7. Prices shown herein are “net” ceiling prices-not to exceed rates. The net ceiling rates include malpractice insurance for non-personal services task-orders and exclude travel and lodging.

9. Background Investigations: Ghost has extensive experience providing the VA with assistance during the security clearance process for our employees. First, we schedule a fingerprint appointment for each candidate, as this is the longest part of the process. The VSC Forms are then sent to the candidate for review and completion. The VSC forms that will need to be completed are: Contract Security Services Request Form 1A, Contract/Employee Fingerprinting Request Form #2, VHA Service Center PIV Sponsorship Form #3, Declaration of Federal Employment, Self- Certification of Continuous Service, and Authorization for Release of Information. The candidate must add their full legal name to all forms, including their Middle Name. If some questions do not apply, they must simply leave it blank. All forms must have original ink signatures and must be properly scanned as OPM will not accept any pictures of these forms. Once the required forms are completed, we review each document to ensure that there are no errors or mistakes and then we send the completed and signed documents to the COR of the contract and to VSC security for processing.

Workweek (Government workweek is Sunday – Saturday): Sunday - Saturday
Overtime:
For Colorado employees (SCA covered): 1.4 x the hourly rate for any hours over 40 hours a week or 12 hours a day, or any 12 consecutive hours.
For California: 1.4 x the employee's regular rate of pay for all hours worked in excess of eight hours up to and including 12 hours in any workday, and for the first eight hours worked on the seventh consecutive day of work in a workweek; and when double time applies, 1.8 x the employee's regular rate of pay for all hours worked in excess of 12 hours in any workday and for all hours worked in excess of eight on the seventh consecutive day of work in a workweek.
For all other states: 1.4 x hourly rate any hours over 40 hours in a work week.
Holidays (only holidays recognized by the Government): 1.4 times the normal hourly rate for all
Federal Holidays worked.
On-Call / Call Back:
Physicians: Must carry a cell phone or pager, $10 per hour On-Call when not at the facility, be within 30 miles or 30 minutes of the facility. Hourly rate applies when physically on site or immediate phone consultation is required for patient care. A minimum of 4 hours billable, when physically on-site, 1 hour billable for phone consultation. Weekend On-Call, 1 hour billable to begin Friday 4:00 pm and end Monday 8:00 am.
Non-Physicians: Must carry a cell phone or pager, be within 30 miles or 30 minutes of the facility.
