Shift Differentials: None Work Week: Sunday – Saturday Overtime Rate(s): 1.2x the billable rate for hours over 40 hours in accordance with applicable laws. Cali-1.2x over 8 hours and 1.5x over 12 hours. Overtime is not authorized for physicians. Overtime will be applicable for SINs 621-030, 621-053, 621-054, 621-058.

Holiday Rate(s): 1.2x the billable rate for the federally recognized holiday worked. Holiday rates are not authorized for physicians. Overtime will be applicable for SINs 621-030, 621-053, 621-054, 621-058.

On-Call Rate(s)/Call Back Rate(s): PHYSICIANS: On call - hourly rate per 12 hour shift to remain within 30 minutes or 30 miles of facility. Call back - Hourly rate with a minimum 2 hours.

NON-PHYSICIANS: On call - $7 per hour to remain within 30 minutes or 30 miles of facility. Call back - If less than 40 hours worked base rate applies if over 40 negotiated overtime rate applies.

Minimum Qualifications of Professionals:

Physician:

Education/Minimum Training: Undergraduate program, medical school (MD or DO), internship and a complete ACGME-approved residency program Experience (# of years): Minimum 12 months experience in the past 24 months or as required Licenses: Active/ unrestricted Certifications: MD or DO who are American Board Certified or eligible by having completed a residency and having at least one year of experience in their respective specialty.

CPR: BLS, ACLS, PALS, and/or ATLS as per RFP or facility requirements Other: Per RFP or facility requirements

Psychologist:

Education/Minimum Training: Master's Degree and/or PhD Clinical Psychology Experience (# of years): Minimum 12 months experience in the past 24 months or as required Licenses: Active/ unrestricted Certifications: Master's Degree and/or PhD Clinical Psychology CPR: BLS, ACLS, PALS, and/or ATLS as per RFP or facility requirements Other: Per RFP or facility requirements

Physician Assistant:
