Scope of Work for Design-Build X-Ray Suite Site Prep

PROJECT # 631-18-008

Department of Veterans Affairs Central Western Mass Healthcare System 421 N. Main Street Leeds, MA 01053

1.0 Project Summary:

The VA Medical Center at Central Western Massachusetts has procured a GE Discovery XR656 GII Radiographic System (XR656) to replace the aging Philips Optimus 80 DR X-Ray (80DR). This project includes designing and the site prep/modification of room 2112 to receive the new XR656.

The XR656 will be installed into Room 2112 by GE engineers upon completion of the site prep work by the Contractor as required by this SOW and in accordance with the GE Health Care Site Planning Pre Install Manual and drawings. The GE references are available for download from http://www3.gehealthcare.com/en/Support/Site_Planning.

The 80DR will remain operational during construction and the installation and commissioning of the XR656. Upon completion of the in XR656 installation by GE Engineers the Contractor shall finish the outstanding work for this room.

Once the XR656 is operational, GE Engineers will decommission and remove the 80DR from room 2108. After removal of the 80DR the Contractor shall prepare room 2108 to receive a yet to be procured Fluoroscope. The Contractor shall prepare the electrical and HVAC loads to receive a notionally selected SIEMENS LUMINOS AGILE MAX or equal Fluoroscopy System. Additionally the design shall prepare the walls, celling, lighting, and doors for this space, as required and remove and install flooring.

This Contract also includes converting rooms 2110 (change room) and 2108 (staff bathroom) into a common control room to serve both 2112 and 2108 rooms. Room 2108A (patient bathroom) shall be modified to serve both rooms with a doors to both rooms and a means to provide access to the room by one patient at a time. The existing layout is shown by figure 1 below as a reference.

2.0 General Design and Construction Requirements:

2.1 The Contractor shall develop a formal schematic design, design development, construction documents and submissions, to include construction cost estimates in accordance with the VA’s master construction and design guidelines, outlined in the Technical Information Library (TIL), located here: https://www.cfm.va.gov/TIL/spec.asp, and also in accordance with GE Health Care Site Planning Pre Install Manual and drawings, located here: http://www3.gehealthcare.com/en/Support/Site_Planning .

2.2 The Contractor shall conduct a survey of the VA facility and all areas where work will be conducted. The survey shall include a validation of all drawings and dimensions provided as references.

2.3 The Contractor shall provide a design to modify rooms 2108, 2108A, 2110 and 2110A, and 2108 into a radiology suite.

2.3.1 The Design shall prepare room 2112 to receive the (XR656) and the following:

2.3.1.1 The Room shall meet the requirements of VA Radiology Service Design Guide, VA PG18-5 Radiology Service (276), and the GE Health Care Site Planning Pre Install Manual (PIN) and Drawings. The GE references are available for download from: http://www3.gehealthcare.com/en/Support/Site_Planning

2.3.1.2 The Contractor shall coordinate the delivery and installation of the XR656 with the GEHC Project Manager (Margaret.Mullin@ge.com), and Project Architect, Jack Timm (John.Timm@med.ge.com).

2.3.1.3 The Contractor shall maintain a presence during the installation, and provide assistance as required.

2.3.1.4 Upon completion of the installation the Contractor shall complete the installation.

2.3.1.5 GE X-ray Arrangement Sketch has been provided (SOW Attachment 1) as a reference for the design. The room depicted in the drawing is not room 2112.

2.3.1.6 The Contractor shall coordinate the design with the GE Health Care (GEHC) Project Manager and incorporate Government approved comments and recommendations provided by the GEHC team.

2.3.1.7 The Control room for this space shall be shared with 2108.

2.3.1.8 Shared bathroom access with room 2108, that meet the VA Barrier Free Design Standard supplement (Available for download from the TIL) of the Architectural Barriers Act Accessibility Standards (ABAAS).

2.3.1.9 This room shall meet the following schedule requirement of 08/30/2018 for the completion of the XR656 install.

2.3.2 The Design shall prepare room 2108 as follows:

2.3.2.1 The room shall meet the 2.3.1.1 The Room shall meet the requirements of VA Radiology Service Design Guide, VA PG18-5 Radiology Service (276).

