Line Item 1 DIVALPROEX 500MG 14HR (ER) SA Tablets, 500 PER BT, QTY 888 NDC: 65162-0757-50, 00378-0473-05 Vendors are encouraged to submit alternate NDC’s for approval/technical acceptability on or before the quotation due date and time.

SOLICITATION:

All responsible SDVOSB open market sources may submit a quotation, which if timely received by offer due date/time specified on Standard Form (SF) 1449 of solicitation, shall be considered by this agency. Solicitation 36C77019Q0128 will be posted at www.fbo.gov approximately November 26, 2018 or shortly thereafter. Please direct any technical questions in writing to Karen R. Griffin Contracting Officer, at email preferred: karen.griffin4@va.gov.

No solicitation packages will be e-mailed, mailed, or faxed to prospective vendors. Responses must be concise and be specifically directed to the requirement referenced above and as detailed on the solicitation. It is the vendor's responsibility to monitor www.fbo.gov for this solicitation and amendments, if applicable.

Vendors shall be registered and active at the System for Award Management (SAM) at https://www.sam.gov at time of offer. SDVOSB shall be verified and active at https://www.vip.vetbiz.gov/ at time of offer. Offerors shall supply their state wholesale distributor licensure with offer verifying compliance with the Drug Supply Chain Security Act (DSCSA) with their quote. Vendors that fail to submit a copy of their state license shall be deemed technically unacceptable.

OFFERORS SHOULD BID ON ITEMS THAT CAN BE PROVIDED BY AGREED DELIVERY TIMEFRAME, AND/ OR IS IN STOCK FOR IMMEDIATE DELIVERY, FOR THE ITEM QUOTED. PLEASE NOTE: FACILITY STOCK IS LOW, AND DEMAND IS HIGH.
