VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source
Attachment 1: Single Source Justification for SAP under the SAT
DEPARTMENT OF VETERANS AFFAIRS
Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)
Acquisition Plan Action ID: 640-19-3-163-0114
1. Contracting Activity: Department of Veterans Affairs, NCO 21, Palo Alto Medical Center
Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The VA Palo Alto Healthcare System (VAPAHCS) is seeking to purchase three Mayfield 2 Series cranialization tools for surgical procedures. The estimated cost of this procurement is $106,452.
2. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Mayfield Series 2 cranial stabilization tools reduces clamp weight and improved handling for surgeons performing surgical procedures. A simplified release mechanism allows one person to smoothly release the clamp, enabling another team member to focus on safely managing the patient. The I-beam design reduces unintentional movement when positioning or releasing the patient. It also has a closed architecture locking knob.
3. Description of market research conducted and results or statement why it was not conducted:
Market research was conducted by searching GSA Advantage, GSA e-Buy, Google, VA NAC Med Surg Catalog, FPDS and the internet. Integra Lifescience, Inc., is the sole source manufacturer of the Mayfield 2 Series, including all parts and replacements.
Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. Note: COs are required to make a determination of price reasonableness IAW FAR 13.106-3. See the S19 Open Market (FAR13) Award Documentation Form with Abstract to document price reasonableness.
