The purpose of this amendment is to respond to the questions asked related to this solicitation on pages 2-4 of this amendment.
Amendment also includes correction's made to CLIN's 3002,4002 and 4003 description. Additional information to section 2.1 of SOW. Provision FAR 52.212-1 and 52.212-2 to include requirement of Bee Abatement Program to be provided.
Basic Hospital map view is also provided as an attachment on this amendment.

Q: Has any of the SOW changed from this Solicitation compared to that of the last awarded contract? If so, what changed or was added?
A: Including Federal Holidays, Contractor service turn-around time shall be no later than 12-18 hours after receiving notice for emergent situations.
Q: Has the government issued any CURE or Notice of Correction for pest control services over the past 24 months at this location?
A: No, the government has not issued any CURE or Notice of Corrections for pest control services over the past 24 months at this location.
Q: Can the government confirm the number of exterior rodent bait stations and interior rodent traps that are present? Will the government require these to be provided by the contractor upon award.
A: The Government cannot confirm the number of rodent bait stations or traps. It will be required by the contractor upon award to provide.
Q: Can the government clarify how many fly lights devices are currently present at the
Southern Arizona VA Healthcare System?
A: The Government maintains fly light devices for the facility.
Q: What is the amount of hours each month the previous contractor spent on-site at
Southern Arizona VA Healthcare System to provide the pest services to all areas outlined within the scope of service?
A: The Government estimates approximately 12-15 hours a month spent on site for pest services.
Q: Can the government clarify (Page 31) "14.6. Inspection and treatment of the canteen storage, offices, store, and cafeteria areas may only be serviced between the hours 3:00 p.m.
- 3:30 p.m." due to the 30 minute timeframe for treatment being restrictive and could affect quality of treatment. Can flexibility be made for services.
A: Yes, there can be flexibility but cannot be before 3pm.
Q: On the CLIN it has Bed Bug assessment and treatment with one price. The assessment and the treatment are two totally different prices. Will there be treatment for bed bug whether it needs it or not?
A: No, treatment for bed bugs will not be needed if determined no presence of bed bugs.
Q: Would pigeon include exclusion or fixing any of the exclusion work that is harboring the pigeons currently (i ask this because you cannot get to some of the issues to trap them due to the failed exclusion work that is in the way).
A: Pigeons outdoors are the responsibility of The Federal Government, pigeons indoors are the responsibility of the contractor
Q: Any 3rd party audits that need specific reports or placement of rodent/flying insect/monitoring stations (common with our other medical accounts).
A: There are no 3rd party audits.
Q: They need to update the SOW to show interior services only. If its interior service only they need to make clear that we are not responsible for exterior issues, other then the Bees, Wasps Bats.
A: Additional language will be added to SOW on section 2.1.
Q: Bats can not be done on a monthly bases because they are federally protected only a few months a year.
A: Bats outdoors are the responsibility of The Federal Government, bats in the interior is the responsibility of the contractor.
Q: How many interior rodent/flying insect/monitoring stations are currently there and how many do they want per the SOW (they had no idea and said to email them).
A: This is an integrated pest management program and provided as needed.
Q: Will the quantity of work be more than the quantity given on the price sheet? Example:
Bed Bug services are on-call for 48 times a year. What happens when it hits 49
A: Quantity of work should not exceed.
Q: What is the range of square footage of the rooms where bed bug treatments will be performed?
A: Ranges from waiting room to patient room. Exact square footage not available at this time.
Q: How many days a week have you called for pest control services last year?
A: Approximately 3 times a month, along with routine weekly service visits.
Q: I would love to see last year's contract if possible
A: This is public information in which a contractor may request a FOIA.
Q: Are there any reasons you expect more or less work this coming year than last year?
A: We do not expect any more or less work this coming year.
Q: How much pigeon and bat work do you expect to have?
A: Pigeons and bats indoors will be the responsibility of the contractor. There have been zero indoor pigeon calls and 3 indoor bat call in the last year.
Q: Who was the last incumbent and what was the last contracted amount for this contract?
A: University Termite and Pest Control $218,700 for the contract.
Q: I have a question regarding CLIN's numbers 3002 and 4003, are they supposed to be showing 48 months or 12 months like the others? I just wanted to make sure prior to submitting my proposal.
A: 12 months is for termite and pest control services, 48 JB is per treatment.
NOTE: Any questions not answered is due to it not having any relevancy to the current solicitation and or is public information in which a contractor may request a FOIA.

