SCHEDULED OF SUPPLIES / SERVICES

A. SCOPE OF WORK

Contractor shall provide a thermal label and wrist band printing system to include D2 interface to the Department of Health and Human Services, Indian Health Service, Resource Patient

Management System (RPMS), installation services, support services and connectivity between printers and RPMS for the CRSU, Clinical, Medical, Laboratory, and Pharmacy Departments. The main service location is Colorado River Service Unit located in Parker, Arizona. Additional areas for equipment are: Peach Springs Health Center, Chemehuevi I.H.S. Clinic; Moapa IHS Health Clinic and

Havasupai Health Clinic.

The wristband printing system (printers) shall include interface to the Resource Patient Management System (RPMS), configuration, and support services to print patient name, Date of Birth (DOB), Health Record Number (Chart Number), Age, and Gender. Health Record Number will be barcoded. This printing system is for patient and medication safety and also a requirement for Meaningful Use, Stage 2. This will be a Brand Name or Equal Solicitation in accordance with FAR

11.104 (b). Brand name or equal purchase descriptions must include, in addition to the brand name, a general description of those salient physical, functional, or performance characteristics of the brand name item that an “equal” item must meet to be acceptable for award. Use brand name or equal descriptions when the salient characteristics are firm requirements.

The CRSU will be implementing multi-function thermal label & wristband printers in efforts to consolidate existing label/ card printing needs. This will be also consolidated with thirty one (31) thermal label/card printers and will be installed at five (5) clinic/department locations to be used for a variety of Resources Patient Management (RPMS) applications.

(Contractors shall submit information; brochure, specifications, training information, and annual maintenance and licensing information with quote).

The RPMS Pharmacy IV and MAR label formats must conform to the Institute for Safe Medication

Practice guidelines for an inpatient pharmacy label format which translates drug names into the FDA recommended Tall-Man Lettering form. Tall-Man Lettering is necessary to reduce errors by altering the appearance of similar drug names and making them easily distinguishable. Tall-Man Lettering utilizes upper and lower case letters to identify one drug from another.

CONFIGURATION:

New developed wristband labels require the contractor to provide programming and mapping. Contractor shall provide draft of label and corrections/adjustments as needed.

ONGOING SUPPORT:

Contractor shall provide technical support, offsite and onsite as required. Contractor shall provide all programming of any changes as stated in the annual service agreement.

USB connectivity required. Printing: 300dpi

Media Types: Roll Feed, or continuous direct thermal labels with or without black line, Tag Stock, and or wrist bands.

(Single, multi-purpose thermal label and wristband printer that can print Resource

Patient Management System (RPMS) Chart Id, and Pharmacy MAR Update labels without changing labels stock (2.25” x 1.25”), Patient Name, DOB, Health Record

Number, Age, Chart Number, and Gender. Printer maybe used to print Medical

Records routing cards or Pharmacy IV labels (3” x 3”, 3”). Printer may need a bar-code reader with stand, depending upon application.

B) RPMS Communication Interface (D2)

Networked Interface that requires unit specific IP Address, Queue name and Driver and USB Interface to the Direct Thermal Printer and handles all communication between RPMS and the Direct Thermal Printer to obtain bar coded wristbands and labels specific and required for BCMA support.

Thermal Wristband Labels Specifications:

The government requires the following label & Wrist band sizes as follows:

1) Adult wristband – 1” x 11.5” Direct Thermal 3” core 375/roll for Industrial

Printer use Direct Thermal – Soft Adult Size/Style.

2) Direct Thermal-Soft Infant Wristband-Tag Style-200 bands/roll @ 2 rolls/box

3) Direct Thermal White Paper Labels; size 3” x 3” for Pharmacy IV, 2.25” x 1.25” for Patient Demographics, MAR and Ward Collect.

4) Direct Thermal Poly/Paper labels, size 4” x 1.25” for Pharmacy BCMA option 10

Employees:

The contracting Officer reserves the right to restrict the employment of any

Contractor employee, or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

Testing:

Prior to submitting an invoice, the Contractor shall start and test the system to ensure proper operation, connectivity between printer and RPMS, and provide the

Project Office an opportunity to witness the testing.

B. PRICING

I. Contractor to submit with RFQ a pricing sheet to include all equipment, software, installation, interfacing, and integration. In addition contractor shall also provide pricing for the annual

Licensing Agreement and Subscription Support Services.

C. GOVERNMENT FURNISHED INFORMATION, PROPERTY AND SERVICE

CRSU shall provide all necessary network drops required for installation. CRSU shall provide contractor with IP addresses and Resources Patient Management System (RPMS) networked printer designations for each printer to be installed. CRSU shall provide network access to contractor while contractor is onsite so that firmware can be loaded and the printers can be configured.

D. PERIOD OF PERFORMANCE

Contract shall submit with offer lead times for ordering equipment and time to install equipment.

Government requests best possible date. Period of performance for Annual Licensing Software and

Subscription Support Services shall be from date of award for base year plus four (4) and (1) year option to review.

E. CONTACTOR’S OBLICATIONS

The Contract shall:

a. Contractor shall furnish own equipment and labor for the job;

b. Contractor shall wear visible identification provided by the CRSU Security Staff at all times on the premises of the CRSU compound. The Contractor shall turn in this identification provided by the CRUS Security Staff upon completion of the project

c. Provide the service described herein independent of Government supervision.

d. Provide all services described within the statement of work which may be required by the I.H.S.

together with personnel, facilities, equipment, supplies, and other items and expertise necessary for, or incidental to, the provision of such services.

F. MINIMUM CONTRACTOR QUALIFICATIONS:

a. All work shall be performed by skilled craftsman that are regularly engaged in work to be performed and shall be at the Journeyman level or directly supervised by a Journeyman.

b. The contractor shall arrange work so as not to cause interference with normal operations at the

Indian Health Services facilities located at the Parker Indian Health Center. The facility is open from 8:00am to 5:00pm. In those cases where some interference is unavoidable, the Contract must make every effort to minimize the impact of the interference and its effects.

c. None of the services furnished under the Statement of Work shall be subcontracted or transferred to another Contractor or organization without the specific prior written approval of the Contracting Officer.

d. Three years of experience in this type of work

G. QUALITY CONTROL:

a. The Contractor shall provide services under the terms and of this contract with staff capable of providing proper and adequate services.

b. The Contractor shall be responsible for monitoring and evaluating the safety, quality and appropriateness of services provided and shall maintain the quality control program.

H. COMPENSATION:

The Contractor shall be compensated at the agreed contract amount based on invoices submitted and only for services in direct performance of this contract.

Per mandate by the U.S. Department of Treasury and effective July 1996, payments by Electronic

Funds Transfer System (EFTS) will be made to the Contractors designated financial institute.

I. METHOD OF PAYMENT AND INVOICE SUBMISSION:

a. Payment shall be made in ARREARS after successful completion of the JOB and payments made in monthly ARREARS for Maintenance and Support;

b. Electronic Funds Transfer (EFT) shall be utilized as method for processing payments;

c. Upon receipt of the Contractor’s properly prepared invoice, the COR is responsible for certifying the invoice for acceptance of service;

d. Improperly prepared invoices will be returned to the Contractor resulting in delayed payment.

The following shall constitute as a “properly” prepared invoice:

1. Purchase Order number and period of performance

2. Fee schedule of line item, description, quantity, unit of price and extended total per line item; total amount of invoice

3. Contractor shall email and electronic copy of the invoice to the following:
