CONTRACTOR SCOPE OF WORK
VA MEDICAL CENTER PROVIDENCE, RI
Project Number: 650-15-201 Project Title: Construct Site Preparation for New CT Scan for Siemens Symbia Intevo SPECT/CT
1.0 GENERAL:
It is the intent of this Scope of Work that a Design-Build Contractor shall provide Architect/Engineer professional services and construction services for site preparation for the installation of the new equipment in Room 284 of Building 1.
This project will provide for construction of utilities services and finishes necessary in the designated locations to allow for the installation of the Symbia Intevo SPECT/CT by the Equipment Contractor. The work shall include, but not limited to, all labor, transportation, materials, apparatus, tools, equipment and permits necessary to provide for the removal of the existing flooring, floor repairs, new flooring, patching and painting of walls, removal of the existing drop ceiling, new ceiling, and relocation of electrical work.
Strict noise and infection control measures shall be followed. Existing life safety systems within Building 1 shall not be impaired at any time during the performance of the work.

2. PROJECT SCOPE OF WORK:
After award, but prior to the start of any construction, the Design-Build Contractor shall contact the VA COR to review proposed methods and plans for completion of work and to review infection control procedures, interim life safety procedures, above ceiling and wall penetration permits, hot work permits and the contractor’s site-specific safety plan. Field work shall not commence until these plans have been submitted, accepted and/or approved as detailed below.
The Design-Build Contractor shall provide the following services for the design, documentation, construction, and construction administration for the preparation of installing a Siemens Symbia Intevo SPECT/CT by the Equipment Contractor.
2.1 Design Services
A. Basic Services: The following tasks shall be performed:
1. Provide the services of a design professional that is registered and licensed in the various design disciplines necessary for the work required, to prepare construction documents and to provide construction support services as further described in this Scope of Work.
1. Provide conceptual designs for review by the VAMC Providence staff to determine the appropriate functional arrangement of the space designated for installation of the new equipment. Revise conceptual designs as required to address comments by VAMC Providence staff. One conceptual design will be accepted by VAMC Providence staff as the basis for design development.
1. Incorporate all items of work required or recommended by the equipment manufacturer for the successful installation and operation of the equipment by the Equipment Contractor.
1. Provide architectural design to meet the needs of the proposed use and in accordance with the accepted conceptual design.
1. Conduct a structural survey and investigation of the project location to the extent necessary to determine the ability of the existing building structure to support all loads imposed by the work of this project. The structural survey and investigation shall be conducted by a qualified structural engineer. The structural engineer shall determine whether the existing structure can support the new equipment. If the structural engineer determines that the existing structure can support all loads imposed by the work of this project and that no supplemental framing and/or reinforcement is necessary, then the structural engineer shall provide a written certification to that effect. Conduct a survey and perform a study of the existing normal and emergency electrical power distribution systems to determine whether sufficient electrical capacity is available to meet the needs of the proposed use. Determine needs for normal and emergency electrical power. All electrical power needs shall be determined by a qualified electrical engineer and the design of electrical work shall be prepared by a qualified electrical engineer.
1. Conduct a survey and investigation of the mechanical, plumbing, and fire protection systems of Building 1 to the extent necessary to determine their available capacities and the ability of these existing systems to meet the needs of the proposed use.
1. Investigate the existing HVAC system and determine if the existing system will meet the need of the proposed use. AC needs shall be determined by a qualified mechanical engineer and the design of HVAC work, if any, shall be prepared by a qualified mechanical engineer. The Metasys System is the Providence VAMC’s site energy management system.
1. Provide telecommunications and data connection devices to suit the needs of the proposed use. Telecommunications and data work shall be performed in conformance with Providence VAMC requirements and standards.
1. Provide all necessary engineering and design for plumbing, fire protection, electrical systems, alarms, security, radiation shielding, architectural, structural, and all other work to meet the needs of the proposed use and for a complete and usable installation. Radiation shielding requirements will be developed and tested by a PVAMC physicist.
1. Prepare construction documents for the proposed work. Documents shall consist of construction drawings and technical specifications and shall be prepared in sufficient detail to fully describe the extent of work required for a complete and fully functional installation and to demonstrate compliance with all VA Standards and contract requirements.
1. Provide construction period services by the Design-Build Contractor’s design professional, consisting of review of product and material submittals and shop drawings, respond to inquiries for clarification of construction documents prepared by the design professional, review change order proposals when requested by the VA, and prepare as-built drawings upon completion of the construction.
1. Provide site visits by the Design-Build Contractor’s design professional during construction, and provide one final inspection of the completed work by the Design-Build Contractor’s design professional to determine whether the work was completed in accordance with the approved construction documents.