E.1 ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—
COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
INSTRUCTIONS TO OFFERORS:
Offerors should review this entire solicitation before submitting an offer to ensure that their offers meet all requirements.
Prospective Offerors are notified that vendors require to be registered with the VAs financial system. Offerors will need to do so using the 10091 webform located in the Customer Engagement Portal (CEP) located at: https://www.cep.fsc.va.gov as the paper/pdf version of the form will no longer be accepted. This registration is required to be completed at the time of submitting an offer. Failure to include a confirmation of this registration may preclude the offeror from consideration for award or considerably delay award.
Prospective Offerors are notified 852.219-75 VA Notice of Limitations on Subcontracting Certificate of Compliance for Services and Construction, paragraph (d) requires certification to be completed, signed, and returned with the offeror’s submission. Failure to include this completed information will preclude the offeror from consideration for award.
System for Award Management: IAW FAR 52.212-4 (t) Contractors submitting offers are required to be registered in the SAM database to be considered for award of a contract.
Amendments to this Solicitation: Contractors are encouraged to monitor the Contract Opportunities website at SAM.gov | Contract Opportunities with respect to this solicitation because any amendments to this Solicitation will be posted on that website.
Submittal Instructions: The offer must be labeled with the Contractor’s name and business address of firm, the solicitation number, and point of contact to include name, telephone and email address. The Government is not responsible for identifying inconsistencies between the two and may rely on either version at its discretion.
The Contractor is advised that all submissions and related materials become the property of the U.S. Government and will not be returned. A Contractor’s offer, if accepted by the Government, will form binding parts of the contract that results from this solicitation.
Therefore, care must be taken to properly address the requirements set forth in this RFQ.
Each Offeror shall submit only one offer which addresses all Government requirements outlined in this solicitation. Offerors shall submit their offers with a cover letter, by date and time noted on the first page of the SF1449 in block 8. The contractor shall submit their offer electronically to Mirna.GastelumRomero@va.gov.

Questions:
All questions must be submitted in writing and emailed to. Questions must be received no later than, 02:00pm PST Time 3 calendar days prior to the closing of the solicitation. The https://www.cep.fsc.va.gov/ mailto:Mirna.GastelumRomero@va.gov
Government reserves the right to not answer questions not meeting this timeline. All responses to questions will be incorporated into a written amendment.
Offer Due Date:
The Offeror shall submit completed offer electronically to Mirna.GastelumRomero@va.gov by the time and date specified on Block 8 of the SF-1449.
Contractors are responsible for ensuring that the Contract Specialist has received the offer before the response deadline. The Government will not be responsible for delays or failures of either the Contractor’s e-mail system or the Government’s e-mail system.
Contractors may be asked to clarify certain aspects of their offer. Communication conducted to resolve minor or clerical errors will not be constituted as discussions. Although the Government intends to make award without discussions with respective vendors, the Government reserves the right to conduct discussions if deemed in its best interest.
Each Offeror shall submit only one offer which addresses all Government requirements outlined in this solicitation. Offerors shall submit their offers with a cover letter, by date and time noted on the first page of the SF1449 in block 8. The contractor shall submit their offer electronically to Mirna.GastelumRomero@va.gov.
Content Organization:
Each offer submitted shall include two separate files with the content as described below for File 1 & 2 n in-order to be considered for award. Deviating from this requirement may exclude your offer from award.
File #1: General Information and Price/Cost Schedule:
The offeror shall submit their pricing within the schedule, Section B, of the SF 1449. The offeror shall properly complete the solicitation document (SF 1449) along with any amendments (SF 30), if any are issued. Complete Blocks 12, 17a (please include SAM UEI number), 30a, 30b, and 30c of the Standard Form 1449. A preliminary review of offers will be conducted if all factors are not addressed the offer will not be forwarded for evaluation this will preclude the offer from any further consideration for award.
File #2: Special Standards of Responsibility:
Contractor shall submit valid proof the Contractor and/or personnel performing on the contract have the following training, certifications, and licenses:
• Manager and alternate shall be EPA certified or a graduate of a Pest Management training program.
• Manager and alternate shall have one-year of specialized experience.
• Trained and qualified IAW VHA Directive 1850.02. Pest Management Operations.
• Compliance with Federal, State, and Local regulations as identified in Public Law
(Pub. L.) 92-516, Federal Insecticide, Fungicide and Rodenticide Act, dated
November 28, 1975, as amended, and Executive Order 12088 (Federal Compliance with Pollution Control Standards) and appropriate OSHA standards regarding hazard communication, personal protective equipment, and exposure standards.
• Bee Abatement Program.
Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:

