C.2 SUPPLEMENTAL INSURANCE REQUIREMENTS

In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:

(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

(b) General Liability: $500,000.00 per occurrences.

(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.

(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR

2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

D.1 STATEMENT OF WORK

1. BACKGROUND:

1.1. The VA Long Beach Healthcare System (LBVAHS) requires a professional and continuous evaluation and assessment program that focuses on the evolving needs of current and future Veterans.

1.2. The VA Long Beach Master Planning and Consulting Services accomplishes this by creating a direction for LBVAHCS and allowing the hospital to maximize options for influencing the future environment.

1.3. The Master Plan involves both new construction and renovation projects of existing buildings and accounts for the temporary displacement of Services while maintaining their capability to provide for Veterans.

1.4. These modifications will leverage new technology, implement new operational flow and healthcare delivery, and upgrade infrastructure to ensure the facility is able to deliver modern care.

1.5. To better adapt to future operational challenges, including the growing and changing Veteran demand for services, the VA will modernize its practices to become a more efficient and agile provider of benefits and care.

1.6. The facilities and the care provided therein must be modernized and updated to not only serve the Veterans of today, but to also serve the Veterans of tomorrow.

1.7. Due to the multi-disciplinary nature of the components of the Master Plan, the dedicated team that develops it must have subject matter expertise in multiple areas including, but not limited to architecture, modern clinical operations, environmental engineering, value engineering, cost estimation, risk analysis, strategic planning, quality assurance/control, construction administration, medical, and healthcare administration.

2. GENERAL REQUIREMENTS:

2.1. Contractor to provide professional strategic planning consultation services, to include current state analysis of the facilities on Long Beach VA, annual review and update of the Long Beach VA Master Plan, annual deep dives into various construction, maintenance, and repair projects, periodic review of the use of space by various departments on Long Beach VA, and support in development space planning recommendations to accommodate new staff, equipment and/or increased patient demand for services.

2.2. The projects may be single or multi-disciplinary including, but not limited to, performance or various studies, archeological, environmental, traffic, architectural, space assessments, flow planning, schematic concepts, and cost estimating.

2.3. The consulting agency shall have experience with commercial healthcare master planning for hospital systems, preferably with the Veteran’s Health Administration

(VHA).

2.4. The consulting agency shall provide, licensed and registered professionals in the following disciplines: Architecture; Structural; Civil; Electrical; Environmental; Value Engineering; Cost Estimating; Risk Analysis; LEED or Green Globe Certification;

Strategic Planning and Consulting; Quality Assurance and Quality Control; Drafting; and Construction Administration.

2.5. To achieve these objectives, the contractor shall provide professional staffing services to support the Engineering Department of Long Beach VA.

2.6. The professional staff contractors shall work with the Planning Section, reporting to the Chief of Planning in support of medical center master planning.

2.7. The professional staff contractors shall be responsible for independently planning and carrying out the assigned tasks to meet the objectives of the service.

2.8. The contractor shall take into consideration the work environment such as the space, people, function, and design.

2.9. Contractor shall provide all personnel, training, protective gear, and supervision necessary.

2.10. This agreement will consist of a one-year base period and four option years to be exercised at the Government’s discretion.

2.11. The consultation services will be required primarily in Long Beach, on an as-needed basis, for specific projects or requirements in the Long Beach VAHS, which encompasses the Tibor Rubin VA Medical Center in Long Beach, and six Community- Based Outpatient Clinics (CBOC’s) in Anaheim, Laguna Hills, Santa Ana, Santa Fe Springs, Gardena, and Villages at Cabrillo in Long Beach.

2.12. Contractor shall notify and coordinate all work activities pertaining to this contract with the assigned COR.

3. SPECIFIC TASKS:

3.1. Contractor shall provide all personnel, resources, credentials, and expertise required to complete ALL subsequent tasks within the specified timelines outlined in section 9 Qualification Requirements.

4. TASK 1 – CONDUCT CURRENT STATE ANALYSIS (ONE TIME TASK):

4.1. Assess current state of hospital support, laydown, and space. Collaborate with the Long

Beach VA Planning Team to maintain awareness of Long Beach VA space management issues.

