SOW – Ambulatory Exam Room Workstations 664-18-3-118-0090

Scope of Work

Ambulatory Exam Room Workstations 3350 La Jolla Village Drive

VASDHCS San Diego, California

1. General

Over the past five years the new Ambulatory Care Clinics have opened in several different phases of construction and all exam rooms were set-up using various types of workstations. With all the diverse types of workstations staff are having issues going back and forth on different provider work station set ups.

The intendent of this procurement is to replace all the workstations in the 5 Ambulatory clinic areas with a total of 51 set ups.

2. Substitution Requests

Substitution requests (“OR EQUALS”) shall meet salient characteristics as outlined. Substitution requests shall be similar to the specified dimensions, color scheme, function and quality. “Or equal” products submissions shall include manufacturer’s product brochure including specification information, warranty data, and physical samples of manufacturer’s full range of finish options. Potential offers must be aware that the items being acquired are procured as Brand Name or Equal. For those items that are “or equal”, a description of the salient characteristics outlined above. The FAR Clause 52.211-6 Brand Name or Equal applies to this acquisition. Any items quoted as equal not confirming to the requirements of clause 52.211-6 will be considered non- responsive and not considered for award.

3. Warranty Requirements

All specified product(s) shall be under standard GSA Contract warranties.

Open Market items and specified fabrics shall meet the standard manufactures stated warranty claims as published.

4. Vendor Furnished Items and Responsibilities

A.The Vendor shall furnish all supplies, equipment, facilities and services required to perform the service under this contract. The Vendor shall be responsible for any missing parts and components not included in order to carry out the installation. The Vendor shall perform all work in accordance with this Performance Work Statement to include the Furniture and all ancillary items necessary to outfit and install per the requirements as stated.
B.Final field verification shall be the responsibility of the VA.
C.The certification and accreditation (authorization) (C&A) requirements for IT Security do not apply to this procurement and a Security Accreditation Package is NOT required.

5. Delivery and Installation

A. Delivery coordination will be thru the VA Designer. Once the Contracting Official (CO) assigns a Contracting Officer’s Representative (COR) to the procurement, typically at the time of award; coordination will be thru the COR.

B. Installation Shall take place on a Weekend, Saturday/Sunday (7am-330p) if time does not allow to be done in two consecutive days it will need to be completed the following weekend.

C. All installers are to be PIV badged or be prepared with day visitor passes by time installation work is to begin. Be prepared with a legal form of ID to present to the police upon arrival to the facility.

D. Existing workstations in the clinic shall be removed by awarded vendor prior to installation of new workstations. Workstations now consist of top and four legs. The legs and tops will need to be broken down and placed back in VA storage in the basement.

E. Any damage or missing parts shall be pre-identified prior to install.

F. All debris shall be cleaned and disposed of within VA dumpsters.

6. Deliverables

a. Provide completed Confirmation of Scope of Work certification form.

b. Provide formal quote based on the SOW, drawings, and Salient Specifications.

7. After Award

a. It is the responsibility of the awarded vendor to confirm as-built dimensions to assure that desk/seating configurations shall fit per plan.

b. Prepare a formal CAD drawing with accurate furniture symbols determining which tables are Right sided and which tables are Left sided based on the configuration of the rooms.
