About this file
This document is a past performance questionnaire for a federal construction contract opportunity with the National Park Service to replace decking at a visitor center in Biscayne National Park. The contractor is requesting a past performance reference from the recipient for a prior or current contract. The questionnaire asks the recipient to rate the contractor's performance, timeliness, quality of work, and overall performance on a previous contract using defined evaluation criteria. It also requests explanatory comments for responses and provides fields to identify the contract being rated, the contracting parties, and the questionnaire respondent's contact information. The completed questionnaire must be returned to the identified National Park Service contracting officer by July 31, 2023 to be considered as part of the contractor's proposal for the deck replacement project at Solicitation Number 140P5423R0022.
