3. Minimum order: 8 weeks at 40 hours per week for an 8-hour shift or 36 hours per week for a 12- hour shift with no guaranteed shifts or lead times.
Placement fees are waived. Short Term/Per Diem requests are 8-hour shifts minimum.

9. Prices shown herein are “net” ceiling prices-not to exceed rates. The net ceiling rates include malpractice insurance for non-personal services task-orders and exclude travel and lodging. Personal Services task-orders will also be accepted. See pages 2 through 3 of this price list.
10. Minimum Qualifications of Professionals: Minimum 12 months of professional experience, preferred.
11. Medical Liability Insurance Limits: $1,000,000 per occurrence; $3,000,000 aggregate.

Overtime Rate(s): Overtime with approval only. Overtime rate is paid at 1.35 x hourly bill rate for any hours worked over 40 hours per week. Overtime will be calculated by State laws. In States where Daily Overtime applies, (such as California) it will be calculated at the hourly bill rate for the first 8 hours; 1.35 x hourly bill rate after 8 hours, and 2.0 x hourly bill rate after 12 hours regardless of shift assignment or over 40 hours per week.

Annual Rebate: 1% annual rebate for all sales made under this contract over
$500,000.00 per rebate year. The rebate percentage will be applied to the difference between the base amount and actual sales. The annual rebate applies to all products/services sold under this contract, including all products/services added subsequent to initial contract award. The base amount will be prorated for any partial year sales reports