0. Structural Engineering Design Services: If the investigations by the Design-Build Contractor’s structural engineer determine that the existing structure cannot support the new work, the Design-Build Contractor shall provide design of whatever supplemental framing and/or reinforcement is necessary to carry the weight of the new equipment.
0. Electrical Engineering Design Services: If the investigations by the Design-Build Contractor’s electrical engineer determine that either the existing normal or emergency electrical power systems, or both, are insufficient to meet the power needs of the new work, the Design-Build Contractor shall provide design of whatever improvements to the existing electrical power systems are necessary for the proposed use.
1. The Design-Build Contractor shall be responsible for the entire design of this project, including confirmation of existing conditions. The Design-Build Contractor shall also be responsible for developing a detailed infection control plan for the construction work. This plan shall be based on medical center infection control policies and will be site specific for the areas involved in the project. The design must include a phasing plan that minimizes any negative effects to the normal operations of the medical center and this building.
2.2 Construction Services
Provide all construction of site preparation as shown on the construction documents approved by VA Providence and in accordance with the instructions and recommendations of the equipment manufacturer.
Demolition
After Notice to Proceed, but prior to the start of any construction work, the Design-Build Contractor shall contact the VA COR to review proposed methods and plans for completion of work and to review infection control procedures, interim life safety procedures, above ceiling and wall penetration permits and hot permits. Field work may not commence until these plans have been approved as detailed below.
1. Remove and dispose of existing suspended ceiling.
2. Remove and dispose of existing VCT flooring.
3. Remove and dispose existing cabinetry and sink.
4. Carefully remove existing window blinds and store in a safe location for reuse.
5. Contractor to provide dumpster(s) for demolition materials.
New Work
1. Provide construction documents consisting of drawings and specifications for the work to be performed. Construction documents shall be prepared by design professionals who are licensed in any US state in their design discipline. See below for further design requirements.
2. Upgrade the room to meet Exam Room criteria including relocation of sprinkler system, if required.
3. Install a new ceiling grid system with 2 x 2 tiles.
4. Install new lighting for the entire room.
5. Install new staff wall-mounted hands-free stainless steel sink as shown on the plans.
6. Repair and level the existing concrete floor using Ardex self-leveling concrete or equal material to achieve the standards for level and flatness specified by the manufacturer of the new equipment. Flooring shall be seamless vinyl sheet with integral base and with trim caps. Patterns and colors to be determined.
7. Clean, patch and repair existing surfaces. Remove and reinstall items mounted on the walls. Apply paint in two coats; body and finish. First coat applied is body coat and second coat is finish coat. Color shall be as selected by the VAMC Providence from the range of colors available from Pittsburgh Paints or matching color by other acceptable paint manufacturers. Paint the new booth walls and remaining existing and new walls, frames, and doors.
8. Contractor shall provide firestopping within the limits of work, including the firestopping of existing penetrations that are exposed by the work of the project.
9. Restore all other surfaces, including window blinds, disturbed by the work to pre-existing condition.
Electrical, Data/Telecom, and Fire Alarm
1. Provide electrical work specified by Siemens, the manufacturer of the new Symbia Intevo SPECT/CT. Provide power connections, outlets, terminal boxes, cables, conduit, pull boxes, controls, switches, conduits and raceways, and devices as required for the installation of the new Symbia Intevo SPECT/CT equipment.
2. Siemens shall install all wiring required for the new equipment as indicated on Siemens documents and plans.
3. Provide data connection means at the designated control booth and other locations as required in accordance with the standards of the VAMC Providence. Installation of both telephone and data must be complete to include all cabling and connections. Work must conform to Providence Cabling and Quad Drop specifications.
4. Termination into the local IT closet and testing of all telephone and data systems is the responsibility of the contractor.
5. Conduit, wiring, connections and programming to the existing Energy Management System are the responsibility of the contractor.
6. Modifications to the existing Fire Alarm system will require fire protection engineering redesign and support using licensed professionals. Existing fire alarm system “as built” or “shop drawings” will be verified by Design-Build A/E against actual system assembly prior to any redesign modification. All modifications will require updated drawings to be delivered to the VA after testing and commissioning has been completed. All work performed must meet current; Healthcare, National Fire Protection Association (NFPA) & The Joint Commission standards.
Modifications to the system in which to add, delete, extend, or relocate, field devices and or panels require extensive knowledge and experience with this state-of-the-art system. Use existing wiring conventions for all circuit types and perform additional load calculations for audio amplification circuits to ensure maintaining a consistent operable and reliable system by manufacturer’s specifications – no exceptions. A 2-Hour Fire Rated Cable/Conduit is required from panel to panel. Wiring inside a smoke zone for another smoke zone must also be in 2-Hour Fire Rated Cable/Conduit. All tamper and flow switch devices must have their own exclusive addressable interface monitor module (Notifier P/N FMM-1 Flash Scan Monitor Module) – no exceptions. There will be no substitutes.
