – Aggregate Crushing CAD SOO PIT
STATEMENT OF WORK - SPECIFICATIONS
SCOPE OF CONTRACT
The work consists of crushing and stockpiling aggregate surfacing material on the Hiawatha National Forest in the CAD SOO Pit, T47N, R4W, Section 36, Bay Mills Township, Chippewa County. The aggregate shall be processed to produce material meeting the specifications and gradation of MDOT 23A. The quantity, unit price, and awarding of this contract will be based on the amount of material the contractor proposes to produce for $41,000.00. Work will include providing and furnishing all supplies, supervision, labor, equipment, transportation, plant, incidentals and materials necessary to furnish the completed product unless specifically listed in these specifications.
CONTRACT TIME
Complete all work and demobilize from the site by August 31, 2017.
PREWORK MEETING
Contact the Government at least 2 weeks prior to mobilizing to the work site. A pre-work meeting will be held with the Government and Contractor to review contract requirements and to identify locations for excavation, stockpiles, and equipment.
PIT DEVELOPMENT AND RESTORATION
The Contractor shall perform all clearing and grubbing as necessary. Slash shall be scattered at locations approved by the Government. Trees of merchantable value will have been removed by others prior to start of this contract. Under no circumstances shall the Contractor excavate banks that will cause standing trees to fall into the pit.
Dispose of all refuse and waste materials in accordance with existing Federal, State and local laws. Keep and utilize the pit area in a manner that is safe to the public and the environment. Keep the work area clean and free of unnecessary clutter. Restore the work area to as near the original condition as practicable. Restore access routes and equipment travel ways to their pre-work condition; level the pit bottom. Final bank sloping will be restored to 2:1 or flatter.
FUEL STORAGE
The Contractor shall not locate any fuel storage tanks within the floor of the excavation or within 200 feet of any surface water. Fuel shall be stored in approved fuel tanks. Fuel storage tanks shall be either doubled walled or be located in a bermed area covered with two layers of 8-mil polyethylene film. The bermed area shall be sufficient in size to hold 1.5 times the volume contained within the storage tank. The polyethylene film shall be of sufficient size so that there area no seams below the secondary containment level. Clean water shall be pumped out within 24 hours of a rain event. The Contractor shall maintain the integrity of the secondary containment system for as long as fuel is stored on the site. The Contractor may submit other methods of providing secondary containment for review and approved by the Government. In no case shall the Contractor store on site more than 250 gallons of petroleum products or waste at any given time, excluding quantities contained in vehicles or other operating equipment.
The Contractor shall have onsite sufficient amount of fuel/oil absorbing material to absorb up to 25 gallons of spilled petroleum products. The Contractor shall have onsite the names and phone numbers of at least one contractor, who is qualified to respond and clean up fuel or other petroleum spills and maintains a 24-hour seven-day a week response service. These numbers shall be given to the Government at the pre-work meeting. The Contractor is responsible for the clean up of all spills to current MDNR-E and Forest Service standards.
HANDLING AND STOCKPILING AGGREGATE
The floor of each stockpile area will be leveled by the Government and surveyed by the Government prior to placing material. Construct stockpiles in layers up to 6’ maximum depth. Deposit aggregate material in such a manner as to prevent segregation (cone shape shall not be permitted where aggregate can run down long slopes). The stockpile shall be neat and regular in form and shall be made to occupy the smallest feasible area.
No track type of equipment will be allowed on the stockpiles. No remixing of the material is allowed by heavy equipment.
No independent sale of gravel to third parties is allowed without a gravel permit being issued by the Government.
MATERIALS
Process aggregate to meet specification MDOT 23A. Mineral filler in addition to that naturally present in the working face of the bank may need to be supplied by the contractor to meet the gradation requirements. The source for this material is the responsibility of the contractor.
GOVERNMENT FURNISHED PROPERTY
There is no government furnished property or material for this contract other than the native aggregate material located in the specified pit location.
TESTING AND QUALITY CONTROL
Contractor shall be responsible for the quality of the material produced. Sampling and testing by the contractor is required to assure that the material will meet the required specification. Written results of sieve analysis performed by the contractor will be submitted to the Government on a daily basis when production is taking place. The Government will take samples for its own use and may test the material for verification throughout the crushing process.
SUBMITTALS
Contractor shall submit at the pre-work meeting:
1. List of emergency phone numbers (fire, police, 911, hospital, spill response, etc.)
2. List of personnel and personnel phone numbers, pager, cell numbers, etc.
3. Schedule of start and completion dates.
4. Type of crusher being used (cone, jaw, etc.), HP, production rates, scale info, operating speeds, etc.
MEASUREMENT AND PAYMENT
The Government will measure the completed stockpile. Final payment quantity will be the total volume in cubic yards based on the Government’s measurements; however the maximum that can be earned by the contractor is $41,000. The final payment will not be adjusted for compaction in the completed stockpile. Method used to calculate the volume will be prismoidal or average end area method. The Government will furnish measurements to the Contractor upon request. Additional measurements requested by the Contractor are to be paid for by the Contractor.
As an alternate measure for payment the Government may decide to accept the contractors weight measurements. This method of measure would have to be agreed to in writing at the prework meeting, including a factor to use in converting tons to cubic yards. Certification of the scales used must be furnished.
