C. Implementation and Training

1. Pre-Implementation Meeting The Clinical Applications Specialist (CAS) assigned to complete your implementation will contact the laboratory by phone to conduct a pre-implementation meeting. The purpose of this meeting is to discuss and mutually agree upon a process, provide valuable information regarding the implementation commitment and to define expectations to achieve a smooth transition and a timely go-live date.

2. Training Training is offered via several formats. Options outlined below are dependent on availability and instrument type. Basic Operator Training (“Training”) and Advanced Operator Training will be provided remotely via live Virtual Instructor Led Training (“VILT”) or self-paced e-Learning based on availability.

• Virtual Instructor Led Training (VILT)-Virtual classroom training via live broadcast. Trainee participates in their lab via internet. VILT training seats are unlimited and available to all users throughout the term of this agreement.

• eLearning Self-paced courses for operators through the Sysmex Customer Resource Center. e-Learning access is unlimited and available to all users throughout the term of this agreement.

3. Integration Planning

Depending on your Sysmex solution project pathway, integration planning (order acceptance to go-live) will be OneVoice or Project Manager driven. This individual works with our Sales and Support organizations on your behalf to coordinate your instrumentation and WAM (if applicable) integration plan. Direct contact will be initiated to begin discussing your specific integration plan. If WAM is involved, an onsite meeting will be scheduled by the Project Manager. The purpose of this meeting is to introduce the customer to the overall Middleware project process, establish expectations and review roles and responsibilities. The Middleware rules and Settings meeting will be held in conjunction with the kickoff meeting if possible.

At this time, the Clinical Applications Specialist (CAS) will instruct the customer on the rules writing process and assist the customer in completing the Rules and Settings documents. This will provide all the information necessary to configure their Middleware server.

Installation site requirements for your instrument(s) will be provided. In advance of delivery, your District Service Manager (DSM) or Field Service Representative (FSR) will contact you to discuss these requirements and conduct a pre-site interview to confirm that the facility is ready for the instrument.

Once a mutually agreed upon date for the delivery of your system has been determined by you and your Sysmex team, a team of installation specialists (systemization) or your local SE will install your system. The installation protocol includes calibration, precision, accuracy, reportable range and carryover studies (as applicable to your analyzers) and data stream verification for LIS interface capabilities.

Once the system is operational, and a Key Operator has completed training, the Clinical Applications Specialist (CAS) will be available to assist you in the implementation of the system(s). Analyzers included in these studies are: XN Series, XN-L Series, X-Series, All Systemization, UF-Series and Cell Image Analysis.

Implementation activities consist of the following:

1. Configuration and Validation Studies: Instrument Configuration and Method Verification Studies: Instrument configuration consists of defining settings for user defined flags, suspect flags, onboard rules and host connection. Method Verification studies include carryover and reportable range for Hematology platforms, linearity for Urinalysis platforms and passing results obtained on a QC location test for Cell Image Platforms. Reportable Range material will be provided to verify the measured parameters (WBC, RBC, HGB, HCT, PLT, WBC- BF, RBC-BF and Reticulocyte % (if applicable).

2. Key Operator Reinforcement Activities: Dedicated time is necessary with this individual to reinforce the knowledge gained through e-Learning and/or instructor led training and to prepare them to serve as your primary resource and instrument lead. The CAS will schedule this time while they are onsite for the implementation.

3. Secondary Operator Training: With Virtual Instructor Led Training (VILT), you are eligible for unlimited training slots.

4. Correlation: The CAS will provide direction to your staff and complete the data reduction for your correlation studies. It is the customer’s responsibility to process the correlation samples.

This will serve to reinforce instrument operation training. CLSI recommends a minimum of forty (40) correlation samples with an appropriate distribution across the dynamic range to be statistically significant. In order to fully validate rare event parameters, additional numbers of samples may be required.

Two techs counting 200-cell differentials is recommended if correlating to the manual differential. Final reports can be expected in ten (10) business days from the point that the CAS receives all of the data to be included in the study Q-Flag Adjustment: The analyzer will be set at manufacturer recommended settings. This optimizes your system and influences manual review rates.

5. Insight™: The CAS will enroll your analyzer into our inter-laboratory QA program. This service is delivered and provides your facility with peer comparison by analyzer. This program will be reviewed with your Key Operator(s). Use of the Insight™ program and QC file maintenance will be provided by the CAS.

Evidence Based Control Limits will be entered into the QC files upon installation.

6. Q-Flag Adjustment: The analyzer will be set at manufacturer recommended settings. This optimizes your system and influences manual review rates.

7. Once the Standard of Work (SOW is complete, the CAS will review the supporting documentation with you and sign the Installation Completion Notice (ICN). This initiates the warranty and invoicing of the instrument as outlined in the contract. A signed copy of the ICN will be e-mailed to you once it is processed.

NOTE: Studies beyond those included in the standard of work are the responsibility of the customer.

4. Post Implementation

Your Sysmex SE, HSAM and CAS will continue to support you and the operation of your system following the integration. Any requests for assistance or performance concerns must be initiated through the Technical Assistance Center (TAC) at 888-8Sysmex (1-888-879-7639).

Appropriate subject matter experts will be assigned to resolve any concerns not resolved through TAC.