2.3.2.2 Demolition of the control booth.

2.3.2.3 Removal of the floor and installation of new flooring.

2.3.2.4 HVAC and Electrical service shall be sized and provided to accommodate a notional SIEMENS LUMINOS AGILE MAX or equal Fluoroscopy System.

2.3.2.4.1 An electrical disconnect switch shall be provide to permit the future installation of the Fluoroscope.

2.3.2.5 The Control room for this space shall be shared with 2112.

2.3.2.6 2.3.1.8 Shared bathroom access with room 2112.

2.3.3 The Design shall include the conversion of rooms 2110 and 2110A to be a common control room serving both rooms 2112 and 2108. The door leading to the hallway from room 2110 shall be removed.

2.3.4 The Design shall include the renovation of room 2108A bathroom to serve as a common bathroom for rooms 2108 and 2112. The bathroom shall meet the VA ABA standards. The bathroom shall include a means to restrict access from either 2108 or 2112.

2.4 The Contractor shall submit the shielding design (drawings, bill of materials, specs) for approval for both the X-Ray and Fluoroscope rooms for evaluation by the Government or the Governments independent Physicist Contractor. Discrepancies and/or recommendations shall be corrected by the Contractor.

2.5 A post effectiveness shielding test and leak survey shall be conducted for both the X-RAY rooms and incorporated into the design and construction package to be conducted after construction and commissioning of the x-ray by the Government or the Governments independent Physicist Contractor. Discrepancies shall be corrected by the Contractor.

2.6 The Contractor shall provide a design that incorporates the color schemes, floor types, cabinet types, lighting, and case work shall follow the color and design types as in the CT Scanner Suite as approved by the VAMC interior designer for all rooms.

2.6.1 A modular storage closet for linen and a separate storage cabinet with solid surface countertops for medical supplies shall be provided for both X-RAY rooms.

2.7 The Design and construction shall include running network connections from the control to the nearest IT closet (TBD). Three network connections for each X-Ray in the control room are required to integrate with the VAMC IT network.

2.8 The design documents shall include the phased demo and installation schedule as described within paragraph 1.0 Project Summary, and shall ensure that the either X-Ray is usable during the construction process to the maximum extent possible. This may include scheduling weekend work to accommodate this schedule. However, if and extended work period is required the contractor shall clearly indicate this in the project plans and schedule so the Medical Center can make alternate exam arrangements.

2.9 Environmental conditions shall be maintained to other spaces.

2.10 Design documents shall clearly show all demo, existing, re-located, and new raceways, transformers, bus connections, wire ways, and cabling to/from equipment, lights, floor and ceiling penetrations, medical gases, monitors, sprinkler heads/distribution lines, smoke detectors, electrical outlets, information technical outlets, Nurse Call, Emergency Call, HVAC machinery, Ventilation and grilles, speakers, etc.

2.11 Design documents shall clearly show all infection control requirements. The Contractor shall coordinate with the station multi-disciplinary committee and infection control prevention staff to design infection control requirements.

2.12 The design shall include the testing of all lead paint (present above and below the ceiling tiles), asbestos glue daubs present on ceiling surfaces, asbestos pipe insulation if present, and any asbestos ceiling, floor tiles and/or mastic. Confirm by testing which locations have these materials present. If present the Contractor shall include abetment.

2.13 Reference drawings have been provided, which include historical drawings for the building. These drawings are provided as-is; the information on these drawings require independent Contractor validation. SOW Attachment 2 – Building 1 Drawings in electronic format including reference historical drawings.

2.14 All rooms require asbestos abatement of floor tile; the attic space above these rooms also requires the removal of unused hung 4” to 10” asbestos insulated HVAC ductwork including unused air handling equipment (Motors, pumps etc.).

2.14.1 All penetrations between floors, walls, and the roof shall be repaired after the removal of the piping, motors and controllers

2.14.2 The abatement and disposal of all hazardous materials is to be specified in the construction drawings and specifications.

2.14.3 VA requires asbestos, lead and other hazardous materials to be completely removed from the building and for the construction contract to be able to deliver certification for HAZMAT free areas.

2.14.4 The Contractor shall provide third party Industrial Hygienist (IH) services for monitoring general contractor during asbestos abatement in conjunction with the Governments IH.

3.0 Discipline Specific Requirements:

3.1 Architectural

3.1.1 Design and construction shall include finishes for immediate and adjacent spaces affected by construction.

3.1.2 Design and construct all spaces using current VA design standards and design standards specific to the site. Contractor shall field verify all room measurements and impacted existing conditions.

