1. This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number 36C26218Q0066 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror’s responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017.
2. This is a competitive solicitation and is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). Offerors must be registered as SDVOSB with the VA’s Office of Small and Disadvantaged Business Utilization at https://www.vip.vetbiz.gov/ at the time of offer submission due date. The North American Industry Classification System (NAICS) code for this acquisition is 561720 – Janitorial Service and the small business size standard is $18 million. Offeror must be registered in the system Award Management (SAM) at https://www.sam.gov in order to be considered for award.

4. Statement of Work: The Department of Veterans Affairs Healthcare is seeking a qualified and licensed contractor to provide janitorial service at VA Loma Linda Healthcare System, March Air Base campus. See Attachment 1, Section B.4 for the Statement of Work (SOW).

B.2 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011)
This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

B.4 STATEMENT OF WORK
1. The VALLHS requires a Contractor to provide all labor, supervision, supplies, equipment, and transportation necessary to perform the work specified herein. Contractor is not responsible for cleaning individual work stations. The estimated square footage of this facility is 24,000. The Contractor shall be responsible for maintaining the entrance/exit of the building.
2. ADDRESS OF FACILITY. Contractor shall provide services at the following physical address:

3. DAILY CLEANING: The Contractor shall accomplish the following cleaning tasks each day. The facility's appearance must fully meet the sanitary and aesthetic requirements. This includes but not limited to bathrooms, staff break rooms, kitchenettes, and lobby.
a. Maintain floors. All floors, except carpeted areas, shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and spray buffed, as needed, to ensure they have a uniform, clean appearance and free from dirt, debris, dust, scuff marks, heel marks, other stains and discoloration, and other foreign matter. Baseboards, corners, and wall/floor edges shall also be cleaned. All floor maintenance solutions shall be removed from baseboards, furniture, trash receptacles, etc. Chairs, trash receptacles, and other moveable items shall be moved to maintain floors underneath these items. All moved items shall be returned to their original and proper position.
b. Remove Trash. Trash containers shall be emptied, cleaned, and returned to their initial location. Boxes, cans, and paper placed near a trash receptacle shall be removed. All plastic trash receptacle liners shall be replaced. The trash shall be collected by the Contractor and delivered to the designated trash storage area. Trash receptacles shall be left clean, free of foreign matter, and free of odors.
c. Clean Interior Glass/Mirrors. Clean all interior glass, including glass in doors, partitions, and windows. After glass cleaning, there shall be no traces of film, dirt, smudges, water, or other foreign matter.
d. Clean Drinking Fountains. Clean and disinfect all porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountain. Drinking fountains shall be free of streaks, stains, spots, smudges, scale, and other obvious soil.
e. Vacuum Carpets. Vacuum carpeted areas. After vacuuming, the carpeted area shall be free of all visible dirt, debris, litter, and other foreign matter. Spots shall be removed by carpet manufacturer’s approved methods as soon as noticed. All damage and any unremovable stains shall be brought to the attention of the COR.
g. Vacuum and Clean Floor Mats. The Contractor shall be responsible for the cleaning and maintaining of interior and exterior floor mats. Vacuum interior floor mats and sweep exterior floor mats. After vacuuming or cleaning, mats shall be free of all visible lint, litter, soil, and other foreign matter. Soil and moisture underneath mats shall be removed and mats returned to their normal location.
h. Restrooms. Clean and disinfect all surfaces of sinks, toilet bowls, urinals, lavatories, dispensers, plumbing fixtures and other such surfaces, using a germicidal detergent. Remove scale from toilet bowls and urinals, the entire surface shall be free from stains, scale, scum, urine deposits, and rust stains. Disinfect all surfaces or partitions, stall walls, stall doors, and wall areas adjacent to wall mounted lavatories, urinals, and toilets. Unclog toilets (when needed). If toilet do not unclog, the COR shall be notified immediately. Wipe all mirrors clean removing all dirt, smudges, water spots, and other foreign matter. Clean, damp mop and sanitize floors. Replenish supplies (paper towels, toilet paper, toilet seat covers and liquid soap).
h. Clean and Disinfect. Completely clean and disinfect all surfaces of sinks, toilet bowls, urinals, lavatories, dispensers, plumbing fixtures, partitions, doors, walls, and other surfaces, using a germicidal detergent. After cleaning, receptacles shall be free of deposits, dirt, streaks, and odors.
i. Disinfect all surfaces of partitions, stalls, stall doors, entry doors, (including handles, kick plates, ventilation grates, metal guards, etc.), and wall areas adjacent to wall mounted lavatories, urinals, and toilets.
