STATEMENT OF WORK
LINENS & SCRUBS LAUNDRY SERVICE
U. S. COAST GUARD BASE BOSTON
SECTION 1 – GENERAL INFORMATION
Background: Base Boston serves as the single DCMS touch point for the First Coast Guard District in support of Coast Guard operations throughout the district's area of operations. Base Boston also supports all other Coast Guard support units and personnel who are stationed throughout New
England, New York and Northern New Jersey.
Base Boston’s unaccompanied personal housing (UPH) consists of thirty (30) transient rooms with a capacity of 60 beds for non-permanent party service members. The number of personnel assigned to the UPH varies month to month with an average of 30 tenants per month.
1.1 Scope of Work
The scope of this project is to provide rental linens and laundry service weekly for linens and Coast Guard owned medical scrubs/ gowns to U.S. Coast Guard Base Boston, unaccompanied personnel housing (UPH) and medical clinic from: 01 January 2022 to 30 September 2022.

Shane Barwick. Parties must arrive no later than five minutes prior to the start time indicated in order to allow coordination by the site representative.

Contractor personnel shall comply with all rules and regulation while on government property, to include posted signs regarding COVID-19 protocols while on federal property. Masks are required to be worn regardless of vaccination status.
1.2 Contractor Personnel
1.2.1 Contract Manager
The contractor must provide a contract manager who must be responsible for the performance of the work.
The name of this person, and an alternate or alternates, who must act for the contractor when the manager is absent, must be designated in writing to the contracting officer.
1.2.1.1 The contract manager or alternate must have full authority to act for the contractor on all contract matters relating to daily operation of this contract
1.2.1.2 The contract manager and alternate or alternates must be able to read, write, speak, and understand the English language.
1.2.2 Contractor employees
The contractor must not employ persons for work on this contract if such employee is considered by the contracting officer to be a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
1.2.2.1 Contractor personnel must present a neat appearance and be easily recognized as contractor employees.
1.2.2.2 The contractor must not employ any person who is an employee of the U. S. Government if employing that person would create a conflict of interest. Additionally, the contractor must not employ any person who is an employee of the U. S. Coast Guard unless such person seeks and receives approval according to U. S. Coast Guard regulations.

1.2.3 Security requirements
Due to heightened Security at USCG Facilities, all contractor employees performing tasks related to this
SOW, while onboard a Coast Guard facility, shall have proper valid identification. Time spent undergoing entrance to USCG facilities will not be considered a government delay of work or differing site condition.
The QAI will brief contractor on routine security protocols for accessing facility upon award.
1.3 Hours of Operation
1.3.1 The contractor must perform the tasks as required by the SOW under this contract during the following hours: Monday through Friday between 8:00am and 2:00pm unless other time is specified.
1.3.2 The government hereby provides notice and contractor hereby acknowledges receipt that government personnel observe the listed days as holidays, and subsequently the moving personnel will be compensated for the holiday as follows:

In addition to the days designated as holidays, the government observes the following days:
Any other day designated by Federal Statue
Any other day designated by Executive Order
Any other day designated by the President’s Proclamation
It is understood and agreed between the government and the contractor that observance of such days by government personnel shall not be a reason for an additional period of performance, or entitlement of compensation except as set forth within the contract.
1.3.3 Special event, or adjusted work hours: If circumstances should arise wherein the contractor desires a different work schedule as required in section 1.3.1 above, the contractor must request same in writing.
Submit written request to the government contracting officer a minimum of 24 hours before the requested adjusted work schedule. If the desired schedule change is to be effective on a Monday, the request must be submitted to the Contracting officer prior to 9:00am on the Friday prior to commencement of work.
1.4 Invoicing
Invoices shall be by emailed to the quality assurance inspector and contracting officer, or mailed to:

