On-Call (Physicians): One (1) hour of billable wage for 12-hour block of on call. Defined as anytime a physician is assigned to carry a pager or be available for telephone consultations relative to a specific specialty Monday - Friday night, normally between the hours of 4:00 p.m. and 8:00 a.m. Weekend On Call rate is defined as a 24-hour period.
Call-Back (Physicians): Normal hourly rate applies when the provider is required to physically be at the facility and see patients.
On Call (Non-Professionals): $5 per hour to carry a pager or cellphone and must be within 30 minutes or 30 miles of the facility. Defined as anytime a non-physician is assigned to carry a pager or be available for telephone consultations relative to a specific specialty.
Call Back (Non-Professionals): Normal hourly rate applies when the provider is required to physically be at the facility and see patients.

Minimum Qualifications:
621-001 Physician - Family and General
Family physicians diagnose and treat illness, provide preventive care, including routine checkups, health-risk assessments, immunization and screening tests, and personalized counseling onmaintaining a healthy lifestyle. Family physicians also manage chronic illness, often coordinating care provided by other subspecialists. From heart disease, stroke and hypertension, to diabetes, cancer, and asthma, family physicians provide ongoing, personal care for the nation’smost serious health problems.
Years of Experience: Minimum 1 year in specialty.
Minimum Education: A Medical Degree (MD), Degree of Osteopathy (DO) and/or Certification of
Education Commission for Foreign Medical Graduates (ECFMG). Completion of a Residency Training
Program in their specialty approved by the American Council of Graduate Medical Education (ACGME).
Completion of a Fellowship Training Program in their sub-specialty (approved by the ACGME) if required for Certification.
Certifications: Board Certified or Board Eligible (within four (4) years of completion of residency or
Fellowship program). The Government can waive this requirement if so requested.

621-002 Physician-Emergency Medicine Level 1
Physician focuses on the immediate decision making and action necessary to prevent death or any further disability both in the pre-hospital setting by directing emergency medical technicians and in the emergency department. This specialist provides immediate recognition, evaluation, care, stabilization and disposition of a generally diversified population of adult and pediatric patients in response to acute illness and injury.
Years of Experience: Minimum 1 year in specialty.
Minimum Education: A Medical Degree (MD), Degree of Osteopathy (DO) and/or Certification of
Education Commission for Foreign Medical Graduates (ECFMG). Completion of a Residency Training
Program in their specialty approved by the American Council of Graduate Medical Education (ACGME).

621-003 Physician-Internal Medicine – Nephrology
A nephrologist is an internist who treats disorders of the kidney, both chronic and acute, disorders of high blood pressure, fluid, electrolyte and mineral balance, and manages complications of kidney failure necessitating dialysis (removal of fluid and body wastes) when the kidneys do not function.
Years of Experience: Minimum 1 year in specialty.

Education Commission for Foreign Medical Graduates (ECFMG). Completion of a Residency Training

621-003 Physician-Internal Medicine-Hospitalist
A hospitalist is a doctor who provides care for patients at a hospital. They have the same education and training as your primary care doctor, but specialize in providing hospital care. They may also have other specialties such as pediatric (childcentered) medicine, internal medicine, or family medicine.
Years of Experience: Minimum 1 year in specialty.
Minimum Education: A Medical Degree (MD), Degree of Osteopathy (DO) and/or Certification of
Education Commission for Foreign Medical Graduates (ECFMG). Completion of a Residency Training
Program in their specialty approved by the American Council of Graduate Medical Education (ACGME).

621-003 Physician-Internal Medicine-Pulmonologist/Critical Care
Critical care pulmonologists work to provide emergency care, working alongside other critical care physicians and intensivists to perform resuscitations and other life-saving procedures. These physicians are often present when patients are suffering from multi-system failure and other life threatening situations.
Years of Experience: Minimum 1 year in specialty.
Minimum Education: A Medical Degree (MD), Degree of Osteopathy (DO) and/or Certification of
Education Commission for Foreign Medical Graduates (ECFMG). Completion of a Residency Training
Program in their specialty approved by the American Council of Graduate Medical Education (ACGME).

621-005 Physician-OB/Gynecologist
OB/GYN provide health care for women related to reproductive health; provide health carebefore, during and after pregnancy and deliver babies; examine patients, order diagnostic tests and make diagnosis; order medication and treatment. These doctorsalso treat issues specific to women such as breast cancer, hormonal disorders and cervical cancer.
Years of Experience: Minimum 1 year in specialty.

Education Commission for Foreign Medical Graduates (ECFMG). Completion of a Residency Training

621-017 Physician-Psychiatrist

621-025 Registered Nurses
A registered nurse (RN) is a nurse who has earned an Associate Degree in Nursing (ADN) or a Bachelor of
Science in Nursing (BSN), has successfully passed the NCLEX-RN exam administered by the National
Council of State Boards of Nursing (NCBSN), and has met all of the requirements to become a nurse outlined by their state’s licensing body (state board of nursing). RN roles range from providing direct patient care and implementing nursing care plans to directing complex nursing care systems, teaching in nursing programs, instructing patients on disease prevention and health maintenance, conducting clinical research, as well as practicing independently in a variety of challenging settings.

621-030 Physician Assistants
A physician assistant (PA) is a licensed medical professional who holds an advanced degree and is able to provide direct patient care. They work with patients of all ages in virtually all specialty and primary care areas, diagnosing and treating common illnesses and working with minor procedures.

Minimum Education: A Medical Degree (MD), Degree of Osteopathy (DO) and/or Certification of

621-039 Medical Assistant/Medical Administrative