4. WEEKLY CLEANING:
a. Scrubbing of Floors. The breakrooms and restrooms floors shall be scrubbed weekly.
b. General Spot Cleaning. Perform spot cleaning as needed. Spot cleaning includes, but is not limited to removing, or cleaning smudges, fingerprints, marks, streaks, spills, etc., from washable surfaces of all walls, partitions, vents, doors, door guards, door handles, push bars, kick plates, light switches, temperature controls, light covers and fixtures. After spot cleaning, the surface shall have a clean, uniform appearance, free of streaks, spots, and other evidence of soil.
c. General Dusting. All horizontal and vertical surfaces must be dusted or cleaned to eliminate dust collection.
5. MONTHLY CLEANING:
a. Descale Toilet Bowls, and Urinals. Descaling shall be performed monthly as a minimum, and as often as needed, to keep areas free of scale, soap films, and other deposits. After descaling, surfaces shall be free from streaks, stains, scale, scum, urine deposits, and rust stains.
b. Sweep and Mop Floor. After sweeping and mopping, the entire floor surface, including grout, shall be free from litter, dirt, dust, and debris. Grout on wall and floor tiles shall be free of dirt, scum, mildew, residue, etc. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film, or standing water. Moveable items shall be tilted or moved to sweep and damp mop underneath. Floors shall be scrubbed, as necessary to maintain sanitary conditions and a clean, uniform appearance, includes the kitchen area.
c. Stock Restroom Supplies. Contractor shall ensure restrooms are stocked sufficiently so that supplies including soap dispensers do not run out. Supplies shall be stored in designated areas. No overstocking shall be allowed. If supplies run out before the next service date, contractor shall refill within 24 hours. Contractor shall notify the COR when supplies are low.
6. QUARTERLY CLEANING:
Clean/Shampoo Carpet. All carpets, area rugs and door mats, shall be cleaned in accordance with standard commercial practices. A heavy-duty spot remover may be required in heavily soiled areas. After shampooing, the carpeted area will be uniform in appearance and free of stains and discoloration. All cleaning solutions shall be removed from baseboards, furniture, trash receptacles, chairs, and other similar items. Chairs, trash receptacles, and other items shall be moved to clean carpets underneath, and returned to their original location.
7. EMERGENCY CLEANING:
Emergency or Special Event Cleaning Services. Upon notification by the Contracting Officer's Representative (COR), the contractor shall perform emergency or special event cleaning required in any area or room covered under this contract. Contractor shall begin emergency work, as determined by the COR within 24 hour of notification, which may be verbal. The emergency or special event cleaning service shall be priced as an hourly rate per the schedule of services and price in Section B.3.
8. GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, SUPPLIES, AND SERVICES:
a. Dispensers for paper towels, dispensers for toilet paper, dispensers for soap, dispensers for sanitizers, dispensers for seat covers, and dispensers for hand lotion. Paper towels, toilet paper, toilet seat covers, anti-microbial hand soap, regular hand soap, hand sanitizer, hand lotion, toilet bowl cleaner, sink cleaner, stainless steel cleaner, descaler, feminine hygiene products (napkins/tampons), disinfectant surface cleaners, standard plastic liners for all size containers, all products needed for flu and hygiene stations, safety goggles (for janitor), and batteries for all dispensers.
b. Storage Areas. The Government will provide the space (i.e., facilities) for the contractor's use as supply and equipment storage spaces, including housekeeping closets. No alterations shall be made to these facilities. Upon completion or termination of the contract, the contractor shall return all Government furnished facilities and furnishings in the same condition and quality as received, fair wear and tear excepted.