4.2. The contractor will identify deficiencies and inefficiencies across the spectrum of patient care as it relates to facility planning.

4.3. Items to look at should include, but are not limited to, patient flow, logistics flow, maintenance areas, accessibility, front and back of the house laydown, security, parking, and efficiencies of key spaces such as exam rooms, labs, and other clinical spaces.

4.4. This will involve project mobilization, weekly space management meetings, leading and participating in working groups with hospital services, and performing database research.

4.5. At the conclusion of this task, the contractor will compile a formal report documenting the current state of the hospital and highlighting the opportunities for improvement regarding construction or space moves.

4.6. Contractor will assist Long Beach VA in determining what the ideal space for different hospital services should be.

4.7. The assessment should look to see if the service has too much or too little space, if the location best serves patient and/or hospital needs, and how the existing space can be optimized.

4.8. The ideal space needs should be determined looking out 10 years and will be based on the VA’s Space Equipment Planning System (SEPS), as well as analysis of both feedback from the hospital services and requirements of industry and how they allocate space.

4.9. Contractor shall assess Long Beach VAMC’s implementation of Veterans Integrated Service network (VISN) and VA Central Office (VACO) priorities and work with Long Beach VA planning team to assess how well Long Beach VA is adhering to VISN and VACO priorities.

4.10. Contractor will look at the current VISN and VACO visions and priorities, determine which ones apply to Long Beach VA, and assist Long Beach VA in translating those visions and priorities into tangible metrics. This task should also consider recommendations and findings from official reports, like the Asset and Infrastructure Review Commission (AIR) and Market Area Health System Optimization (MAHSO).

4.11. The current state of Long Beach VA will be measured against those tangible metrics and the contractor will produce a report of how well to which they were adhered, using a visual reference that is easy to read and understand, with a green, yellow, and red color-coded system.

4.12. The task will conclude with a formal report documenting what Long Beach VAHS priorities should be over the next 10 years.

5. TASK 2 – REVIEW AND UPDATE Long Beach VA MASTER PLAN (REOCCURRING

TASK):

5.1. Contractor shall provide ALL resources and technical expertise to review and update the existing Long Beach VA Master Plan every year.

5.2. Areas of review should include the facility’s use of space, compliance with new VA and

Federal requirements, comparison of past projections to current state, and analysis of future projections and their veracity. This should also include data analytics support for key metrics related to space planning and compliance.

5.3. Contractor will access current Facility Condition Assessment (FCA) data from the Capital Asset Inventory (CAI) database and associate each potential Rec # to ALL associated future recommended projects, for the next Strategic Capital Investment Planning (SCIP) cycle and the following five (10) years.

5.4. All potential FCA’s and future project recommendation will be reviewed and approved by the COR and the Technical Evaluation Team (TET) prior to implementing into the final draft of the Master Plan.

5.5. Contractor shall coordinate with Long Beach VA stakeholders to ensure that the future projections and associated projects are still valid and accurate.

5.6. Activities will include participating in an initial working group to review new requirements (from previous year), working groups with hospital services to determine their assessment of the new requirements, finalizing the plan of action for the Master Plan, translating space requirements into project plans, and determining yearly space moves out to 10 years based on updates.

5.7. The contractor will update the Long Beach VA Master Plan based on the outputs of ALL previous tasks.

5.8. This will involve preparing a summary of changes to be made to the Master Plan for review, assisting in decision briefings to leadership, updating the 10-year Long Beach VA Master Plan brief, updating the timeline for the Long Beach VA Master Plan with the major projects over the next 10-years, and updating the Long Beach VA Master Plan floor area diagrams (as needed) for major moves - out to 10 years.

5.9. The Master Plan diagrams will require a stack plan for every building with different services and room functions identified on the plan.

5.10. The 10-year update should include a visualization of space moves, renovations, and construction projects across timelines and show the interdependencies between different actions (i.e., we should be able to see the second and third order impacts and when projects overrun or when services are displaced).
