1. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
Contractor to provide all labor, supervision, materials, supplies, tools, equipment, and transportation to remove four (4) fallen trees, grind each stump sufficiently below ground level, then backfill all remaining holes using stump chips created during the disposal process. Contractor shall prevent any damage to surrounding buildings, landscape, handrails, and irrigation systems. Estimated amount: $17,605.00
2. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
FAR 6.302-2, Unusual and Compelling Urgency: A tree toppled over and blocked Benton Road entering the facility. The tree needed to be removed immediately before it threatened hospital services and created safety concerns to patients and employees.
3. Description of market research conducted and results or statement why it was not conducted:
Customer provide Suggested Source information. CO validated that Suggested Source met all government requirements prior to award.
4. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