E.3 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND
COMMERCIAL SERVICES (NOV 2021)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Price.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the
Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

Basis For Award
This procurement is being conducted pursuant to FAR Part 13 procedures. The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, offers the lowest price, whose offered price does not exceed the amount of funding available for the procurement, and whose offered price is found to be reasonable. Price alone is the sole evaluation factor. In addition to the general standards of responsibility found at FAR 9.104-1, in accordance with FAR 9.104-2, the special standards of responsibility described below apply to this procurement. Offerors who fail to meet any special standard of responsibility will not be eligible to receive contract award.
Special Standards of Responsibility -The following special standards of responsibility apply to this procurement: The contractor shall be required to provide fully qualified, trained or certified service, delivery, and management personnel in sufficient numbers to actively and efficiently service and support the SAVAHCS to provide Termite, Pest, Insect and Rodent Control services during the contract period. The contractor shall submit valid proof of required qualifications in a capability statement to include any supporting certification.
• Manager and alternate shall be EPA certified or a graduate of a Pest Management training program.
• Manager and alternate shall have one-year of specialized experience.
• Trained and qualified IAW VHA Directive 1850.02. Pest Management Operations.
• Compliance with Federal, State, and Local regulations as identified in Public Law (Pub.
L.) 92- 516, Federal Insecticide, Fungicide and Rodenticide Act, dated November 28, 1975, as amended, and Executive Order 12088 (Federal Compliance with Pollution Control Standards) and appropriate OSHA standards regarding hazard communication, personal protective equipment, and exposure standards.

Failure to include this completed information will preclude the offeror from consideration for award.
Cost/Price - The Government will evaluate offers for award purposes on the total cost to determine that cost is fair and reasonableness. The Government anticipates adequate price competition and intends to award without discussions with respective offerors. However, the Government reserves the right to conduct discussions if deemed in its best interest.
Evaluation Process - Offers will be evaluated in the following manner. The lowest-priced offer will be identified. The lowest-priced offer will be evaluated to determine if the offered price exceeds the amount of funding available for the procurement. If the offered price does not exceed the amount of funding available for the procurement, the offered price will be evaluated to determine if it is reasonable. If the offered price is found to be reasonable, the offeror who submitted this offer will be identified as the “successful offeror.” The contracting officer will then determine if the successful offeror is responsible using the general standards of responsibility and the special standards of responsibility applicable to this procurement. If contract award cannot be made to the successful offeror, the contracting officer will evaluate the next lowest-priced offer following the procedures described above. This process will continue until a contract award can be made, or the contracting officer determines that no contract award can be made.
The contracting officer may find all offers not acceptable and cancel the solicitation if the lowest-priced offer exceeds the amount of funding available for the procurement. The contracting officer may likewise find an offer under evaluation, as well as all remaining offers not acceptable and cancel the solicitation if the offer currently under evaluation exceeds the amount of funding available for the procurement. A preliminary review of offers will be conducted if all factors are not addressed the offer will not be forwarded for evaluation this will preclude the offer from any further consideration for award.
In addition to submitting their pricing, offerors must also establish their qualifications and demonstrate their ability to perform all services pertaining to this solicitation.
The Government anticipates adequate price competition and intends to award without discussions with respective offerors. However, the Government reserves the right to conduct discussions if deemed in its best interest.