c. Utilities and Services. The Government will furnish water, electricity, and sewer as may be required to perform work under the contract at no cost to the contractor. Utilities will be provided at existing outlets.
d. All repairs to the facility not due to the Contractor's negligence shall be the Agency's responsibility.
e. All repairs to the facility investigated and agreed that it is due to the Contractor’s negligence shall be charged and reimbursed to the Agency.
f. Removal of trash and waste from containers (e.g. dumpsters).
g. Supplies, Materials, and Equipment. The Government shall provide the following materials to be used by the Contractor only in performing the services required by this contract:
· Keys for areas to receive housekeeping services, other than restricted areas.
· New or replacement dispensers for paper towels, dispensers for toilet paper, dispensers for soap, dispensers for sanitizers, dispensers for seat covers, and dispensers for hand lotion.
9. CONTRACTOR FURNISHED ITEMS:
a. The Contractor shall furnish and maintain all cleaning/ maintenance equipment required to complete all phases of the job requirements ensuring that the areas are properly cleaned and neat in appearance at all times. The contractor shall be responsible for furnishing all cleaning chemicals supplies that have been approved by the COR. The Contractor shall provide the following:
· Disinfectant Floor Chemicals
· Carpet Cleaning Chemicals
· Window Cleaning Chemicals
· Mops/Mop Handles
· High Dust Cloths/Handles and Cleaning Rags
· All Types of Gloves (for Housekeepers
· Vaccum
· Mop and Mop Heads
· Brooms
· Dusters
· Carpet Cleaning Machine
· Buffer Machine
10. Approval of Supplies and Equipment.
a. Materials shall comply with Government fire and safety regulations. No materials bearing an Interstate Commerce Commission (ICC) red label shall be used. Proposed supplies and equipment for which specifications are not provided herein must be of commercial quality and must be of a suitable type or grade for the required housekeeping task. This list shall be a part of the Contractor's instructions and procedures manual. Any new item of material, supplies, or equipment shall be submitted to the CO or COR for review prior to use at the VA.
b. No supplies shall be left unattended and should be kept locked up when not in use.
c. Supplies to be stored not higher than 18 inches from ceiling sprinklers.
d. Abrasives. Steel wool, abrasive metal cleaners, or any other cleaning supplies or equipment, which could cause damage to Government property, shall not be used.
11. EQUIPMENT STANDARDS:
a. Sanitation. All original and replacement equipment introduced into the VA shall be in proper working order, as specified by the equipment manufacturer, and shall be cleaned with a disinfectant detergent free of soil prior to introduction into the VA. All equipment removed from a designated use area shall be cleaned with a disinfectant detergent prior to its removal from and reintroduction into the area.
Equipment must be kept clean and in good working order throughout the term of the contract.
b. Electrical Equipment. All electrically operated equipment shall be Underwriters Laboratories (UL) approved.
c. Bumper Guards. All wheeled and movable equipment shall be equipped with protective, non-marking wheels and rubber bumpers or guards. No part of the equipment, except handles, shall protrude beyond the rubber bumpers. Bumpers and guards shall be maintained in good repair at all times. Equipment with improper bumpers or guards shall be removed from service immediately and shall not be used until repaired. Any repairs to Government prope1ty required as a result of improperly protected equipment shall be made at the Contractor's expense.
12. MOP HEADS:
a. Cleaning of mop heads shall not be accomplished on the VA premises. All mop heads shall be treated as if contaminated.
b. All mop heads shall be clean and free of odor.
13. OIL-BASED TREATMENTS:
Contractor shall not use oil based treatments on floors. Chemical treatments shall not have a "flash point" of less than 330°F, and shall be free of specifiable matter. Chemical treatments shall not heat spontaneously under service conditions. Total treatment residue in mop heads shall not exceed 28 percent by weight and shall contain no carbon residue.
14. CLOTH USED FOR DUSTING AND SWEEPING:
Dusting and sweeping cloths shall be tubular cotton with 18 percent to 22 percent by weight, and shall be non-oil impregnated. The impregnation compound shall meet UL specifications and requirements relative to safety from combustibility, shall have an affinity for dust, and shall be registered with the EPA.