In addition to the hardware modifications, extensive programming and reprogramming may be required to update graphic files and audio file messages based on smoke zones and floor plans of field device modifications. Voice activated messages may have to be added, deleted, or modified within an evacuation zone or modify alert messages going out to additional DVC’s – DAA’s or FACP’s around the campus outside of the initiating zone. In this case, the ONYX Workstations (2), ONYX First Vision (2), DVC’s (17), NCA-2 (1 each), NFS2-3030 (2), NFS2-640 (16), may require system reprogramming. Service personnel must be authorized and certified by Notifier to add, delete, modify, or repair any fire alarm system component. No exceptions will be allowed.
3.0 SUBMITTALS
Design Submissions
The Design-Build Contractor shall retain file copies of all items forwarded to the Contracting Officer or his/her Representative (COR) under various design review submissions. Intent of such a requirement is to save time by resolving questions over the telephone, to ensure that copies are available in the event of loss or damage to the originals during reproduction, and to facilitate rework of documents for amendments made necessary by discovery of problems during later stages of project development. The same official of the Design-Build Contractor with whom the contract was negotiated shall sign all correspondence with the Contracting Officer and his/her Representative (COR). All submissions (drawings, specifications, cost estimates, design analysis, etc.) shall be legibly identified as the percent (%) of design submission. Each submission shall incorporate the comments made by the VA at the prior review stage.
Note that there is a limitation of 6% of construction cost, imposed by law, for the preparation of contract drawings and specifications per 48 CFR, Chapter 8, and Part 836.606-73.
a. Drawings: Base all original drawings on standard VA guidelines. Drawings shall be done in plan view at a minimum scale of 1/8" = 1 '0", where necessary, and generally 1/4" = 1'0" or larger when possible to fit on sheet. Details shall have a minimum scale of 1/4" = 1'0", and if possible, it is preferred larger. All details, title blocks, base plans, etc., shall be drawing as "XREFS" bound to the drawing for submission purposes and final issue only. All other usage shall be drawing from a common Master Set, daily if required, from a source maintained on the architect’s master database. The database shall be available by modem to all design disciplines and the VA. The purpose of this requirement is to insure consistency and to eliminate design conflicts by guaranteeing that common elements of the design stay throughout the design life of the project.
Drawings shall be independent for architectural, mechanical, electrical, structural, civil, and any other design disciplines for the project, and shall show, on separate views, required demolition and reconstruction details for each function.
Drawings shall be prepared in accordance with United States National CAD Standard and VHA Program Guide 18-3, VHA National CAD Standard Application Guide. Electronic version of drawings shall be submitted using IBM compatible "AutoCad" 2014 and in Portable Document Format (.pdf). Final drawings shall be plotted in ink on 30" x 42" polyester film sheets. Sepias are not acceptable as original drawings. Final drawings submitted by the Design-Build Contractor shall be originals, shall be the result of Design-Build Contractor field investigation, and shall not be direct takeoffs or tracings of any drawing provided as information.
Final drawings shall bear the VA title block and nomenclature on polyester sheets as are the sample sheets supplied by the VA. All drawings in a project set shall be on the same type and size sheets. All drawings are required to show existing conditions as of the date of the design submission.
All contract drawings shall be produced with a view to the use of half-size reductions. Pertinent criteria to assure this result are as follows:
0. Drawings shall provide uniform photographic quality.
0. Minimum lettering height shall be not less than 1/8 inch.
0. Legends and symbols as well as size, spacing and location of notes, dimensions, etc. shall be such as to produce legible half-size reproductions.
