1. Prices for Contract Line Items (CLINs) No. 000100 and 000200 above have established at a ceiling which, if the vendor exceeds, it would be at its own risk. These CLINs will include the total level of effort estimated by the vendor to perform this contract. Payment for these CLINs will be made on a monthly basis of in accordance with FAR Clause 52.212-4, Alternate I (JAN 2017), (i) Payments (1) i, Hourly rate, which requires the vendor to show clearly the level of effort provided per month, i.e., number of labor hours, number of days and labor categories employee.
2. The cost of travel is considered “Materials” (see FAR Clause 52.212-4 Alternate I (Jan 2017), (i) Payments, (1) ii, Materials. All travel is to be performed in accordance with Federal Travel Regulations and/or the Joint Travel Regulation. The vendor may be required to submit evidence supporting any claim for travel related expenses. All requests for federally reimbursable travel shall be pre-approved by the Contracting Officer Representative and reimbursed to the extent that it is reasonable and supported. 3. Except for CLINs 000100 and 000200, the prices for the rest of the CLINs above are firm-fixed. Payment will be made IAW/52.212-4 (i) Payment.
4. This order includes various options that the Government may exercise by a unilateral modification to the order IAW/the option clauses of this order. Options may be exercised at any time within the time indicated at the CLIN. However, the price or the Option PoP may have to be modified and adjusted prior to exercising the option.

(d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

Contracting Officers Representative (COR) Technical Guidance (Sep 2009)
Performance of the work hereunder shall be subject to the technical directions of the designated COR for this contract.
As used herein, technical directions are directions to the Contractor which fill in details, suggests possible lines of inquiry, or otherwise completes the general scope of work set forth herein. These technical directions must be within the general scope of work, and may not alter the scope of work or cause changes of such a nature as to justify an adjustment in the stated contract price/cost, or any stated limitation thereof. In the event that the Contractor feels that full implementation of any of these directions may exceed the scope of the contract, he or she shall notify the originator of the technical direction and the Contracting Officer in a letter separate of any required report(s) within two (2) weeks of the date of receipt of the technical direction and no action shall be taken pursuant to the direction. If the Contractor fails to provide the required notification within the said two (2) week period that any technical direction exceeds the scope of the contract, then it shall be deemed for purposes of this contract that the technical direction was within the scope. No technical direction, nor its fulfillment, shall alter or abrogate the rights and obligations fixed in this contract.
The Government COR is not authorized to change any of the terms and conditions of this contract. Changes shall be made only by the Contracting Officer by properly written modification(s) to the contract.
The Government will provide the Contractor with a copy of the delegation memorandum for the COR. Any changes in COR delegation will be made by the Contracting Officer in writing with a copy being furnished to the Contractor.

Evaluation of Contractor Performance Utilizing CPARS
In accordance with FAR 42.15, the Centers for Disease Control and Prevention (CDC) will review and evaluate contract performance. FAR 42.1502 and 42.1503 requires agencies to prepare evaluations of contractor performance and submit them to the Past Performance Information Retrieval System (PPIRS). The CDC utilizes the Department of Defense (DOD) web-based Contractor Performance Assessment Reporting System (CPARS) to prepare and report these contractor performance evaluations. All information contained in these assessments may be used by the Government, within the limitations of FAR 42.15, for future source selections in accordance with FAR 15.304 where past performance is an evaluation factor.
The CPARS system requires a contractor representative to be assigned so that the contractor has appropriate input into the performance evaluation process. The CPARS contractor representative will be given access to CPARS and will be given the opportunity to concur or not-concur with performance evaluations before the evaluations are complete. The CPARS contractor representative will also have the opportunity to add comments to performance evaluations.
The assessment is not subject to the Disputes clause of the contract, nor is it subject to appeal beyond the review and comment procedures described in the guides on the CPARS website. Refer to: www.cpars.gov for details and additional information related to CPARS, CPARS user access, how contract performance assessments are conducted, and how Contractors participate. Access and training for all persons responsible for the preparation and review of performance assessments is also available at the CPARS website.
The contractor must provide the CDC contracting office with the name, e-mail address, and phone number of their designated CPARS representative who will be responsible for logging into CPARS and reviewing and commenting on performance evaluations. The contractor must maintain a current representative to serve as the contractor representative in CPARS. It is the contractor’s responsibility to notify the CDC contracting office, in writing (letter or email), when their CPARS representative information needs to be changed or updated. Failure to maintain current CPARS contractor representative information will result in the loss of an opportunity to review and comment on performance evaluations.