3.1.3 Contractor shall provide code compliance review. Drawing set shall include code analysis sheet.

3.1.4 Contractor shall verify door openings are adequate with a minimum door size of 39” wide x 82” high.

3.1.5 A reflected ceiling plan shall show structural support, ceiling tiles/construction, overhead lighting, HVAC, sprinklers, speakers, detectors, RFID, etc. All ceiling tiles shall be 2x2 Acoustic ceiling tiles, the ceiling tile type shall be submitted to the Government for approval.

3.1.6 Install new Ceilings, Wall Finishes, and Floors.

3.1.7 Provide required maintenance and servicing clearances and clearly indicate on floor plan.

3.1.8 Provide ceiling openings for maintenance access as required. Contractor shall design and construct wall blocking for equipment mounting.

3.1.9 Contractor shall be provided with specific purchase orders that will describe accessory items. Contractor shall be responsible for locating, detailing, and building accessory mounting systems.

3.1.10 Contractor shall coordinate electrical raceways and floor penetrations with all other work, and shall be designed to meet radiological service per the VA specifications.

3.1.11 Lighting and structural support modifications shall be included in the design and construction.

3.1.12 Relocations and modifications of HVAC, electrical and plumbing shall be included in the design and construction, as necessary to bring up to code or as needed to support new equipment.

3.1.13 New data and phone cabling shall be included in the design and construction.

3.2 Interior Design

3.2.1 Contractor shall work with the VA Interior Designers for finish schedule. Submit color board for each room when finishes have been selected to the Government for approval.

3.3 Structural

3.3.1 Contractor shall be responsible for design and detail of floor, wall, and ceiling structures in accordance with the weights, moments, and forces as shown in design documents.

3.4 Electrical/Low Voltage

3.4.1 Existing electrical systems and components shall be field-verified by the Contractor’s electrical engineer.

3.4.2 Electrical work shall meet current National Electrical Code (NEC) requirements.

3.4.3 Design and Construction shall include all electrical plans, elevations, sections, details, diagrams and schedules showing existing power, new power, and low voltage.

3.4.4 Emergency Call System switches shall be relocated into the control room a switch shall be provided for each x-ray room.

3.4.5 Contractor shall show all necessary normal and emergency service panels, circuits, cabling, wiring, conduit, relays, switches, and disconnects for a complete and usable electrical system to support the new suite including any new HVAC systems and ancillary equipment. Each component shall be identified and labeled on design drawings.

3.4.6 Incoming power supply for equipment shall be dedicated back to source, isolated, and insulated from any other equipment such as elevators, generators, HVAC systems, etc.

3.4.7 Contractor shall design and provide a main power disconnect, installed locally into each room, to serve the XR656 and the future fluoroscope. To allow the GE Contractor to connect, and for the future fluoroscope engineer to connect.

3.4.8 Contractor shall design and provide 3-phase, 480V AC system as required. Including the reuse or replacement of existing transformers. Contract shall include all low voltage systems necessary to meet code and VA standards. The low voltage systems include; fire alarm, public address, voice, data, wireless systems, and Radio-frequency identification (RFID)

3.4.9 The Contractor shall evaluate the use of an onsite transformer (installed for temporary used during the CT upgrade. For temporary use during the phasing of this contract to allow both X-Ray rooms to simultaneously operate.

3.4.10 The Design shall include all low voltage systems necessary to meet code and VA standards. The low voltage systems include; fire alarm, public address, voice, data, wireless systems, and Radio-frequency identification (RFID).

3.4.11 An Electrical Outlet and Floor Space shall be provided for a blanket warming cabinet Enthermics Medical Systems EC350 BL (http://www.enthermics.com/) for the fluoroscope and the x-ray rooms. The Government will procure and install the blanket warmer after commissioning of the XR656.

3.4.12 Utilize site-specific construction specifications for low voltage and telecommunications.

3.4.13 The Contractor shall coordinate electrical raceways and floor penetrations with all other work.

3.4.14 The Contractor shall coordinate testing and installation to ensure the Fire Alarm System meets requirements.

3.4.15 The Design shall include fire alarm system with full area smoke coverage that is compatible with system at specific site.

