PERFORMANCE WORK STATEMENT (PWS)
For
Animal Feed 15 August 2013
1. SECTION I – DESCRIPTION OF SERVICES. The purpose of this Performance Work Statement (PWS) is to define the requirements for the contractor to provide Animal Feed to include delivery at the United States Department of Agriculture (USDA) located at 10300 Baltimore Avenue, BARC-East BLDG. 177C Beltsville, MD 20705
1.1. PERIOD OF PERFORMANCE. The Period of Performance for this effort consists of a base period and four 1-year options:
Base Period:
26 September 2013 through 25 September 2014 Option One: 26 September 2014 through 25 September 2015 Option Two:
26 September 2015 through 25 September 2016
1.2. COMPLIANCE WITH REGULATIONS. The Contractor shall perform all work in accordance with:
1.2.1. Federal and Maryland laws, regulations, codes, licenses and directives;
1.3. Performance Objectives for Delivery and Quality of Animal Feed.
1.3.1. Ordering. The Contractor will deliver requested animal feed within 4 days of receiving a call from the designated USDA person or persons. Personnel will be named by either position or name and are the only personnel authorized to order animal feed.
1.3.2. EQUIPMENT. The Contractor shall provide all equipment to perform delivery of animal feed to the appropriate location. Deliveries will be made in clean, closed vehicles not used in the transportation of livestock. The Contractor shall proactively maintain and replace all required equipment. Knowingly using non-functioning or unserviceable equipment may present a grave security risk to contractor and government personnel as well as the general public. The COR has the authority to identify equipment as unserviceable.
1.3.3 QUALITY STANDARD.
1.3.3.1 The contractor shall assure that the material conforms to the specifications in the contract prior to delivery.
1.3.3.2 Animal feed will meet the criteria listed on the attachment 1. Type of feed and minimum mineral ingredients.
1.3.3.3 Each bag must be clearly marked with a date of manufacturing (milling date).
1.3.3.4 Each bag shall contain a tag of guaranteed analysis, ingredients and feeing instructions.
1.4.2.7 Items purchased under this contract will be used to feed animals in the conduction of research. Since diet is an essential control factor necessary in the Center’s research, no substitution or deviation from the formulations is permissible without prior approval from the USDA.
1.3.4 SCHEDULE. Services shall be on an as needed basis as scheduled by the COR. All deliveries shall be performed between the hours of 7:00 AM and 2:30 PM, Monday through Friday, excluding Federal holidays. Scheduling of deliveries shall be coordinated with the COR or designated USDA representative for that location.
1.3.5 CAUTION. The contractor shall demonstrate caution to protect all public buildings, facilities and planted areas.
1.3.6 SAFETY. The contractor shall follow all safety rules during delivery of animal feed.
1.4 DELIVERABLES
1.4.1 PRICE LISTS. All Price Lists will be updated annually or upon request of the contracting officer.
1.4.2 DELIVERY-ANIMAL FEED.
1.4.2.1 All requested animal feed shall be delivered within 4 days of request.
1.4.2.2 Upon delivery, at destination, the Government reserves the right to reject any or all parts of the shipment, if in the opinion of the Government, does not meet the requirements contained herein.
1.4.2.3 Bags that are torn sol that contents are exposed on arrival at the BARC-E will not be accepted for use.
1.4.2.4 Bags shall be loaded and delivered on returnable or disposable pallets so that the items can be unloaded by a government forklift operator at the government facility. If returnable pallets are used they shall be provided at no additional cost to the government and the contractor shall be responsible for pick up and return of these pallets after delivery.
1.4.2.5 The Contractor shall provide the Government with a certified copy of both the empty and loaded weight tickets at the time of each delivery for all bulk feed deliveries, which must be obtained at official weighing station with the firm’s name inscribed on the weigh ticket
1.4.2.6 A delivery ticket must accompany each order (allowing for signature of person making the delivery and signature of Government personnel receiving the delivery).
1.5 INVOICES
1.5.1 INVOICING. All monthly invoices shall be submitted to the COR or designated USDA representative and the contracting officer. Invoices are submitted on a monthly basis.
1.5.2 TRACKING. USDA BARC-EAST shall develop a tracking log for all calls annotating date called, product received, invoice number, cost, remaining cost and any other pertinent information to keep track of orders and invoices.
1.5.2 INVOICE PROCESSING. USDA BARC-EAST shall review and accept invoices and submit to the contracting officer or designated person for processing within 3 days of receipt.
1.6 Cancellation Either party may request cancellation of this agreement with a written 30 day advance notice to the contracting officer.
