SF30, Block 14, Continued

a. Solicitation Section F.3.2, change as identified in red font below:

“Refer to Section F.4 below for identification of Legal Holidays.”

b. Delete solicitation Section G.3.4.4 in its entirety as there is no requirement to submit the invoice other than in IPP as stipulated in G.3.4.

c. Section H.17.2, Commercial General Liability Insurance, change as identified in red font below:

“...The Contractor shall carry all liability insurance for three years following the final acceptance of work under this contract.”

d. Section I, add the following FAR clause:

I.5 FAR 52.228-5 Insurance—Work on a Government Installation (Jan 1997)

(a) The Contractor shall, at its own expense, provide and maintain during the entire performance of this contract, at least the kinds and minimum amounts of insurance required in the Schedule or elsewhere in the contract.

(b) Before commencing work under this contract, the Contractor shall notify the

Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government’s interest shall not be effective—

(1) For such period as the laws of the State in which this contract is to be performed prescribe; or

(2) Until 30 days after the insurer or the Contractor gives written notice to the

Contracting Officer, whichever period is longer.

(c) The Contractor shall insert the substance of this clause, including this paragraph (c), in subcontracts under this contract that require work on a Government installation and shall require subcontractors to provide and maintain the insurance required in the Schedule or elsewhere in the contract. The Contractor shall maintain a copy of all subcontractors’ proofs of required insurance, and shall make copies available to the Contracting Officer upon request.

e. Solicitation Section L.5.3.1, change as identified in red font below:

f. Replace page 1 of Attachment J-6 with the attached revised Attachment J-6, page 1. Page 1, paragraph 1, has been changed to read (change identified in red font):

“1. The Court Services and Offender Supervision Agency (CSOSA) is conducting an acquisition to obtain Financial Systems Consultant Services.”

g. Attachment 1 to this amendment identifies all the questions received and each response. No more questions will be accepted.

Amendment 0001 Revised Attachment J-6 Past Performance Questionnaire

When filled in, this document is Source Selection Sensitive Information IAW FAR 2.101 and 3.104.

1. The Court Services and Offender Supervision Agency (CSOSA) is conducting an acquisition to provide Financial Systems Consultant Services.

2. You have been listed by (enter name of entity submitting a quote) as a current or past client reference who can serve as a past performance reference. The following information is being collected to help the CSOSA Contracting officer evaluate past performance during the evaluation of proposals.

3. The survey should be completed by the individual most knowledgeable of the Contractor’s day-to-day operations and overall condition of services being rendered. That individual is encouraged to supplement their own knowledge of the Contractor’s performance with the judgment of others in their organization, as applicable.

4. Handwritten responses are sufficient. Please make them legible.

5. Please provide explanatory narratives for as many responses as possible. These narratives need not be lengthy, just descriptive. Space for narrative comments is included on the last page of the survey. If more space is needed, additional pages may be added.

6. As the survey relates to an ongoing source selection for the services for above identified requirement, we request that all information provided within the survey be safeguarded against unauthorized disclosure.

7. Upon completion of the questionnaire, please e-mail it to Cathy Collins at catherine.collins@csosa.gov no later than August 27, 2018 at 1:00 PM Eastern Daylight Time

Attachment 1 - Questions & Responses

QUESTION #1: (Solicitation Section H.10)

a. Is a DD254 required?

b. What Security Clearance, if any, is required?

RESPONSE:

a. No DD254 is required.

b. Refer to Solicitation Section H.1 for all security requirements for this acquisition.

QUESTION #2: (Solicitation Section H.17) Based on a review of the insurance requirements listed in section H.17 of the solicitation, we have the following questions:

a. Normally we carry General Liability, Automobile Liability, Worker's Compensation and Errors & Omissions insurance. It appears that other insurance requirements are CSOSA-specific and are expensive to obtain for small businesses. Will there be a CLIN provided to charge the cost of the additional insurances such as Employer's Liability, Commercial Umbrella Liability, Crime Insurance, Cyber Liability Insurance and Employment Practices Liability?

b. Could you please clarify if we need to maintain the Commercial General Liability Insurance for 5 years past the contract end date?

RESPONSE:

a. All costs associated with performance of the work identified in the solicitation shall be included in the burdened hourly rate.

b. See Amendment 0001, block 14, paragraph c. for a change to the solicitation requirement for Commercial General Liability Insurance. The Contractor is expected to comply with the amended terms and all other solicitation terms and conditions.

QUESTION #3: (Solicitation Section L.5.1) – Are Attachments J-1, J-2, J-3, and J-4 to the solicitation only required after award?

RESPONSE: Yes.
