National Aeronautics and Space Administration Statement of Work
1. SUMMARY: The National Aeronautics and Space Administration requires vendor to supply and deliver the following items:
a. One (1 ea.) 5K Forklift w/8' forks + transportation, for KSC Surge 1/KSC VAB then moved to CCAFS Area 57 locations.
b. One (1 ea.) 15K Forklift w/10' Forks for KSC Surge 1/KSC VAB locations.
c. One (1 ea.) 15K Forklift w/10' Forks for CCAFS SLC-46 location.
d. One (1 ea.) 80’ PALD/Bucket lift for KSC VAB/KSC Surge 1 locations.
e. One (1 ea.) 32’ Electric Scissor lift for KSC VAB location.
f. One (1 ea.) 80’ PALD/Bucket lift for CCAFS SLC-46 location.
g. One (1 ea.) 24’ Stake bed truck for multiple locations.
h. One (1 ea.) 19K Lbs. tug for KSC Surge 1 location.
i. Four (4 ea.) Solar Powered Light trailer with four (4 ea.)100' extension cords for CCAFS SLC-46 location.
j. One (1 ea.) Solar Powered Light trailer w/100' extension cord for Guard Shack location.
k. One (1 ea.) 30K Lbs. Tug for CCAFS SLC-46 location.
l. One (1 ea.) TRS/GCA enclosure for KSC Surge 1 location.
m. Fifty (50 ea.) Propane bottles for multiple locations.
n. Two (2 ea.) Portable toilets for KSC Surge 1 location
o. Two (2 ea.) Portable toilets for CCAFS SLC-46 location
p. Two (2 ea.) Portable Hand wash stations for KSC Surge 1 location.
q. Two (2 ea.) Portable Hand wash stations for CCAFS SLC-46 location.
2. DESCRIPTION OF EQUIPMENT, DELIVERY, AND TASKS TO BE PERFORMED: Equipment and Deliver ies to be performed are subject to the requirements and standards contained in this Statement of Work. The contractor shall provide all equipment, materials, and labor to accomplish these requirements.
2.1 The contractor will provide (1) 5K Forklift, Propane operated, Minimum Drawbar Pull Capacity of 2,000 lbs., Lift height- 10 ft. (top of fork) with a hitch Pin diameter of 3" (Must fit a Lunette Ring of 3" inner diameter and 6.25" outer diameter). Forklift shall have soft tires so it can be driven for long distances on an improved surface. Each instance of forklift use shall include 8ft. forks.
2.1.1 Scheduled delivery of 5K Forklift is required by January 4, 2019. Rental period required from January 4, 2019 until May 15, 2019.
2.1.2 Forklift will need an additional transportation move from delivery location of KSC Surge 1 facility to CCAFS Area 57, at a date to be determined.
2.1.3 Contractor is responsible for delivery of the forklift to the Kennedy Space Center, RPSF KSC Surge 1 facility. Vendor shall submit a delivery schedule by email before delivery, and ensure the common carrier calls 48 hours in advance to schedule physical delivery. Item shall be delivered fully fueled.
2.1.4 Contractor shall provide training/demonstration of the use of the new unit.
2.1.5 Contractor will provide documentation to indicate delivery and the items listed.
2.1.6 Contractor will ensure Warranty information is provided at point of delivery for all applicable items.
2.1.7 Delivery Address for KSC shall be at National Aeronautics and Space Administration, Kennedy Space Center. The address is: Kennedy Space Center, RPSF KSC Surge 1 facility, 32899. Hours of service for delivery: 8:00 AM to 5:00 PM Monday through Friday excluding Federal Holidays. Delivery driver shall be certified to perform deliveries into a Federal Installation.
2.1.8 Inspection and Acceptance will be performed at the location of delivery.
2.2 The contractor will provide (1) 15K Forklift (at Surge 1location), Propane operated, Minimum Drawbar Pull Capacity (lbs) of 3,500. Lift height- 16 ft (top of fork) with a hitch Pin diameter of 3" (Must fit a Lunette Ring of 3" inner diameter and 6.25" outer diameter). Any Forklift Meeting Minimum Capability of Hyster Model 155 To Be Considered. Each instance of forklift use shall include 10ft. forks.
2.2.1 Scheduled delivery of 15K Forklift is required by January 4, 2019. Rental period required from January 4, 2019 until March 20, 2019.

2.2.4 Contractor shall provide training/demonstration of the use of the new unit.

2.2.6 Contractor will ensure Warranty information is provided at point of delivery for all applicable items.

Hours of service for delivery: 8:00 AM to 5:00 PM Monday through Friday excluding Federal Holidays. Delivery driver shall be certified to perform deliveries into a Federal Installation.
2.3.8 Inspection and Acceptance will be performed at the location of delivery.
2.4 The contractor will provide (1) Propane Personal Aerial Lifting Device (PALD at Surge 1 location), 80 foot platform height, weight capacity of 2 personnel plus 100 lbs. of tools (500 lbs.).
2.4.1 Scheduled delivery of 80’ PALD is required by January 4, 2019. Rental period required from January 4, 2019 until March 20, 2019.

