6. REQUIRED REGISTRATION WITH SAM:

Federal Acquisition Regulations require all contractors conducting business with the

Government to be registered in two separate and individual online databases: 1) the System for

B.2 PRICE/COST SCHEDULE

It is the intent of the Department of Veterans Affairs to award a Fixed-Price Contract to provide Main Steam

Isolation Valve Maintenance Services, Quadax Butterfly Valves for the Department of Veterans Affairs

Long Beach Healthcare System (VALBHS). The Contractor shall provide all labor, travel, parts, supervision, materials, equipment, tools, supplies, control and disposal of hazardous/nonhazardous materials, testing, abatement, licensing, surveys, documentation, and permits when required to perform all services required by the statement of work for Main Steam Isolation Valve Maintenance Services, Quadax

Butterfly Valves at VA Long Beach Healthcare System (VALBHS) 5901 E. 7th Street, Long Beach, CA

90822.

Option period pricing shall apply to any extension pursuant to FAR 52.217-8

B.3 STATEMENT OF WORK /PERFORMANCE REQUIREMENTS:

Upon award the Contractor shall meet with the Contracting Officer Representative (COR) to establish the priority work, a timeline and a schedule during off hours and on weekends to provide the required services. The Contractor shall verify and check the new piping for leaks and proper operation. Services under this Firm Fixed priced contract shall start upon award.

QUADAX BUTTERFLY VALVES

Location: Building 128 Mechanical Room C104.

1. Remove existing 4” steam By-Pass Manifold and weld both ends with caps.

2. Relocate Pressure Relief Valve on main supply line, Relief Valve will be a Kunkle Model Type

6252AKH01-AS (relief capacity of 3,809 lbs. hr., set at 25 psi., 2” FMTH x 3” FMTH), and new

Kunkle Drip Pan Elbow model 0299-K, with ¾” bottom drain line, and reconnect & re-route to existing 4” steel vent pipe.

3. Redo Top steam line ¾” Drip Leg line (Low pressure side) to condensate return line next to wall, with new ball valves strainer, unions, check valve, steam trap and proper supports.

4. Redo Bottom condensate ¾” line that connects to the High-pressure side drip Leg, to include proper supports.

5. Redo the 1” copper Hot Water Return line from entrance wall to the start top side of ceiling/2nd floor, with K-type Copper Pipe and proper hangers.

6. Remove All old piping Insulation on Steam Piping, Condensate Return & Discharge Piping, Hot

Water Return Piping, HVAC Heating Supply & Return Piping, two HVAC Heating Heat

Exchangers, Flash Tank, Steam Vent Piping, install insulation blankets to Condensate Pump and to Relief Valve Drip Elbow. All Insulation to be Fiberglass with White PVC Protected Insulation

Covering on all elbows, thickens to be minimum of 1” for ½”, ¾”, minimum of 11/2” for 1”, minimum of 2” for 11/2”, and a minimum of 3” for anything above 2” piping or as recommended by the Building Energy Efficiency Standards.

Location: Building 126OP Mechanical Room 140P

Work with Pipe Shop supervisor to do Lock-out/Tag-out on all related steam system in the building.

1. Remove two existing 1” steam By-Pass Manifold and plug both ends.

2. Replace existing Pressure Relief Valve on main low-pressure supply line, Relief Valve will be a

Kunkle Model Type 6252FKH22-AS (RELIEF CAPACITY OF 4,735 lbs. hr., set at 40 psi., 2”

300 flange x 3” 150 flange).

3. Install two Unistrut Hanger Supports and two Ceiling Hanger Support, to existing ½” steam supply line to operate liquid mover.

4. Install new ½” Steam Drip Leg to existing steam supply to liquid mover, with two ½” ball valves, strainer, check valve, two unions, one steam trap and connect to condensate return pipe next to liquid mover.

7. Remove all old Piping Insulation on Steam PRV Manifold Station, from wall entrance to the bottom corner of steam header (to include two end insulation caps on header) and to the ½”

Steam Supply Line from PRV Station to the Liquid Mover. All Insulation to be Fiberglass with

White PVC Protected Insulation Covering on all elbows, thickens to be minimum of 1” for ½”, ¾”, minimum of 11/2” for 1”, minimum of 2” for 11/2”, and a minimum of 3” for anything above

2” piping or as recommended by the Building Energy Efficiency Standards.

