Statement of Work VA Loma Linda Healthcare System (VALLHCS) requires a purchase for a Zeiss FORUM patient data management software.

DELIVER REQUIREMENTS: Core Biologics shall provide one delivery method. The primary method will be FOB destination.

PRODUCTS: Zeiss FORUM patient data management software to be used in the new eye clinic.

PERIOD OF PERFORMACENCE: 12 months from the date of award.

INVENTORY: Not applicable.

INITIAL ILNVENTORY: A total of one Zeiss FORUM system for initial inventory. VALLHCS will provide adequate space for the item.

LIABILITY: The Government assumes no liability for lost or damage to these products until signed for and received.

REPLACEMENT ITEMS: Not applicable.

INVENTORY MAINTENANCE: Not applicable.

EXPIRING INVENTORY: Not applicable.

DEFECTIVE ITEMS: Defective products will be replaced by Contractor at no charge to the Government, unless the Government has misused or lost the item.

INVENTORY: Contractor’s personnel will meet with staff, after installation, to ensure they are comfortable with the use of the product.

HOURS OF OPERATION: The contractor shall not be required, except in case of emergency, to furnish the product during non-standard workday hours as listed by each individual facility.

Standard work days are described as Monday through Friday. Any service performed during other than normal work days must be approved in advance by the COR.

When a National Holiday falls on a Sunday, the following Monday shall be observed as a National Holiday. When a National Holiday falls on a Saturday, the proceeding Friday is observed as a National Holiday by the Federal Government. Also included, would be any other day, specifically declared by the President of the United States of America to be a national Holiday.

PAYMENT:

Payment for the product will be made by Purchase Order.

VA BADGES

All Contractor personnel are required to obtain a Contractor Identification Badge from the VA Police Station. The badge must be worn during the entire time he/she is on the Department of Veterans Affairs Grounds.