15. VACUUM CLEANERS:
a. All vacuum equipment shall have a hospital use, micro static, impaction-type filtration system, which filters out dust and bacteria particles larger than 0.3 microns. New micro static impaction filters and disposable paper filter bags shall be installed in micro static vacuums no less that every 30 calendar days.
b. Dry vacuum cleaners shall be commercial type with micro static, impaction-type exhaust air filter.
c. Upright vacuums shall be heavy duty and have a motor-driven beater brush.
d. Backpack vacuum units and wet-vacuums, used as components of combination scrubbers, are exempt from the above requirements.
16. CARPET SHAMPOOER/SOIL EXTRACTOR:
Shampooers/extractors shall be a commercial type, upright design, with agitator brush and motor, storage tank, compressor, dispenser, and shall have positive action vacuum.
17. CARTS:
a. Carts shall have a low platform for mop buckets, mop wringer, and other gear, with compartments for tools and cleaning supplies, trash collection device and lockable. Mops shall fit on the cart so that mop heads do not protrude above the top of the cart. Carts shall fit in assigned storage closets.
b. Carts shall be free of debris and clutter. Clean in appearance and serviceable.
18. TRASH COLLECTION CONTAINERS:
Contractor shall furnish a collection containers to be used for the overall trash pickup throughout the interior of the facility. Containers shall be rigid with a smooth interior finish and shall be equipped with a snug fitting cover and hard rubber, non-marking, silent-running casters. All trash containers shall meet the Government fire regulations. These containers shall be of such size that they do not block passageways or prevent building evacuation.
19. MOP BUCKETS:
Mop buckets may be either single or double compartmented and shall be constructed of a non porous, acid resistant, seamless material.
20. TOOLS:
All mops. brooms, and sweeping tools, which are equipped with handles of over 48 inches in length, shall be capped with non-marking rubber, vinyl, or plastic tips to prevent the marking or scarring of walls or other surfaces.
21. SIGNS:
All cleaning signage (i.e., floor care, restrooms, etc.) shall contain easily understood directions and guidance in English.
22. HANDLING OF SUPPLIES AND EQUIPMENT:
Specific routine infection control requirements are as follows:
a. All containers of cleaning chemicals and similar products shall be marked with a factory label, to identify contents, and all other labels shall be removed or defaced. Materials bearing Department of Transportation red labels shall not be used.
b. Contractor furnished supplies and equipment not in immediate use shall be stored in designated storage areas provided by the Government.
c. Supplies (e.g., toilet paper, paper towels, hand soaps, and detergents) shall be transported in carts designed for supply movement (e.g., housekeeping carts), not in trash barrels, nor mop buckets, all materials not immediately in use shall be properly stored in designated storage spaces provided by the Government.
23. SAFETY DATA SHEETS (SDS):
The Contractor shall furnish the VA with SDS sheets for all chemicals that are used at the ACC. This list shall be kept current at all times. This information is required by the VA for emergency treatment in the event of ingestion of and/or contact with the material by humans, and is required by OSHA regulations. Contractor must maintain a binder of all SDS sheets on site for use and inspection at all times.
24. PERSONNEL MANAGEMENT:
a. Discipline. The Contractor shall take prompt, appropriate action in all instances of employee behavior that may reflect adversely upon the Government. The Contractor shall furnish to the COR, for review, a written report of corrective action taken in those instances where an infraction of a Government regulation has been reported and substantiated.
b. Communication, All Contractor employees, must be able to read, write, speak, and understand the English language.
c. Standards of Conduct. While on duty and executing the specifications of this contract, Contractor employees are prohibited from using and/or possessing alcohol, firearms and prohibited items. Non-compliance with this requirement, confirmed by the COR, shall be grounds for immediate removal of the Contractor employee from the VA and baring the individual from performing further work under this contract. Contractor employee shall be required to provide customer service consistent with that of the VA's mission.
25. UNIFORMS:
All employees shall wear Contractor provided standard uniforms, including pants, shirts and blouses which are clearly identifies contractor name. Sandals or other open-toed shoes shall not be worn.