Specific Tasks.

Notify Pipe Shop Supervisor for job related issues, exact location/equipment and unlock security doors.

MATERIALS:

The Contractor shall provide new, brand name or 100% equal in all specifications The Contractor shall have the COR approve all materials prior to instillation:

ADDITIONAL REQUIREMENTS:

a. Supervisor: Shall have 30 Hour OSHA Construction Safety Training.

b. Personnel: Shall have 10 Hour OSHA Construction Safety Training.

c. The Contractor shall provide the latest results of TB Test within 10 days of award.

d. The Contractor shall submit an “Infection Prevention Construction Risk Assessment”

(ICRA) permits.

e. The Contractor shall be responsible for all measurements.

a) REMOVAL OF MATERIAL AND EQUIPMENT:

All Contractor material and equipment shall be removed from the facility or stored and secured properly at the end of the workday. Remove all empty containers, boxers, etc. from Government property.

b) SAFE PREPARATION AND USE OF MATERIALS AND EQUIPMENT:

Materials shall be prepared according to manufacturer’s recommendations. Contractor shall use equipment for the purpose it was intended for and in accordance with all OSHA regulations and safety standards. Contractor shall furnish Material Safety Data Sheets (MSDS) on all chemicals to the COR when requested.

c) WARNING SIGNS AND PROTECTIVE BARRIERS:

Contractor shall place signs designating work areas to warn others of work in progress, as well as to protect persons, property, equipment, etc.

d) Contractor shall coordinate with the COR prior to performing work to prevent conflicts with the treatment of patients and the functioning of the facility. If needed, no medical apparatus shall be moved without approval.

e) A proposed work schedule shall be submitted to the COR for coordination within twenty-four (24) hours after notice of contract award. In addition, the Contractor shall submit a daily schedule to the representative of the areas to be serviced in advance for each workday.

f) The Contractor is responsible for complying with all appropriate regulations and guidelines of the

JCAHO, OSHA, and any other pertinent federal, state, and local references.

g) All tasks accomplished by the Contractor personnel shall be performed to preclude damage or defacement of Government-owned furnishings, fixture, equipment and facilities. The Contractor shall report any damage observed or caused by the Contractor personnel to the COR.

h) The Contractor shall be responsible for repair or replacement of government-owned items or facilities to previous condition, if determined to be at fault by the Contracting Officer.

SUPPLIES, EQUIPMENT AND TOOLS:

The Contractor shall furnish all materials, equipment, and supplies necessary to perform the services of this contract. All cleaning materials and supplies shall be provided in their original containers with Material Safety Data Sheets (MSDS) immediately accessible to the user(s).

a. The Contractor shall provide environmentally preferable products (soap, chemical, etc.) and is required to provide a MSDS for all potentially hazardous agents used within the facility to ensure the safety of the occupants and employees.

b. The equipment shall have bumpers and guards to prevent marking, scratching or marring of furniture, fixtures or building surfaces.

c. All Contractor electrical equipment shall be Underwriter’s Laboratory (UL) listed and maintained in a safe condition for the operator and bystanders. All equipment must operate using existing building circuits. Contractor shall not operate electrical equipment or combinations of electrical equipment, which require power exceeding the capacity of the site.

B.4 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND

COMPLIANCE (JUN 2011)

This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned

Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause.

To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of

VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within thirty (30) days.

C.3 52.219-27 NOTICE OF SERVICE-DISABLED VETERAN-OWNED SMALL

BUSINESS SET-ASIDE (NOV 2011)

(a) Definition. "Service-disabled veteran-owned small business concern"—

(1) Means a small business concern—

(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and

(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.

(2) "Service-disabled veteran" means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).