The CORE enables IQVIA to provide customized solutions for clients leveraging the world’s largest curated healthcare information source, advanced analytics, leading technologies and extensive industry knowledge across diseases, geographies and scientific methods.
IQVIA is a global leader in protecting individual patient privacy. A turnkey and unique solution that put analytics in the hands of end users through intuitive visualizations, E360 can be used with IQVIA or non-IQVIA real-world data.
Proprietary applications offered on e360 platform are:
· E360 Cohort Builder lets users manage, build and apply code lists combining this to build, apply and test clinical events. Cohort Builder can apply various demographic and geographic filters on multiple datasets spanning geographies across the globe.
· Cohort Visualization – visualize cohort results such as via map view or time series plots of selection criteria
· Attrition Reporting – visualize protocol based patient attrition reports either by user setup, or ‘most to least’ views.
· Cohort descriptive statistics – base descriptive statistics such as age-gender splits, to quickly verify that cohort selection is robust
· Patient timeline visualization – deep-dive on individual patient histories to understand what the patients really look like, and their pathway experience
· Reporting & Export – build base reports on cohort results. CDC will not be able to export data or reports during the pilot; however, the agency will have that ability if CDC purchases access to E360 after the pilot is complete. · E360 Dataset Explorer - search on different selection criteria (i.e., geography, therapeutic area, data type, etc.) and view meta-data about specific data assets to help you identify options. Dataset Explorer provides you advanced features such as dataset comparison reporting, interactive visualizations of a dataset, and Top Therapy and Diagnosis charts, all of which can be downloaded as PDFs and spreadsheets.
· E360 Recruitment Simulator - predicts potential study recruitment success rates using historic EMR and IQVIA data on a selected patient group, combined with your assessments for criteria such as site numbers and readiness, screening success rates, and recruitment periods.
· E360 Python Analytics Workbench - A fully independent set of applications that run either as an online service, or on your desktop, but have the ability to integrate and publish to users’ E360 workspaces. PAW works on all E360 loaded datasets; OMOP, LPD and native format, and on non-E360 data.
· E360 Workspaces – intuitive workspaces and sharing of various items (i.e., cohorts, codelists) that allow teams to collaborate, such as around therapy areas or on specific studies.
· E360 Codelist Management – seamlessly create, share and re-use codelists, that are otherwise cumbersome to manage, and that form the basis of any protocols or disease definitions.
· E360 Data Completeness Report - helps to assess the quality and consistency of key data variables to help inform study feasibility. Reports on how many patients have a record for a test, diagnosis or prescription variable in a given timeframe. Completeness statistics for recording of key study variables with top-line statistics including counts, and mean and IQR for numeric data.
Access to this new IQVIA e360 platform was recently provided to a limited number of CDC scientists through a three months pilot project. The scientists became familiar with the data and tools provided on IQVIA e360 and explored its value for public health. Continuing to have direct access to this unique e360 platform that offers RWE datasets and complex applications, CDC scientists will be poised to conduct complex analysis and develop use cases on disease patterns, progression, and health outcomes; evaluate clinical guidelines uptake, treatments and interventions; and conduct public health surveillance type queries. These use cases could address a range of HHS, CDC, and programmatic priorities, such as opioids abuse, mental health, hypertension, diabetes, healthcare-associated infections, latent TB and many other chronic and infectious conditions.
Study findings will inform public health programs and surveillance strategies on the importance of using clinical, claims, laboratory and pharmacy data on the same platform as the integration of public health and health care continues to evolve at a rapid pace.
SECTION 2 – PURPOSE
The purpose of this acquisition is to provide full access to IQVIA’s E360 platform and tools, including access to PharMetrics plus and longitudinal cohorts, and downloads of data for analysis outside the platform. Three seats will be first purchased on the base year. A seat transfer process will be developed to allow the scientists participating in the IQVIA pilot to maintain their skills, continue their studies and support others across the agency learn more about the use of Electronic Medical Records on e360 platform. The users will have access to IQVIA’s Ambulatory Electronic Medical Records (AEMR) data, PhaMetrics, longitudinal patient cohorts and linked data. The number of seats to be purchased in the following year will be determined based on the experience gained by CDC scientists, the findings of their studies and implications for public health practice, and the emerging needs for having access to this kind of data from large clinical practices and physician networks that IQVIA e360 provides
Ambulatory Electronic Medical Records (AEMR)
The purpose of this acquisition is to provide access to IQVIA’s E360 platform. The users will have access to IQVIA’s Ambulatory Electronic Medical Records (AEMR) data in the OMOP common data model format, Oncology EMR data in the OMOP common data model format, PhaMetrics, longitudinal patient cohorts and linked data.
Ambulatory Electronic Medical Records (AEMR)
The IQVIA AEMR asset is comprised of approximately 46 million patient records with a face-to-face physician interaction since 2006 and are sourced from an “opt-in” provider research network. The aggregated database comprises records collected by over 40,000 physicians from large practices and physician networks. Approximately 40% of the contributing physicians are primary care practitioners and the remaining are specialists.
Key information collected includes:
· Patient age/ gender / race / 3-digit
· ZIP/longitudinal tracking
· Vitals (e.g. blood pressure, heart rate, weight, height)
· Risk factors (e.g., smoking, alcohol), co morbidities, medical history