26. IDENTIFICATION BADGES:
All personnel shall display identification badges, except for protective clothing, which shall include the employee's full name and Contractor's full name.
27. PROTECTIVE CLOTHING:
When required the Contractor personnel shall wear special protective clothing and shoe covers while working in designated areas.
28. PERSONAL HYGIENE:
Contractor personnel shall be free of odor and wear a clean uniform at all times when in public areas. Fingernails shall be clean and free of dirt, and hair shall be neatly trimmed and combed.
29. EATING AND SMOKING:
Eating or smoking by Contractor personnel are permitted only in designated areas.
30. PHYSICAL SECURITY:
a. The Contractor shall be provided keys or allowed access to all rooms requiring housekeeping services. The Government may choose to accompany Contractor personnel when in certain secured areas. Keys provided to the Contractor shall not be duplicated or removed from the VA premises. The Government at the Contractor's expense shall replace any keys lost by Contractor personnel. In the event a master key is lost or duplicated, all locks and keys on that lock system will be replaced by the Government, and the total replacement cost shall be the responsibility of the Contractor. If a key to a single area is lost, the Government will replace the lock for that area, and the total replacement cost shall be the responsibility of the Contractor. The Contractor shall immediately report the occurrence of a lost key to the COR.
b. Securing Spaces. Contractor employees shall not allow anyone to use any key in their possession. They shall not open locked areas to permit entrance by persons other than Contractor employees performing assigned duties. All areas that are to be locked shall not be left unattended during the cleaning process and shall be relocked by Contractor personnel after completion of housekeeping duties. Contractor personnel shall turn off all lights in all unoccupied areas.
31 GENERAL SECURITY:
a. Passes and Badges. All Contractor personnel requiring access to the VA shall obtain employee identification badges required by the Government. Prior to the start of this contract the Contractor shall submit to the COR the names of all personnel performing work on this contract and all information required to prepare identification badges for all Contractor personnel. The Government will issue these badges without charge to the Contractor. Contractor employees shall wear the Government furnished badge on the front of their outer clothing, except for protective clothing. When an employee leaves the Contractor's employment, the Contractor shall return all identification badges and passes of the employee issued by the Government to the COR or security personnel upon completion of their employment.
b. Consequences of Illegal Activities. Contractor employees who are found in violation of Government security regulations shall be immediately removed and barred from performing further work under this contract.
c. Loitering. Contractor employees shall not loiter on the MAB grounds when in a non- duty status. After completing assigned work shifts, all Contractor personnel shall promptly depart the station.
32. SAFETY:
All work performed in delivering the services specified in this contract shall comply with applicable Federal, State, and Local safety regulations. The Contractor shall have a working knowledge of the following codes, standards, recommended practices, and manuals:
a. Public Law 91- 596, Occupational Safety and Health (OSHA) Act of 1970 and current amendments
b. National Fire Codes and Standards pertaining to the health care field developed by National Fire Protection Association, current listings
c. VA Disaster Control Plan
d. Safety Data Sheets (SDS).
e. Failure to Comply. Contractor fails or refuses to promptly comply with applicable safety directives, the Contracting Officer may issue an order stopping all work until satisfactory corrective action has been taken by the Contractor.
f. Emergency Treatment. The Government shall furnish only emergency first aid treatment to Contractor employee who may suffer on-the-job injuries.
33. CONTRACTOR RESPONSIBILITIES:
a. Contractor employees shall not place mops, brooms, electrical cords, hoses, machines, and other equipment in traffic lanes or other locations in a manner that creates a safety hazard.
b. Contractor employees shall not use parallel wet mopping or cleaning procedures when the cleaning of public traffic areas (e.g.• lobbies and corridors) may result in a temporarily wet or slippery floor surface. Contractor employees shall accomplish floor cleaning such that at no time will it be necessary for staff personnel, or visitors to cross a wet floor to gain access to other areas of the VA. Contractor employees shall display warning signs in all areas where housekeeping operations may cause traffic obstruction or hazard to staff personnel or visitors.
c. Contractor employees shall interrupt their work at any time to allow the passage of staff personnel, visitors, equipment, or carts.
d. Contractor employees shall wear personal protective clothing and/or equipment when handling, pouring, or working with chemicals or cleaning agents, which may expose the employee to injury and/or occupational illness.
