ADDENDUMS 52.212-1/ 52.212-2
ADDENDUM TO 52.212-1, QUOTE PREPARATION INSTRUCTIONS
A. To assure timely and equitable evaluation of the quote, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of three parts:
Part I – Price Part II – Technical Approach Part III – Past Performance
B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
C. Questions and Answers. Questions concerning this solicitation shall be submitted no later than 31 August 2020 at 5:00 p.m. CDT.
D. Specific Instructions:
In order for a quote to be considered, it must include:
1) A pricing quote (a completed and signed SF 1449).
2) A technical approach quote.
3) Past performance summary.
1. PART I – PRICING QUOTE – The offeror is required to submit one electronic copy of the completed SF 1449.
(a) Complete all fields on the SF 1449 to include company information, pricing for the base year and pricing for option years. The offeror must fill out the solicitation completely and submit a signed copy of the SF 1449. In doing so, the offeror accedes to the proposed contract terms and conditions as written in the solicitation, with attachments.
2. PART II – TECHNICAL APPROACH SUMMARY – Offerors are required to submit only one technical quote document. The document shall be no more than 2 pages with the following:
(a) The offeror must provide clear statements as to their ability to provide the services described in the PWS. The statements required are:
i. Disclosure of whether the offeror fully understands the entirety of the PWS or not.
ii. Disclosure of whether the offeror can provide all of the services as they are described in the PWS or not.
iii. Brief description of how the offeror plans accomplish the requirements within PWS paragraph 1.3. “Repair Service Calls”. Plan must include enough information for the contracting officer and resident subject matter experts to determine if the plan is sound.
3. Part III – PAST PERFORMANCE SUMMARY – Offerors are required to submit up to 3 examples of you/your company successfully completing work of a similar scope and dollar value within the last 5 years.
(a) Must include the project name and performance location.
(b) Must include customer POC information so the past performance can be verified.
(c) Must include contract number if applicable.
(d) Must include the CAGE and DUNS of the company performing the work if applicable.

ADDENDUM TO FAR 52.212-2:
BASIS FOR CONTRACT AWARD: This is a solicitation that utilizes FAR Part 13, Simplified Acquisition Procedures, and will utilize the best value to the Government evaluation procedures and fair and reasonable determination. By submission of its offer in accordance with the instructions provided in clause FAR 52.212-1, Instructions to Offerors, the offeror accedes to the terms of this solicitation. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The evaluation process shall proceed as follows:
A. Price Quote Evaluations.
A.1 Evaluation of the option CLINs shall not obligate the Government to exercise such options CLINs.
A.2 Pricing will be evaluated as acceptable or unacceptable based on the following:
i. Complete all fields on the SF 1449 to include company information, pricing for the base year and pricing for option years. The offeror must fill out the solicitation completely and submit a signed copy of the SF 1449. In doing so, the offeror accedes to the proposed contract terms and conditions as written in the solicitation, with attachments.
ii. Pricing must be good for a minimum of 30 days from submission of quote.
iii. Reasonableness: For a price to be reasonable, it must represent a price to the Government that a prudent person would pay in the conduct of competitive business. The Government will determine prices fair and reasonable through the use of one or more of the price analysis techniques at FAR 15.404-1(b)(2).
iv. Unbalanced pricing: Contractors are cautioned against submitting an offer that contains unbalanced pricing. Unbalanced pricing may increase performance risk and could result in payment of unreasonably high prices. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one (1) or more contract line items is significantly over or understated. The Government shall analyze offers to determine whether they are unbalanced with respect to separately priced line items or sub-line items. An offer that is determined to be unbalanced may be rejected if the Contracting Officer determines that the lack of balance poses an unacceptable risk to the Government.
B. Technical Approach Quote Evaluations.
B.1 Evaluation of the quotes are based on a technical evaluation of acceptable or unacceptable based on the Government’s evaluation of the offeror’s submission of the following:
B.1.1. The three mandatory statements from FAR 52.212-1 Addendum:
i. Disclosure of whether the offeror fully understands the entirety of the PWS or not.
ii. Disclosure of whether the offeror can provide all of the services as they are described in the PWS or not.
iii. Brief description of how the offeror plans accomplish the requirements within PWS paragraph 1.3.1. “Emergency Services”. Plan must include enough information for the contracting officer and resident subject matter experts to determine if the plan is sound.
C. Past Performance Evaluations.
C.1 Evaluation of the past performance summary will be based on a past performance evaluation rating of acceptable or unacceptable, based on the Government’s evaluation of the offeror’s submission of the following:
C.1.1. Submission of up to 3 examples of you/your company successfully completing work of a similar scope and dollar value within the last 5 years.
| (i) | Must include the project name and performance location. |
| (ii) | Must include customer POC information so the past performance can be verified. |
| (iii) | Must include contract number if applicable. |
| (iv) | Must include the CAGE and DUNS of the company performing the work if applicable. |
D. Best Value To The Government.
Offerers are cautioned to submit sufficient information and in the format specified in the RFP to permit a meaningful assessment of technical and past performance acceptability. Offerers may be asked to clarify certain aspects of their quote.
E. Award.
The Government will start evaluations by identifying the quote with the lowest price first. That quote will then be evaluated for price, technical approach, and past performance. If the price, technical approach, and the past performance are all determined to be acceptable, then the Government shall award to that offeror with no other evaluations taking place. If the lowest priced quote is not acceptable, then the Government shall evaluate the next lowest priced quote using the same procedures. The Government intends to award the contract without discussions. If the Government decides to conduct discussions, you will be notified as soon as possible.