· Claims The information collected allows linking key clinical variables such as lab values and blood pressure to therapeutic outcomes and to prescriptions / diagnosis / hospital metrics and connecting patient vitals, health behaviors and risk factors to diagnosis and treatment.
AEMR contain the clinical data of approximately 46 million patients since 2006 from all 50 states recorded by 40,000 providers who are affiliated with over 315 ambulatory large practices and physician networks. About 40% of the contributing physicians are Primary Care Physicians (PCPs) and the remaining 60% are specialists. IQVIA AEMR data is available from 2006 and updated on a monthly basis. These data are used to link key clinical variables such as laboratory values, weight and blood pressure to prescriptions, diagnoses, and therapeutic outcomes. AEMR are also used to connect patient vitals, health behaviors and risk factors to diagnosis and treatment and develop insights based on provider treatment decisions and prescribed medications (compared to dispensed medications).
AEMR data includes the following key attributes:
· Patient demographics (age, gender, race, 3-digit zip-code, ID for longitudinal tracking)
· Vitals (e.g. blood pressure, heart rate, weight, height)
· Risk factors (e.g., smoking, alcohol use), co- morbidities, medical history

Data Availability: New source AEMR data contributions are processed monthly. Users will typically see a 45 day lag before new information is added to the database. Updates are made less often in e360 and may not take place during the pilot project. IQVIA AEMR data housed in e360 will be in the OMOP data format. In addition to having access to these data, CDC scientists will be granted access to and will be able to use all applications offered on e360 platform.
Considering the complexity of the RWE data sets including the monthly updates and e360 applications, IQVIA will conduct an online or onsite training session as need be and provide weekly Q&A sessions via webinar for all users. The IQVIA training and webinars will cover the E360 platform applications. Technical assistance will also be available from 8:30AM to 5:00PM Monday through Friday. Data downloads can be performed as necessary and needed by the CDC scientists.