0. No lettering, shading or marking on the reverse side is acceptable.
0. Clearly distinguish between new work and existing work.
0. Graphic scales shall be provided on all drawings.
b. Specifications: Provide complete project specifications, to include all sections of CSI Divisions 1-50 that, in the opinion of the Design-Build Contractor, are necessary to properly and adequately construct the project by a construction contractor. Specifications shall be based on the VA Master Construction Specifications. Specifications shall be prepared in MS Word, IBM compatible format. The Design-Build Contractor shall review and edit the applicable sections of the specifications as to completeness, relevance, and correctness. The intent is to provide specifications that are suitable for the project intention and detailed enough to enable the contractor to perform the work in the best and simplest possible manner. It is equally important that the Design-Build Contractor "edit out" portions of the specifications that add additional cost to the project without additional value or quality. Not every portion of the master specification is appropriate to this particular project. Where choice of items is referenced in the specification section, the Design-Build Contractor shall consult with VAMC Facilities Management Service for direction on which option to select. The contractor shall not be provided options or choices to select from but must be provided the specific selection. The Design-Build Contractor shall obtain from Facilities Management Service the name and type of specialty materials and equipment that are required for use on the project. All items shown in the specifications shall, if named specifically by brand or production name, carry the statement "or approved equal". Exception may occur due to previously purchased or installed items interacting with new construction. In this situation, items used should be the same as existing or matched accordingly. The VA shall provide information on such items. The Design-Build Contractor shall specify construction in detail in all the specification sections. Tolerances, and workmanship, shall not be allowed to default to "common industry/construction practices”, or any similar vague standard. The legal precedence of the specifications over the drawings shall not be used to resolve design ambiguities between the drawings or notes necessary for clarity. Any contradiction, ambiguity, or statements subject to various interpretations shall be immediately corrected to reflect a single clear intention.
d. Minimum Requirements for A/E Submissions: The cost of reproduction of all review materials is included in this contract.
(1) CONSTRUCTION DOCUMENT SUBMISSION (60%). The following quantities of design review material are required:
1. 3 sets of Plans (Blueline 2-full and 1-half)
1. 2 sets of Construction Cost Estimate
1. 2 sets of marked-up Sections of the VA Master Construction Specifications
(2) CONSTRUCTION DOCUMENT SUBMISSION (100%): The following quantities of design review material are required:
1. 3 sets of Plans (Blueline 2-full and 1-half)
1. 2 sets of Construction Cost Estimate
1. 2 sets of Construction Specifications
1. 1 Electronic Media (CD ROM) copy of the above documents
1. 1 copy of responses to VA comments of previous submission
(3) CONSTRUCTION DOCUMENT SUBMISSION (Final): Upon completion of all revisions, submit the following items:
0. 2 copies of the final project documents on Electronic Media (CD ROM), with all project documents recorded on a single CD ROM, and each copy containing the following:
0. drawings on IBM compatible "AutoCad" version 2014 and in Portable Document Format (pdf)
0. the final specifications in Microsoft Word format
0. the final specifications assembled into a single file in Portable Document Format (pdf)
0. final construction cost estimate in Microsoft Excel
0. Final Design Analysis in a single file in Portable Document Format (pdf).
0. 3 sets of Plans (Blueline- full 2-full and 1-half)
0. 1 set of Original Reproducible Plans with seal of the design professional on each sheet
0. 1 set of Original Reproducible Specifications
0. 1 copy of responses to VA comments of previous submission
e. Delivery Media and Format for Final Documents
1. Format: CAD Drawings shall be prepared in accordance with VHA Program Guide 18-3, VHA National CAD Standard Application Guide. A copy of all data and files developed under this project shall be delivered to the Government in digital format. The data files should be provided on compact disk (CD ROM) in ISO 9660 format. A “Readme.txt” file must be included with the delivered digital media that includes normal transmittal information.
2. Label: The external label for each digital media shall contain the following information:
a. Contract Number and date.
b. Format and version of operating system software, name and version of utility software used for preparation and copying files to media.
c. Sequence number of digital media.
d. List of file names on the digital media
3. Quality Assurance/Quality Control: Before a file is placed on the delivery digital media the following shall be performed:
1. Remove all extraneous graphics outside of the border area and set the active parameters to a standard setting.
1. Check to ensure that all reference files are attached without device or directory specifications.
1. All digital files shall be delivered to the Government uncompressed.
1. Include all files both graphic and non-graphic required for the project (i.e. color tables, pen tables, font libraries, symbol libraries, user command files, plot configuration files, AML plot routines, etc.).
1. Make sure that all support files such as those listed above are in the same directory and those references to those files do not include device or directory specifications.
1. Include any standard sheets (i.e. abbreviation sheets, standard symbol sheets, etc.) necessary for a complete project.
1. Document any fonts, tables, symbols, cells/blocks, line styles/types, details, reference drawings, etc., developed by the contractor, or not provided among the Government furnished materials. The contractor shall obtain Government approval before using anything other than the Government standards.
1. Each finished CADD drawing or map shall have its own separate plot file. The plot file for each finished drawing shall be provided to the Government in a specified plotter configuration.
f. Ownership: All electronic files created and reimbursed for this project shall be the property of the Department of Veterans Affairs.
Prior to commencing any construction work, the Design-Build Contractor shall submit to and receive approval of the VAMC Providence, RI for the following:
Construction Submittals
· The contractor shall submit product samples, manufacturers’ literature, and data as required per the project specifications. In addition, the contractor shall pay specific attention to ensure timely submission of all product samples, manufacturers’ literature, and data covering the materials listed below that will be installed in this project.
a. Project Schedule
b. All floor construction material.
c. All electrical construction material
d. All telephone & data material
e. Paint
f. Control booth partition
g. Infection Control Plan (including dust/infection control barrier drawings)
h. Safety and Interim Life Safety Plan
i. Waste Management Plan
· The contractor shall submit one (1) copies of each submittal for approval prior to installation of the related equipment. Materials shall not be ordered or installed prior to approval of the submittals. Direct all submittals to the designated COR through the CO.