(b) Applicability. This clause applies only to—

(1) Contracts that have been set aside or reserved for service-disabled veteran-owned small business concerns;

(2) Part or parts of a multiple-award contract that have been set aside for service-disabled veteran-owned small business concerns; and

(3) Orders set aside for service-disabled veteran-owned small business concerns under multiple-award contracts as described in 8.405-5 and 16.505(b)(2)(i)(F).

(c) General.

(1) Offers are solicited only from service-disabled veteran-owned small business concerns. Offers received from concerns that are not service-disabled veteran-owned small business concerns shall not be considered.

(2) Any award resulting from this solicitation will be made to a service-disabled veteran-owned small business concern.

(d) Agreement. A service-disabled veteran-owned small business concern agrees that in the performance of the contract, in the case of a contract for—

(1) Services (except construction), at least 50 percent of the cost of personnel for contract performance will be spent for employees of the concern or employees of other service-disabled veteran-owned small business concerns;

(2) Supplies (other than acquisition from a nonmanufacturer of the supplies), at least 50 percent of the cost of manufacturing, excluding the cost of materials, will be performed by the concern or other service-disabled veteran-owned small business concerns;

(3) General construction, at least 15 percent of the cost of the contract performance incurred for personnel will be spent on the concern's employees or the employees of other service-disabled veteran-owned small business concerns; or

(4) Construction by special trade contractors, at least 25 percent of the cost of the contract performance incurred for personnel will be spent on the concern's employees or the employees of other service-disabled veteran-owned small business concerns.

(e) A joint venture may be considered a service-disabled veteran owned small business concern if—

(1) At least one member of the joint venture is a service-disabled veteran-owned small business concern, and makes the following representations: That it is a service-disabled veteran-owned small business concern, and that it is a small business concern under the North American Industry Classification Systems

(NAICS) code assigned to the procurement;

(2) Each other concern is small under the size standard corresponding to the NAICS code assigned to the procurement; and

(3) The joint venture meets the requirements of paragraph 7 of the explanation of Affiliates in 19.101 of the Federal Acquisition Regulation.

(4) The joint venture meets the requirements of 13 CFR 125.15(b)

(f) Any service-disabled veteran-owned small business concern (nonmanufacturer) must meet the requirements in 19.102(f) of the Federal Acquisition Regulation to receive a benefit under this program.

C.4 52.232-19 AVAILABILITY OF FUNDS FOR THE NEXT FISCAL YEAR

(APR 1984)

Funds are not presently available for performance under this contract beyond September 30. The

Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond

September 30, until funds are made available to the Contracting Officer for performance and until the

Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer.

(a) Definition. For the Department of Veterans Affairs, “Service-disabled veteran-owned small business concern or SDVSOB”:

C.9 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS

(NOV 2012)

(a) Definitions. As used in this clause—

(1) Contract financing payment has the meaning given in FAR 32.001.

(2) Designated agency office has the meaning given in 5 CFR 1315.2(m).

(3) Electronic form means an automated system transmitting information electronically according to the

Accepted electronic data transmission methods and formats identified in paragraph (c) of this clause.

Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests.

(4) Invoice payment has the meaning given in FAR 32.001.

(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.

(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.

(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:

(1) VA’s Electronic Invoice Presentment and Payment System. (See Web site at http://www.fsc.va.gov/einvoice.asp.)

(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the

Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI).

The X12 EDI Web site (http://www.x12.org) includes additional information on EDI 810 and 811 formats.

(d) Invoice requirements. Invoices shall comply with FAR 32.905.

(e) Exceptions. If, based on one of the circumstances below, the contracting officer directs that payment requests be made by mail, the contractor shall submit payment requests by mail through the United States

Postal Service to the designated agency office. Submission of payment requests by mail may be required for:

(1) Awards made to foreign vendors for work performed outside the United States;

(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;

(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;

(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA

Financial Services Center in Austin, Texas; or

(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.10 VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984)

The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of California.

(d) Comptroller General Examination of Record. E.1 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS (JAN

2017)

(a) North American Industry Classification System (NAICS) code and small business size standard. The

NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.

(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show—

(1) The solicitation number;

(2) The time specified in the solicitation for receipt of offers;

(3) The name, address, and telephone number of the offeror;

(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