34. HOURS OF SERVICE/COVERAGE:
a. The Contractor shall perform janitorial services five (5) days a week, Monday through Friday, 8:00 – 5:00 p.m. exception for National Holidays (see below).
b. Contractor employee shall be present and working during the specified timeframes. The COR must approve any changes to the Contractor's schedule in advance.

c. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday by U.S. Government agencies. Also, included would be any other day specifically declared by the President of the United States to be a National Holiday.
35. CONSERVATION OF UTILITIES:
The Contractor shall instruct Contractor employees in utilities conservation practices. The Contractor shall operate under conditions which preclude the waste of utilities, which shall include:
a. Lights shall be used only in areas when work is actually being performed. Otherwise, lights shall be turned off.
b. The Contractor’s employees shall not adjust mechanical equipment controls for heating, ventilation, and air conditioning systems.
c. Water faucets or valves shall be turned off after their required use.
d. Government telephones shall not be used for personal use.
36. AUTHORIZED SERVICES:
Only those services specified herein are authorized under this contract. Contractor shall be cautioned that only the CO may authorize additional services and that reimbursement shall not be made unless prior authorization is obtained.
37. CHANGES:
a. The contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which may affect the contract terms and conditions, quality, quantity, price, and/or delivery. The contractor must contact the Contracting Officer for approval in the event there are changes prior to proceeding. No other individual has the authority to make this binding changes and/or commitments.

In the event that the Contractor commits to such changes at the direction of any person other than the Contracting Officer, the changes shall be considered to have been made without proper authority and no adjustment shall be made in contract price to cover any increase in costs incurred as a result thereof. | |
| 38. | CONTRACTING OFFICER REPRESENTATIVE (COR): |
A COR may be delegated to assist the Contracting Officer in the administration and monitoring the resulting contract. A Delegation of Authority letter issued to the COR, a copy of which will be sent to the Contractor, will state the responsibilities and limitations of the COR. The COR is not authorized to change any of the terms and conditions of the resulting order.
39. IDENTIFICATION OF CONTRACTOR EMPLOYEES:
a. The parties agree that the contractor personnel when the resulting contract will be performed shall not be considered VA employees for any purpose and shall be considered employees of the contractor.
b. The contractor shall provide a representative who shall be responsible for routine liaison in the performance and administration of the resulting contract. In addition, the representative shall act as the primary point of contact (POC) between the Contracting Officer and the contractor. The representative shall have full authority to act on behalf of the contractor on all matters relating to daily operations of contract performance. An alternate POC shall also be designated. The contractor shall provide the names and telephone numbers of the POC(s) in the space below:

40. Display of ID Badges:
Contractor personnel shall wear the ID badge at all times during Contract Performance. Unless otherwise specified in the contract, each Contractor employee shall wear the ID badge in a conspicuous place on the front of exterior clothing and above the waist. I.D. badge must have picture identification, name of the individual and the represented company depicted on it.
41. Standards of Conduct:
a. Contractor personnel shall present a neat, well groomed, professional appearance at all time. Contractor personnel shall not wear sandals, flip-flop style footwear, or open toed shoes.
b. Alcohol/Drug use in the workplace will not be tolerated. Any Contractor employee found to be under the influence of, consuming, or using alcohol or drugs while on duty will be subject to administrative action from the Contracting Officer, which may include contract termination.
c. The contractor shall ensure that all contractor personnel have met qualifying criteria. Ultimately, the decision to accept or reject Contractor personnel rests with the Contracting Officer. Non-acceptance of contract personnel does not relive the Contractor from satisfying and fulfilling the contract requirements.
d. The Government reserves the right to refuse acceptance of Contractor personnel whose personal or professional conduct jeopardized or interferes with the operation of the facility. Breaches of conduct include intoxication or debilitation resulting from drug use, theft, dereliction or negligence in performing directed tasks, or other abuse, or other conduct resulting in formal complaints by staff members to designated Government Representatives. Standards for conduct shall mirror those prescribed by current Federal Personnel Regulations.