3.4.16 The Contractor shall include necessary cable trays/ducts/conduits, correctly sized and located in accordance with vendor equipment drawings.

3.4.17 The Contractor shall include circuit breakers, emergency off button, warning lights, call switches, and door switch connections.

3.4.18 The Contractor shall include layout for equipment cabling. There are cable length limitations that shall be assessed and described on drawings. The Design shall include raceways and floor penetrations.

3.4.19 The Contractor shall follow and describe each junction box and wire duct/raceway guidance provided in vendor drawings.

3.4.20 The Contractor shall include wiring with tails and labeling as indicated in vendor drawings.

3.4.21 The Contractor shall include circuit breakers that meet load current rating and short circuit current rating described in vendor drawings.

3.4.22 The Design and Construction shall be suitable for radiological service.

3.4.23 Electrical specifications shall include requirement to test and verify equipment power supply to ensure it meets specifications.

3.4.24 The Contractor shall include grounding and bonding requirements.

3.4.25 Upgrade existing 120V systems to meet current National Electrical Code, (NEC) as required.

3.4.26 The Design shall include 2 ea. hospital grade 120V electrical outlets installed on opposite walls.

3.4.27 The Contractor shall provide a commissioning plan with checklists for the electrical service and equipment.

3.4.28 The Contractor shall provide an Arc Flash and Coordination Study. Arc Flash labels shall be provided.

3.5 Mechanical

3.5.1 Field verify existing conditions. Design for installation and replacement/repair of ductwork, VAV boxes, controls, valves, and other components, as needed.

3.5.2 Exam the potential use of the recently HVAC system that services the CT room, adjacent to room 2108 for the X-Ray suite. If the capacity exists modify the system as required to serve the suite. If not the design shall include the installation of a new HVAC system to serve the suite. Cooling shall be provided to the suite 24 hours a day 7 days a week.

3.5.3 Each room shall have independent climate controls.

3.5.4 Contractor shall include all mechanical plans, elevations, sections, details, schedules for controls, sequence of operations, pumps, valves, piping systems, gages and other devices to support the installation of the new mechanical systems.

3.5.5 Contractor shall study the capacity of existing air handling unit to support the required air exchanges.

3.5.6 Design and install for the appropriate amount of air exchanges in accordance with VA HVAC design manual dated November 1, 2017.

3.5.7 Whenever possible, use existing piping inside the building to minimize installation cost and hospital disruption.

3.5.8 Contractor shall design and connect to the existing Building Automation System (BAS) infrastructure and software as required for any new mechanical equipment.

3.5.9 Rebalance existing supply, return, and exhaust for the immediate imaging area and adjacent spaces. Rebalance existing air handler as required.

3.5.10 A complete “BAS Design” that includes: sequence of operations, instrumentation drawings, and points list.

3.5.11 The design shall include the safety components required by the current ANSI/ASHRCONTRACTOR Safety Code.

3.5.12 Provide necessary isolation valves. Include replacement of existing isolation valves that may need replacement due to age.

3.5.13 Each piece of equipment shall be specified to achieve the highest practical level of energy efficiency.

3.5.14 Contractor shall include any required modifications to the fire sprinkler system.

3.5.15 Contractor shall provide a commissioning plan with checklists for HVAC equipment.

4.0 Schedule Requirements:

4.1 The Schedule shall meet the dates and criteria, in addition to the Project Schedules requirement of Attachment 5 Section 01 32 16, as follows:

4.1.1 The Design, installation, and commissioning of the XR656 shall be completed by 31 August 2019. The punch list for room 2112 shall be completed within 7 calendar days after commissioning of theXR656. Commissioning includes light-off, testing, and calibration, but does not include training,

4.1.2 The CONTRACTOR and the Contractor are responsible for contacting GE Health Care Program Manager, Marge Mullin (Margaret.Mullin@ge.com), and Project Architect, Jack Timm (John.Timm@med.ge.com) to coordinate the installation and commissioning schedule for the XR656 and the decommissioning and removal of the DR80.

4.1.3 Work for the X-RAY suite shall be completed within 30 calendar days after removal of DR80. This includes resolving the punch list for the remaining rooms.

4.1.4 The Schedule shall include a design review for the Government Shieling Analysis.

4.1.5 The schedule shall be provided to the Government for review and approval.
