About this file
This is a Required Vendor Documentation Form for the Allegheny County Department of Administrative Services, Division of Purchasing and Supplies in Pittsburgh, Pennsylvania. The document applies to all Invitation for Bid (IFB), Request for Proposal (RFP), and Request for Quotation (RFQ) submissions, including RFP 9457 for Parks & Recreation Application System Evaluation. All bidders and vendors must complete, sign with live or electronic signatures, and submit this documentation with their electronic bids or quotations. The form does not specify particular products or services, response dates, or contract terms, as it serves as a standard requirement across all County procurement opportunities rather than being specific to a single solicitation.
The documentation form requires vendors to certify compliance with Allegheny County's Anti-Sweatshop Provisions under Administrative Code Article 903, affirming that proposed goods and products were not made under sweatshop conditions. Vendors must also complete a Buy American Certificate declaring domestic source end products and listing any imported components. The form includes optional questions regarding availability of bid items to other governmental entities within Allegheny County and throughout Pennsylvania at the bid price for the entire contract period, though such availability does not apply to the City of Pittsburgh due to cooperative purchasing provisions. Vendors must provide company information including registration status with the County's Minority, Women and Disadvantaged Business Enterprises program, and identify whether they qualify as a Minority Business Enterprise (MBE), Women Business Enterprise (WBE), or Small Business. All signatures must be executed in the presence of witnesses, with corporate submissions requiring attestation by a secretary and signature by the president, along with the state of incorporation.
