1. Contracting Activity:
Department of Veterans Affairs Network 15 Contracting Office (NCO) Consolidated Mail Outpatient Pharmacy (CMOP) Leavenworth, KS the, 3450 S. 4th St Trafficway, Leavenworth, KS 66048.
Items to be delivered to:
VA CMOP Chelmsford - 761 10 Industrial Ave Chelmsford, MA 01824-3610.
2. Nature and/or Description of the Action Being Processed: This is a Justification and Approval request for the use of other than full and open competition. The requirement is for stock replenishment of medical supplies prescribed for patients and filled by CMOP at a firm fixed price. The purpose of this J&A request is to provide for the continued uninterrupted supply of direct patient care items required by the Facility.

5. Demonstration that the Contractor’s Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority):
The site is not requesting purchase from any certain vendor. Multiple vendors offer these items. These products have been prescribed for patients by VA Physicians. Pharmacy employees do not have the training and/or authority to countermand physician orders. “Brand Specific” products are the same thing as “dispense as written” on a physician prescription. No substitutions are permitted. In the VA, Pharmacy Benefits Management (PBM) has determined that CMOP IDs will most always identify “brand specific” products in the VA PRINT NAME.
6. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable:

7. Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable:
The IGCE contains the estimated cost to the government and serves as the recognition of F&R.
8. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted:
Market research was done to determine there were no contract sources for procurement.
