Instructions
General: The “ARRA-Performance Progress Report” form (SF-PPR-Recovery and SF-PPR-Recovery-B) is to be used to only report on activities and use of funds (aka, “recovery funds”) under the American Recovery and Reinvestment Act of 2009, Public Law 111-5 (“Recovery Act”). The report is to be submitted within ten
(10) days after each calendar quarter.
Item Data Elements Instructions
1.
Awarding Federal agency and Organizational Element to Which Report is Submitted
Enter the name of the awarding Federal agency and organizational element identified in the award document or otherwise instructed by the agency. The organizational element is a subagency within an awarding Federal agency.
2.
Federal Grant or Other Identifying Number Assigned by the awarding Federal agency
Enter the grant/award number contained in the award document.
3a. DUNS Number Enter the prime recipient organization's Data Universal Numbering System (DUNS) number or Central Contract Registration extended DUNS number.
3b. EIN Enter the recipient organization's Employer Identification Number (EIN) provided by the Internal Revenue Service.
3c. CFDA Enter Catalog of Federal Domestic Assistance (CFDA) number on the award document or provided by the awarding agency
4. Recipient Organization Enter the name of recipient organization and address, including zip code.
This should be the same name and address that appears in recipient’s Central Contract Registration profile.
5.
Recipient Account Number or Account Number
Enter the account number or any other identifying number assigned by the recipient to the award. This number is strictly for the recipient's use only and is not required by the awarding Federal agency.
6. Project/Grant Period
Indicate the project/grant period established in the award document during which Federal sponsorship begins and ends. Note: Some agencies award multi-year grants for a project/grant period (e.g., 5 years) that are funded in increments known as budget periods or funding periods. These are typically annual increments. Please enter the project/grant period, not the budget period or funding period.
7.
Reporting Period End Date
The frequency of required reporting is quarterly. Enter the ending date of the reporting period. For quarterly reports, the following calendar quarter reporting period end dates shall be used: 6/30; 9/30; 12/31, or 3/31;. For final PPRs, the reporting period end date shall be the end date of the project/grant period.
8. Final Report Mark appropriate box. Check “yes” only if this is the final report for the project/grant period specified in Box 6.

Section 1 Project / Activity Information Please provide requested information for each project of activity for which recovery funds were expended or obligated.

ARRA-1-01 Name of Project or Activity Enter a brief descriptive title of the project.
ARRA-1-02 Total Amount of Recovery Funds Received from Federal Agency Indentified in Block 1:
Enter the cumulative amount of actual recovery funds (cash) received from the Federal agency as of the reporting period end date.
ARRA-1-03 Amount of recovery funds received that were obligated or expended to projects or activities:
Enter cumulative total for the amount of Federal fund expenditures. For reports prepared on a cash basis, expenditures are the sum of cash disbursements for direct charges for property and services; the amount of indirect expense charged; the value of third-party in-kind contributions applied; and the amount of cash advance payments and payments made to subccontractors and subawardees. For reports prepared on an accrual basis, expenditures are the sum of cash disbursements for direct charges for property and services; the amount of indirect expense incurred; the value of in-kind contributions applied; and the net increase or decrease in the amounts owed by the recipient for (1) goods and other property received; (2) services performed by employees, contractors, subcontractors, subawardees, and other payees; and (3) programs for which no current services or performance are required. Also include program income expended in accordance with the deduction alternative, rebates, refunds, or other credits.

Section 2 Project / Activity Information Please provide requested information for each project of activity for which recovery funds were expended or obligated.
ARRA-2-01 Name of Project or Activity Enter a brief descriptive title of the project.
ARRA-2-02 Description of Project or Activity
Select activity code(s) from list in the instructions.
ARRA-2-03 Evaluation of completion status of the project or activity.

Options include not- started, Less than 50% completed, Completed 50% or more, Fully Completed
ARRA-2-04 Estimate of the number of jobs created by this project or activity.
Provide estimated number and/or narrative description of jobs created. For estimated number, include those new positions created and filled, or previously existing unfilled positions that are filled, as a result a result of funding by the American Recovery and Reinvestment Act (ARRA).
This definition covers only positions established in the United States and outlying areas (Outlying areas” means—
(1) Commonwealths.
(i) Puerto Rico.
(ii) The Northern Mariana Islands;
(2) Territories.
(i) American Samoa.
(ii) Guam.
(iii) U.S. Virgin Islands; and
(3) Minor outlying islands.
(i) Baker Island.

The number shall be expressed as “full-time equivalent” which shall include full-time, part- time, temporary, permanent, positions as expressed as a “person-year,” consistent with the recipient’s existing personnel procedures. This includes positions at the prime recipient’s level, and the prime recipient’s estimate of

ARRA-2-05 Estimate of the number of jobs retained by this project or activity.
Provide estimated number and/or narrative description of jobs created. For estimated number, provide an estimate of those previously existing filled positions that are maintained and an estimate of previously filled positions that are maintained as a result of funding by the American Recovery and Reinvestment Act. This definition covers only positions established in the United States and outlying areas. The number shall be expressed as “full-time equivalent” which shall include full-time, part- time, temporary, permanent, positions as expressed as a “person-year,” consistent with the recipient’s existing personnel procedures. Purposes:
(1) To preserve and create jobs and promote economic recovery.
(2) To assist those most impacted by the recession.
(3) To provide investments needed to increase economic efficiency by spurring technological advances in science and health.
(4) To invest in transportation, environmental protection, and other infrastructure that will provide long-term economic benefits.
(5) To stabilize State and local government budgets, in order to minimize and avoid reductions in essential services and counterproductive state and local tax increases.

ARRA-2-09 For State/local infrastructure investment:

Provide name, phone number, address and email address of the appropriate contact in the state/local government.
ARRA-2-10 NEPA Compliance Status The state of National Environmental Policy Act (NEPA) compliance will be reported either as (1) completed with the type of NEPA compliance, or (2) in progress as follows:
“NEPA is not triggered – For example: No NEPA because no agency discretion for (descriptive name and statutory citation for the activity or program).
“Categorical Exclusion: cite CE used and date completed (the date will be the date of the document when agency NEPA procedures require documentation, or a date after the determination has been made that there are no extraordinary circumstances).
“Environmental Assessment (EA): identify the EA and provide: (1) date of public involvement as practicable, if not practicable then date interagency involvement; (2) date of notice of Finding of No Significant Impact (FONSI) (if required); or (3) date EA/FONSI are completed.
“Environmental Impact Statement (EIS): identify the EIS and provide: (1) date of Notice of Intent: (2) date of EPA published notice of availability of the draft EIS; (3) date of EPA published notice of availability of the final EIS; (4) date the Record of Decision is signed; or (5) appropriate date of any subsequent supplemental NEPA review and documentation.”

ARRA-3-11 For the five most highly compensated officers of the entity: the names and total compensation
Provide the names and total compensation of the five most highly compensated officers of the recipient entity if--
(1) the recipient in the preceding fiscal year received--
(a) 80 percent or more of its annual gross revenues in Federal awards; and
(b) $ 25,000,000 or more in annual gross revenues from Federal awards; and
(2) the public does not have access to information about the compensation of the senior executives of the entity through periodic reports filed under section 13(a) or 15(d) of the Securities Exchange Act of 1934 (15 U.S.C.
78m(a), 78o(d)) or section 6104 of the Internal Revenue Code of 1986 [26 USCS § 6104].
“Total Compensation” means the complete pay package of each of the recipient’s compensated officers, including all forms of money, benefits, services, and in-kind payments, consistent with the regulations of the Securities and Exchanges Commission at 17 CCR 229.402.