· Certification from the Contractor/Installer that the system specified meets all identified code and insurance requirements as required by the Government.
· Material Safety Data Sheets (MSDS) for all materials to be used in the construction of the work.
Upon completion of construction work and before submitting invoice for final payment, provide to the VAMC Providence, RI the following:
· Operation and Maintenance Manuals for new equipment
· As-built drawings
4.0 CODE REQUIREMENTS
All materials and workmanship shall comply with the codes and standards adopted by the VA.
a. OSHA
b. NFPA Life Safety Code (NFPA 101), latest edition
c. International Building Code, latest edition
d. National Standard Plumbing Code, latest edition
e. All applicable Department of Veterans Affairs (VA) master design specifications and master construction specifications (accessible at http://vaww.va.gov/facmgt/standard/spec_idx.asp or at www.va.gov/facmgt).
f. Providence VAMC Cabling and Quad Drop Specifications
g. National Electrical Code, latest edition.
h. EIA/TIA Standards
Note that where different specifications require different considerations for the same item of work, the more restrictive specification will generally have precedence and will be followed regardless of the specification publisher.
All materials and workmanship shall comply with the codes and standards adopted by the VA. All work shall be done in a first class manner according to the best trade practices and to the satisfaction of the COR.
5.0 PLANNING INFORMATION AND STANDARDS
The applicable portions of Government criteria in conjunction with industry standards shall govern the design and preparation of contract drawings, specifications, cost estimate, and design analysis. The design shall also satisfy applicable State of Rhode Island criteria. Particular emphasis is placed on the following documents that establish minimum requirements:

3. VA Design Manual
4. International Building Code (latest edition), the National Standard Plumbing Code (latest edition), the National Electrical Code (latest edition), and other codes and standards cited in VA criteria, standards, and similar documents.
5. United States National CAD Standard and VHA Program Guide 18-3, VHA National CAD Standard Application Guide
6. VHA Program Guide 18-4, Design and Construction Procedures
7. The Illuminating Engineering Society of North America (IESNA) Handbook
8. VA Manual for Preparation of Cost Estimates for Hospital Projects
9. The project shall incorporate features that meet VA standards for the dignity, security and privacy of patients who utilize the facilities constructed by this project. Minimum standards are provided in Appendix A of VHA Handbook 1330.01, Health Care Services for Women Veterans.
10. Guidelines for Design and Construction of Health Care Facilities, latest edition published by the American Institute of Architects
The Design-Build Contractor or A/E shall obtain all data necessary to prepare the design documents required. The Design-Build Contractor shall confirm all existing conditions and field verify any document provided by the VA. The Design-Build Contractor or A/E is wholly responsible for both the accuracy and the level of completeness of the final product and failure to verify information is not acceptable. Information (drawings and other documentation) provided to the Design-Build Contractor or A/E shall be used for reference only.
6.0 DESIGN SITE VISITS and MEETINGS
The Design-Build Contractor or A/E shall visit the proposed project site to obtain the details and specifics of the relevant existing field conditions in order to fully complete the work of this project without omission. Such visits shall include a thorough inspection of normally enclosed or inaccessible areas such as spaces above the ceilings, within walls or chases, in pipe crawl spaces, and in or under basement floors. Where necessary, Design-Build Contractor or A/E shall arrange to have walls or ceilings opened, at no additional expense to the Government, to examine the interior or hidden conditions to provide complete drawings. Arrangements for visits to make inspections shall be made by contacting Facilities Management Service at least 48 hours before the visit.
The Design-Build Contractor or A/E shall attend meetings at the time and place requested by the Contracting Officer and his/her COR. Minutes of the meetings shall be taken by the Design-Build Contractor and submitted to the Contracting Officer and his/her COR. Meetings shall include the following:
a. Pre-Design Meeting - The Design-Build Contractor or A/E shall meet with the Contracting Officer and/or his/her COR, and, if applicable, the Using Service's Representative, to determine project requirements, the arrangement of the areas, and to clarify the scope of work.
b. Design Review Meetings-The Design-Build Contractor or A/E shall attend meetings with the Contracting Officer and his/her COR, and the Using Service's Representative during field investigations and for review conferences as needed to advance progress of the work. As a minimum, attend one review conference after submission of the 100% documents.
c. Attend one Pre-Construction meeting.
