STATEMENT OF WORK (SOW)

SPECIAL EDUCATION SERVICES FOR ORIENTATION/MOBILITY THERAPY

Tuba City Boarding School

May 5, 2022

1. BACKGROUND

This requirement falls under the Individuals with Disabilities Education Act (IDEA), 34 Code of Federal Regulations§ 300.323. IDEA requires a free appropriate public education to eligible children with disabilities throughout the nation and ensures special education and related services to those children.

2. SCOPE

Contractor shall provide Orientation/Mobility Services as follows:

✓ Review student's medical records, physician referral, child study team referral, parent request and/or school psychologist evaluation recommendation for further testing;

✓ Perform evaluation procedures to establish performance guidelines;

✓ Provide formal written comprehensive test results of student assessment 30 days from date of evaluation;

✓ Plan, prepare, and establish appropriate written Individual Education Plan OEP) goals and objectives;

✓ Attend student's scheduled IEP meeting;

✓ Administer and implement orientation/mobility program;

✓ Re-assess and revise program, as necessary, as status of student changes;

✓ Instruct and teach students, families and teachers as indicated on IEP;

✓ Make referrals to other health professionals and integrate orientation/mobility therapy program with other aspects of health care;

✓ If necessary, confer with physician and other health practitioners to obtain information and make recommendations and revisions in treatment program;

✓ Provide services, on-site or Flagstaff during the school year; as warranted by student need;

✓ Provide a progress report for individual students' to coincide with the school's grading periods.

Services shall be provided up to 2 times a month for up to 8 hours a day for 40 weeks throughout the school year and coordinated with the school.

3. KEY PERSONNEL MINIMUM REQUIREMENTS

The Contractor shall provide key personnel that meet or exceed the following minimum requirements. Evidence of meeting these requirements shall be provided to the Government with the Contractor's quote/proposal.

EDUCATION: Master's Degree in Orientation/Mobility Services

# OF YEARS OF DIRECTLY RELATED PROFESSIONAL EXPERIENCE: 5 years of experience, preferably with Native American students

CERTIFICATION(S): National Board for Certification in Orientation/Mobility Therapy

(preferred)

LICENSE(S): AZ State license (License in state where services are to be provided)

Replacement of key personnel will require prior coordination and approval of the school.

4. DELIVERABLES

The Contractors shall provide the following deliverables within the specified timeframe:

5. PLACE OF PERFORMANCE

Contractor shall provide services at Tuba City Boarding School which is located at 306 Main

Street or Flagstaff, AZ. Contractor may bill travel if included and funded in the contract.

Incurred costs to travel for performance shall be pre-approved by a Government authorized representative. Travel costs shall be billed at the current year's mileage and per diem rates in accordance with the Federal Travel Regulations (FTR).

6. PERIOD OF PERFORMANCE

The period of performance shall begin on August 1 2022 and continue through June 30, 2023 with 4 optional years that may be exercised unilaterally by the Contracting Officer if:

a. There is an ongoing requirement;

b. Performance is satisfactory;

c. Prices are reasonable based on current market rates; and

d. Funds become available for the option year.

7. SECURITY REQUIREMENTS

Contractor personnel shall present Government issued identification to the front desk and check in with the Special Education Coordinator/School Administrative Office prior to entering the school campus. Contractor's personnel are subject to background investigations and must successfully pass the background prior to performance under the contract.

8. PAST PERFORMANCE ASSESSMENT

The Government will evaluate the Contractor's performance under this contract which will be shared with other Government agencies and used as past performance references.

Orientation & Mobility Therapy Service Summary:

Contractor shall sign in at front office for attendance and cross referencing.

Invoice shall contain the following information:

a. Purchase Order Number

b. Date of services

c. Initial of students

d. Time Spent with students

e. Evaluation information to include student initials.

f. New Consultants shall comply with Background Investigation as required prior to commencement of any work to be performed. 25 U.S.C. 3201 (P.L. 101-630); Contact with or control over Indian children are subject to a background investigation and determination of suitability for employment.