The following information must be included when submitting an invoice:
(1) Name of business and address.
(2) Invoice date.
(3) Contract number.
(4) Billing period. Insert the beginning and ending dates (month, day, and year) of period in which costs were incurred. If there is any overlap with previous billing period, explain,
(5) Description of supplies and/or services.
(6) Business DUNS.
SECTION 2 – GOVERNMENT FURNISHED PROPERTY AND SERVICES
2.1 General Information
The Government must provide the facilities, equipment, materials, and services listed here.
2.1.1 Government Furnished Facilities
2.1.1.1 Linen storage area.
2.2.1 Government Furnished Equipment
2.2.1.1 None.
2.3.1 Government Furnished Material
2.3.1.1 Scrubs and patient gowns.
SECTION 3 – CONTRACTOR FURNISHED ITEMS AND SERVICES
Except for those items, services, or commodities specifically stated in section 2 as government furnished, the contractor must furnish everything needed to perform this contract according to all its terms.
SECTION 4 – SPECIFIC TASKS:
The contractor is responsible for all trained personnel, labor, equipment, material, and supplies in performance of this contract. Detailed specification requirements that is performed by contractor as follows:
4.1.1 The contractor shall provide and maintain a stock of clean linens at the service location listed in section 1.1.2.1, Unaccompanied Personnel Housing. The following minimum linen quantities shall be maintained through the performance of this contract.
Pillow case (standard size) x 60 each
Flat sheet (66”x 104”) x 120 each
Poly fleece blanket (66” x 90”) x 60 each
4.1.2 Each week in accordance with section 1.3, all soiled linens shall be picked up and replaced with clean linens in the designated supply storage area on a weekly basis. The number of linens needed for pick-up/delivery may fluctuate week to week based on usage. The contractor shall not leave linens in excess to the required quantities above. Pick-up and delivery shall be performed on a consistent day of the week coordinated through the QAI.
4.1.3 The contractor shall maintain records of linen inventory loss/gains and notify the QAI of inventory discrepancies.
4.1.4 The contractor shall maintain records of weight in pounds (lbs.) and service performance dates for billing.
4.1.5 Contractor personnel shall check-in and check-out with the QAI or designated UPH representative in his absence upon arrival and completion of work.
4.2.1 Each week in accordance with section 1.3, contractor shall pick-up soiled scrubs and patient gowns from the location listed in section 1.1.2.2, Base Boston medical clinic and deliver laundered scrubs/gowns within 7 days. Pick-up and delivery shall be performed on a consistent day of the week coordinated with the QAI.
4.2.2 Scrubs and gowns shall be picked up and dropped off at the area designated by the QAI in section
1.1.4.2.
4.2.3 The contractor shall maintain records of weight in pound (lbs.) and service performance dates for billing.
4.2.4 Contractor personnel shall check-in and check-out with a Coast Guard medical clinic staff upon arrival and completion of work.
SECTION 5- SPECIAL CONTRACT REQUIREMENTS:
5.1.1 Access to Government Property:
The QAI will make all necessary arrangements for access by contractor personnel to the work site(s). While on government property, the contractor personnel shall comply with all rules and regulations pertaining to health and safety, not only as they apply to themselves, but also other personnel and property at the site.
5.2.1 Permits and Licenses:
In the performance of work hereunder and with the price herein, the contractor shall obtain and maintain in effect all necessary permits and licenses required by federal, state, or local government, or subdivisions thereof, or of any other duly constituted public authority. Further the contractor shall obey and abide by all applicable laws and ordinance.
5.3.1 Indemnification:
The contractor shall hold harmless and indemnify the government against any and all claims and costs of any kind and nature, for any loss or damage occurring in connection with the contractor’s acts or omissions incident to its performance under this contract. This indemnification includes the acts or omissions of the contractor, any subcontractor or employee, agent, or representative of the contractor or subcontractor.
5.4.1 On-Site Contractor Performance:
During all operations on government premises, the contractor personnel shall comply with the rules, regulations and procedures governing the conduct of personnel and the operation of the facility, as expressed in DHS, U.S. Coast Guard written and/or oral procedures.
5.5.1 Safety and Health:
In order to provide safety controls for protection to the life and health of contractor personnel; for prevention of damage to property, materials, supplies and equipment, and the avoidance of work interruptions in the performance of the contract, the contractor shall comply with the safety, health and fire protection policies and procedures as may be provided. The contractor shall maintain an accurate record, and shall report to the KO all incidents and accidents resulting in death, traumatic injury, and/or damage to property, materials, supplies and equipment incident to work performance under this contract. The contractor shall take or cause to be taken such additional safety measures as the KO may determine to be reasonably necessary.
SECTION 6- Period of Performance
The service period shall start 01 January 2022 and end 30 September 2022, for a total period of performance of nine (9) months.