7.0 ANSWERING INQUIRIES & ISSUING AMENDMENTS
The Design-Build Contractor or A/E shall provide prompt answers and recommendations to inquiries made by the Government. In the event of error or omission, the Design-Build Contractor or A/E shall issue design amendments to the project documents to clarify the documents. Those amendments required to correct omissions or errors made by the Design-Build Contractor shall be at no additional cost to the Government.
8.0 WARRANTIES / PROJECT CLOSEOUT REQUIRMENTS
The Design-Build Contractor shall guarantee all provided materials, equipment, and workmanship for a period of one (1) year from the Date of Acceptance of the entire project. During the term of the guarantee, the Design-Build Contractor shall repair or replace all defective material or equipment within 72-hours of notice without additional cost to the Government.
When the work is completed, the Design-Build Contractor shall provide one copy of as-built drawings that indicate the changes made during construction from the final design documents. The Design-Build Contractor shall also provide operation & maintenance manuals for any equipment installed by the Design-Build Contractor for which the manufacturer recommends periodic maintenance.
9.0 PROPOSED SCHEDULE:
Work shall commence upon receipt of a Notice to Proceed from the government and shall be completed within a total of 270 calendar days after receipt of Notice to Proceed.

10.0 WORK LOCATION AND RESTRICTIONS:
The work site location is at the Providence VA Medical Center in Providence, RI. The contract work is intended to be performed at Room 284 in Building 1.
All work shall be performed between the hours of 7:00 AM and 4:30 PM Monday through Friday, holidays excepted, unless other times are arranged in advance and approved in writing by the Project Manager. When the Design-Build Contractor’s work interferes with hospital functions, such as when work produces excessive noise, odors, dust, utility service interruptions, or other interferences with normal hospital operations that cannot be contained within the area of work, the contractor shall perform said work at other than normal hours and as directed by the COR.
11.0 PARKING:
Parking is rigidly controlled throughout the Medical Center. Parking of privately-owned vehicles by Design-Build Contractor personnel is prohibited on the hospital campus and is only allowed at the Davis Park location off Chalkstone Avenue. Parking in designated patient parking areas is strictly prohibited. Parking on grass is also prohibited. Parking for equipment necessary to perform the work will be authorized in advance of starting the project. Parking passes will be issued by the VA Police. Parking by contractors will be regulated in accordance with Providence VA Medical Center Policy Memorandum 07B-3 entitled Registration of Privately Owned Vehicles at Attachment G.
12.0 STORAGE OF EQUIPMENT & MATERIALS:
The Design-Build Contractor shall arrange with the COR for allocation of required workspace and for the storage of equipment and material to be used for this project. Storage space is very limited. There are no exclusive areas within the campus that can be given to the contractor for their storage needs. Additionally, no space will be made available for the placement of a contractor trailer for this project. The Design-Build Contractor should schedule delivery of materials to limit the amount of storage space and time.
13.0 DEBRIS REMOVAL
The Design-Build Contractor shall be required to remove waste materials and debris daily from the campus. All waste material and debris shall be removed off the site by the contractor and shall be disposed of in accordance with applicable State and Federal regulations.
14.0 SITE UTILITIES:
The Design-Build Contractor should note this scope of work does not detail all existing structures, utilities, or components that may potentially interfere with the contract work required. The contractor shall note any obstruction, utility, or condition that may hinder or interfere with the execution of this contract and the contractor shall make provisions in their contract price to resolve such interferences and other conditions that may hinder the proper completion of the work. All proposed utility relocations, reallocations, and shutdowns shall be approved by the COR prior to commencing such work. The contractor shall verify all existing utility installations and take appropriate action prior to working around any potential utility installation.
In the event a shutdown, restriction, or interruption of any utility services is required, a written request must be submitted (at least 15 calendar days in advance) and approved by the VA COR. All utility shutdowns must be reviewed and approved by the VA.
15.0 INFECTION CONTROL:
All work shall be performed in accordance with the Construction Specifications for Infection Control of the Providence VA Medical Center, which is attached (Attachment B). For purposes of this project, the work shall be considered Class III and shall be accomplished using the controls indicated in the specifications and on the Infection Control Construction Permit attached. No work shall proceed until an Infection Control Construction Permit has been completed and signed and all protective measures required by the permit are in place.
16.0 PROJECT SAFETY:
Design-Build Contractor shall comply with the Contractor Safety Manual of the Providence VAMC, dated January 2009 (Attachment A). The US Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, is incorporated by reference and the contractor shall comply with the requirements of this manual. In the event of a conflict between the requirements of EM 385-1-1 and the Providence VAMC Contractor Safety Manual, the more stringent requirements shall apply.
