ATTACHMENT 9 – PAST PERFORMANCE QUESTIONNAIRE (PPQ)

Brief Description of Work Being Procured:
The United States Patent and Trademark Office (USPTO) intends to acquire Patent Search Services for bio-sequences, chemical structures, and general text search services in order to meet the USPTO mission, which is the examination, granting and dissemination of Patents.
In order to accurately evaluate the offeror’s past performance, your assistance is requested in obtaining and verifying the past performance history of this offeror.
In order to assess potential offerors under USPTO Solicitation Number 1333BJ20R00151003, the USPTO contracting office is obtaining past performance information with respect to each offeror in the performance of the work described above. As a reference for work performed by the firm identified below of this questionnaire, your candid response will assist in the evaluation process. Please complete the questionnaire as thoroughly as possible.
Please complete and sign. References are to submit a fully completed questionnaire to the Contracting Specialist, Mark Motta, with the Contracting Officer, Heather Watroba, cc’d, via email no later than the closing date and time of the proposal submission due date of 12:00 PM EST (noon) on December 4, 2019.