42. CONTRACTOR SECURITY REQUIREMENTS:
a. The Office of Security and Law enforcement provides Department-wide policy on the assignment of appropriate position sensitivity designations associated with Department of Veterans Affairs (VA) positions involving national security and public trust responsibilities, and on the level of background investigations required for applicants for, and incumbents of, those positions.
b. IAW VA 0710 Handbook, appointees and contract personnel appointed to Low/Moderate/High Risk positions must be subjects of a background investigation conducted by OPM and receive a favorable adjudication from the VA Office of Security and Law Enforcement to ensure compliance with such policy, If such investigation has not been completed prior to contract commencement, the Contractor shall be responsible for the actions of those individuals performing under the contract.
c. Should the contract require Contractor’s personnel to maintain U.S. citizenship, the Contractor shall be responsible for compliance. Regardless of U.S. citizenship requirements, Contractor’s personnel are required to read, write, speak, and understand the English language, unless otherwise specified herein or agreed to by the Government. The level of sensitivity shall be determined by the Government based on the type of access required.
d. The Contractor shall be required to furnish all applicable employee information required to conduct the investigation, such as, but not limited to, the name, address, and social security number of the Contractor’s personnel. The VA will provide all the necessary instructions and guidance for submission of the documents required to conduct the background investigation. Background investigations shall not be required for contract personnel assigned to Low Risk/Non-sensitive positions for 180 days for less under a single contract or series of contracts. However, a Security Access Clearance (SAC) background screening will be required for appropriate preliminary checks IAW VA Directive 0710.
e. Background Screening: In accordance with VHA Directive 0710 all Contractor personnel providing services under this resulting contract shall be subject to a background screening and must receive favorable adjudication from the VA prior to contract performance. The level of screening for this resulting contract is:
Special Agreement Check (SAC). Any employee whose background investigation yields unfavorable results shall be removed immediately from performance under this resulting contract. In the event the screening is not completed prior to contract performance, The Contractor shall be responsible for the actions of those individuals performing under the contract.
f. Contractors, contract personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding VA information and information system security.
43. INSURANCE LIABILITY:
Prior to commencement of work to be performed, the Contractor shall furnish an insurance certificate indicating that the coverage outlined in paragraphs a and b have been obtained and that it may not be changed or canceled without a guaranteed thirty (30) day notice to the CO.
(a) The Contractor shall maintain Workers Compensation and Employee’s Public Liability Insurance in accordance with the laws of California.
(b) The Contractor shall comply with all applicable Federal and State laws regarding liability for the injury or death of an employee in performing the work under this contract and shall hold the Government harmless against any or all loss, cost, damage, claim expense or liability for accident or injury to persons or property occurring in the performance of this contract.
44. SAFETY AND FIRE PREVENTION:
The Contractor shall take such safety precautions as necessary to protect the lives and health of the occupants of the building. Fire and Safety deficiencies, which exist and are part of the responsibility of the contractor, shall be immediately corrected. If the Contractor fails or refuses to correct deficiencies promptly, the CO may issue an order stopping all, or any part, of the work. The Contractor shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. The Contractor personnel shall follow applicable facility policies concerning fire/disaster programs.
45. OTHER REQUIREMENTS:
(a) VA Policy: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court.
(b) Parking Policy: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available for the VA Police. The VA will not validate or make reimbursement for parking violations of the Contractor’s personnel under any circumstances.
(c) Smoking Policy: Smoking shall be permitted only in designated areas provided by the Contractor.
SECTION C - CONTRACT CLAUSES
C.1 ADDENDUM to FAR 52.212-4 CONTRACT TERMS AND CONDITIONS—COMMERCIAL ITEMS The following clauses are incorporated into 52.212-4 as an addendum:
C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

C.3 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

(a) Definition. For the Department of Veterans Affairs, “Service-disabled veteran-owned small business concern or SDVSOB”:
(1) Means a small business concern:
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans or eligible surviving spouses (see VAAR 802.201 Surviving Spouse definition);