All employees of the Design-Build Contractor and its subcontractors who perform any activity or serve any function on the project site shall have completed the 10-hour or 30-hour OSHA Construction Safety course and other relevant competency training. Contractor shall submit training records of all such employees for approval before the start of work.
The Design-Build Contractor shall identify in writing to the government’s Contracting Officer and his/her COR a “Competent Person” as defined by 29 CFR 1926 (OSHA Construction Industry regulations). The designated competent person shall be on the project site whenever the general contractor or its subcontractors are present on the project site.
All employees of the Design-Build Contractor and its subcontractors are required to attend a 1-hour VA safety briefing before the employee commences any work activities on VA property.
Obtain a Crane Permit when use of a crane is intended. Comply with requirements of Providence VA Medical Center Facilities Management Service SOP Policy Memo 138-16 at Attachment H.
Design-Build Contractor shall submit a site-specific Safety Plan that provide project-and site-specific activity hazard analyses and accident prevention plans. The Contractor’s site-specific Safety plan shall be submitted to the VA for information purposes. The Safety Plan shall conform to the requirements of FAR 52.236-13 and shall include, as a minimum, provisions for the following:
· Site access and control to restrict access by unauthorized persons and allow for separation of VA staff, patients and visitors from construction personnel.
· Site security to restrict unauthorized entry by contractor personnel into areas of building 1 determined by the VA to be non-accessible; and to address the need for identification badges to be worn by construction personnel; key control; and loading/unloading of materials and wastes.
· The contractor’s substance abuse policy and training requirements
· Contractor’s plan for site safety and health inspections
· Contractor’s plan for safety and health training
· Contractor’s site-specific fall protection program
· Contractor’s site-specific electrical safety plan
· Contractor’s requirements for use of personal protective equipment (PPE).
· Contractor’s accident reporting and investigation program. The contractor shall submit a written incident report to the VA Project Manager within 24 hours after any accident, injury, occupational illness, or other safety-related incident occurs, regardless of how minor the nature of the incident.
· Contractor’s emergency action plan and fire prevention and protection plan, to include training of contractor personnel in the provisions of these plans.
· Contractor’s minimum safely training requirements for its personnel and the personnel of its sub-contractors.
· Contractor’s requirements for sub-contractor conformance to the site-specific Safety Plan
· Identity of the Contractor’s designated “Competent Person” as defined by 29 CFR 1926 (OSHA Construction Industry Regulations). The contractor shall provide a Competent Person who shall be on the project site whenever the general contractor or its subcontractors are present on the project site.
· Contractor’s protocol for inspections by regulatory agencies.
17.0 PENETRATIONS OF FIRE AND SMOKE BARRIERS:
1. Prior to any installation of equipment, cables, power connections, conduit, piping or other work that penetrates a smoke or fire barrier, all such work must be approved by Facilities Management Service (FMS) of the VAMC Providence. A penetration permit must be secured from FMS prior to disturbing the integrity of the barrier. The permit must be available for inspection at the project location (Attachment D). After the work is completed, the penetration must be repaired (sealed) according to accepted practice and utilizing materials (including UL/FM-listed through penetration fire stopping materials) that meet the original barrier construction requirements in order to restore the designed specifications for smoke and fire compartmentalization. All penetrations and miscellaneous openings must be protected according to NFPA 101, Chapter 8.
2. Ensure that all penetrations made in fire resistance assemblies of the existing hospital building, to include smoke barriers, fire separation assemblies, and fire walls, are properly fire stopped within 4 hours after making the penetration.
3. Identify through-penetration fire stop systems with pressure-sensitive, self-adhesive, preprinted vinyl labels. Attach labels permanently to surfaces of penetrated construction on both sides of each fire stop system installation where labels will be visible to anyone seeking to remove penetrating items or fire stop systems. Include the following information on labels:
· The words: "Warning -Through Penetration Fire stop System-Do Not Disturb. Notify Building Management of Any Damage."
· Contractor's Name, address, and phone number.
· Through-Penetration fire stop system designation of applicable testing and inspecting agency.
· Date of Installation.
· Through-Penetration fire stop system manufacturer's name.
· Installer's Name.
4. Upon completion of any penetration fire stopping, a visual inspection for approval must be requested from, and completed by FMS.
18.0 INTERIM LIFE SAFETY PROTECTION MEASURES:
Design-Build Contractor shall participate with the VA in the preparation of an interim life safety measures and fire protection Plan (Attachment E) that will be implemented during construction of this project. At a minimum, contractor shall comply with the following requirements of an interim life safety measures and fire protection plan:
· Ensure exits provide free and unobstructed egress for all building occupants. If required by contractor’s operations, establish and mark alternate means of egress for all building occupants.
· Contractor shall maintain escape facilities from the work area for construction workers at all times. Means of egress in construction areas will be inspected daily.