2.5.1 Scheduled delivery of 32’ Scissor lift is required by January 4, 2019. Rental period required from January 4, 2019 until March 20, 2019.

2.5.3 Contractor is responsible for delivery of the 32’ Scissor lift to the Kennedy Space Center, VAB facility. Vendor shall submit a delivery schedule by email before delivery, and ensure the common carrier calls 48 hours in advance to schedule physical delivery. Item shall be delivered fully fueled.
2.5.4 Contractor shall provide training/demonstration of the use of the new unit.
2.5.5 Contractor will provide documentation to indicate delivery and the items listed.

2.7 The contractor will provide (1) Diesel 24' Stake Bed Truck, 10,000lb capacity, Width- 8', can accommodate 3 persons, with cargo straps.
2.7.1 Scheduled delivery of 24' Stake Bed Truck is required by January 4, 2019. Rental period required from January 4, 2019 until May 15, 2019.

2.7.3 Contractor is responsible for delivery of the 24' Stake Bed Truck to the Kennedy Space Center, RPSF KSC Surge 1 facility. 2.10.4 Contractor shall provide training/demonstration of the use of the new unit.

2.12 The contractor will provide (10) Propane bottles per month, for operating Material Handling Equipment (MHE) fleet. Total number of propane bottles is 30 ea.
2.12.1 Scheduled delivery of propane bottles is required by January 4, 2019. Rental period required from January 4, 2019 until March 20, 2019.

2.12.3 Contractor is responsible for delivery of the Propane bottles to Kennedy Space Center, RPSF Surge1 facility. 2.12.4 Contractor should provide training/demonstration of the use of the new unit.

2.12.8 Inspection and Acceptance will be performed at the location of delivery.
2.13 The contractor will provide (1) Aluminum Framed clear film enclosure as follows:
· Dimensions (Approximate)- 24’ W x 24’L x 22’H- Tent will house a test article with the following dimensions: 20’W x 20’ L x 15’H. (232” W x 232” L x 175” H).
· Roof/Wall materials- Clear Plastic film, Class1 fire rated, as per NFPA 76 and ASTM E-84 standards, or equivalent. Material shall be permanently static dissipative to protect against Electro-Static discharge (ESD)
· Door material- Clear Plastic film strips, Class1 fire rated, as per NFPA 76 and ASTM E-84 standards, or equivalent. Material shall be permanently static dissipative to protect against Electro-Static discharge (ESD)
· Structure shall be repositionable, and shall use lockable casters.
· Enclosure shall have a 12” duct flange, with provisions for attaching an HVAC duct that provides ventilation/positive pressure to enclosure internal volume.
· Enclosure will protect test article from any water intrusion from facility roof.

2.13.1 Scheduled delivery of enclosure is required by January 4, 2019. Rental period required from January 4, 2019 until May 15, 2019.

2.13.8 Inspection and Acceptance will be performed at the location of delivery.
2.14 The contractor will provide (2) Portable toilets (KSC RPSF Surge 1 Location)
2.14.1 Scheduled delivery of Portable toilets is required by January 4, 2019. Rental period required from January 4, 2019 until May 17, 2019.
2.14.2 The contractor shall service portable toilets as required. Servicing shall be coordinated in advance to ensure there are no conflicts with mission operations. Driver shall coordinate with Area coordinator before servicing, and follow operational guidelines.
2.14.3 Contractor is responsible for delivery of the Portable toilets to Kennedy Space Center, RPSF Surge1 facility. Vendor shall submit a delivery schedule by email before delivery, and ensure the common carrier calls 48 hours in advance to schedule physical delivery. Item shall be delivered fully serviced.

2.17.1 Scheduled delivery of Portable Hand wash Stations is required by February 4, 2019. Rental period required from February 4, 2019 until May 17, 2019.
2.17.2 The contractor shall service Portable Hand wash stations as required. Servicing shall be coordinated in advance to ensure there are no conflicts with mission operations. Driver shall coordinate with Area coordinator before servicing, and follow operational guidelines.
2.17.3 Contractor is responsible for delivery of the Portable Hand wash Stations to Cape Canaveral Air Force Station, Launch Complex 46. Vendor shall submit a delivery schedule by email before delivery, and ensure the common carrier calls 48 hours in advance to schedule physical delivery. Item shall be delivered fully serviced.

2.17.5 Contractor will provide documentation to indicate delivery and the items listed.
2.17.6 Contractor will ensure Warranty information is provided at point of delivery for all applicable items.
2.17.7 Delivery Address for Cape Canaveral location: Cape Canaveral Air Force Station, Launch Complex 46, Camera Road B. Hours of service for delivery: 8:00 AM to 5:00 PM Monday through Friday excluding Federal Holidays. Delivery driver shall be certified to perform deliveries into a Federal Installation.
2.17.8 Inspection and Acceptance will be performed at the location of delivery.