· Ensure free and unobstructed access to all areas of the project site for emergency services and for emergency forces.
· Do not impair means of egress, automatic sprinklers, smoke and heat detection, and fire alarm systems, except for portions immediately under construction, and temporarily for connections. Provide fire watch in accordance with NFPA standards for impairments more than 4 hours in a 24-hour period. Request approval of impairments from the Project Manager in writing a minimum of 72 hours in advance.
· When required, a fire watch shall be by a qualified person provided by the contractor who shall maintain constant observation of the affected area and have no other duties. The person providing the fire watch shall be trained in fire prevention and in the use of fire extinguishers, occupant hose lines, occupant fire protection system, in sounding the building fire alarm and in notifying the local fire department, and in understanding the particular fire safety situation for the project.
· Provide written procedures and guidelines for construction personnel and post in the immediate areas of construction detailing what to do and who to call in the event of fire or emergency.
· Provide signs to identify exit access, exits, and exit discharges as needed for interim life safety measures that are required for the contractor’s work.
· Maintain the construction area to minimize the potential for fire or safety hazards resulting from storage of construction material, construction waste and debris during construction operations.
· All temporary construction shall be built of noncombustible/fire retardant materials and shall be smoke tight.
· Ensure that all penetrations made in fire resistance assemblies of the existing hospital building, to include smoke barriers, fire separation assemblies, and fire walls, are properly fire stopped within 4 hours after making the penetration.
19.0 PREVENTION OF FALSE FIRE ALARMS:
Design-Build Contractor shall comply with the requirements to prevent false fire alarms as provided in Attachment F. Design-Build Contractor shall provide a fire watch when impairment of the fire alarm system exceeds 4 hours in a 24 hour period.
Sprinkler systems will not be shut down except for portions of the sprinkler system under renovation, modification or construction, or for new connections to the sprinkler system. Sprinkler systems will not be shut down to avoid accidental discharge of the sprinkler system caused by unintentional damage to the sprinkler system from construction activity. Provide metal head guards at each sprinkler head within the limits of work.
20.0 MATERIAL & WORKMANSHIP QUALITY:
All materials and workmanship shall comply with all codes, standards, and recommendations of all Authorities Having Jurisdiction (AHJ). All work shall be done in a first class manner according to the best trade practices and to the satisfaction of the COR.
21.0 SECURITY:
All contractor personnel shall obtain a short-term identification badge issued by the COR. Such badge shall be worn by the individual and prominently displayed at all times while on VA property. No employee of the contractor shall enter the project site without a valid identification badge issued by the VA. In order to obtain a short-term identification badge, contractor personnel shall present to the Project Manager a valid (non-expired) photo identification issued by a US federal, state or local government agency.
All contractor personnel are subject to inspection of personal effects when entering or leaving the project site.
22.0 UTILITY SERVICES
Utilities Services: Maintain existing utility services for Medical Center at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uninterrupted services. Where necessary to cut existing water, steam, gases, sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (including telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by the COR.
| 1. | No utility service such as water, gas, steam, sewers or electricity, or fire protection systems and communications systems may be interrupted without prior approval of the Medical Center. Electrical work shall be accomplished with all affected circuits or equipment de-energized. When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without the Medical Center Director’s prior knowledge and written approval. |
| 2. | Contractor shall submit a request to interrupt any such services to the Medical Center COR, in writing, a minimum of 15 calendar days in advance of proposed interruption. A minimum of 15 calendar days is required by Medical Center staff to assess interruption impacts and to prepare mitigation measures; therefore, the requirement to make utility interruption requests a minimum of 15 calendar days in advance of the planned interruption will not be waived. Request shall state specific details of the interruption to include identification of the utility system and device to be interrupted, its location, reason, date, exact time of, and approximate duration of such interruption. |
| 3. | Contractor will be advised (in writing) of approval of request, or of which other date and/or time that such interruption will cause least inconvenience to activities of Medical Center. Interruption time approved by Medical Center will occur at other than Contractor's normal working hours unless the VA determines that the interruption is of a minor nature and will not cause inconvenience to hospital activities. |
| 4. | If the Medical Center determines that the requested interruption is of a minor nature and will not cause inconvenience to hospital activities, the Medical Center may allow the interruption to proceed in less than 15 calendar days after receipt of the contractor’s request for interruption. |
| 5. | Contractor shall not manipulate any utility device that will interrupt any utility service. Medical Center staff will operate utility devices to initiate and to terminate an approved utility service interruption. Contractor shall comply with lock out/tag out procedures. |
| 6. | In case of a contract construction emergency, service will be interrupted on approval of Project Manager. Such approval will be confirmed in writing as soon as practical. |
