SAMPLE: PERFORMANCE WORK STATEMENT

1. General Overview

Contractor shall provide the San Francisco VA Medical Center with all labor, materials, transportation, disposal, supervision and technical expertise to provide mold abatement and remediation services to remove contents consisting of hospital related biological samples and deactivate the black mold. The contractor shall perform to the standards in this contract and all applicable commercial, industry, federal, state, city, and local laws and regulations.

3. Place of Performance

a. The work to be performed under this contract will be performed at San Francisco VA Medical Center located at San Francisco, CA 94121.

4. Period of Performance and Schedule

a. Period of Performance. This will be a one-time service requirement.

b. Schedule. All work shall be done Monday through Friday, excluding in accordance with emergency incidents with Environmental Health and Safety.

c. The frequency of these tasks will be determined by Environmental Health and Safety.

5. Equipment List

a. List the equipment to be serviced with the make, model, description, location, and serial numbers.

6. Contractor Requirements

a. Contractor shall be currently certified in mold remediation.

b. Removal of mold and contents with mold: The contractor is required to remove contents contaminated with mold and deactivate the black mold present. After removing the mold contents, the contractor shall remove all liquid, solid debris, garbage and other matter that has accumulated in accordance with the contract. The contractor shall clean the area and have the space safe to enter.

c. Disposal of mold contents : The Contractor shall be responsible for removal and disposal of all mold, garbage and debris from VA property in accordance with applicable laws, ordinances and regulations of Federal, State and local guidelines. Waste Disposal Manifests and disposal documentation showing amounts disposed of will be provided to the COR immediately after each service.

d. As a part of the cleaning, inspect and make a written report on any abnormal conditions found. This Service Completion Report will be provided to the COR at the completion of each service visit.

e. Rescheduling Services: In an unforeseen event such as weather that may prevent the service from being conducted, the contractor shall be notified by the POC and at the government’s discretion work schedule. The contractor shall provide this service at no additional cost to the government.

f. The contract certifies that all applicable services offered under this contract shall conform to all common industry standards. The contractor for these services will meet any regulatory requirements.

g. The Contractor shall possess and maintain all necessary required insurance, license and permits required for contract performance and in accordance with local, state, federal regulatory authorities and the Federal Acquisition Regulation. Contractor is responsible for any injury to themselves, or their employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by employees’ fault or negligence.

h. Reporting “on-the-job” Injuries: The Contractor is required to report all “on-the-job” injuries incurred by the Contractor, its agents or employees, resulting from performance of this contract. Contractor will notify the contracting officer representative (either orally or via telephone) within twenty-four (24) hours of the injury and provide details and exact location of the incident. This will be followed up by a written notice to the COR.

7. Reporting Requirements

a. As a part of the cleaning, inspect and make a written report on any abnormal conditions found. This report will be provided to the COR at the completion of each service visit.

The Contractor is required to report all on-the-job injuries incurred by the Contractor, its agents or employees, resulting from performance of this contract. Contractor will notify the COR (orally and electronically) within 24 hours of the injury and provide details and exact location of the incident.

7. General Requirements

a. It shall be understood that, throughout performance of the contract, the Contractor’s personnel, while on VA property, shall adhere to all requirements and regulations that govern the VA Health Care System and its property.

b. Among all other requirements, the VA Health Care System and its property is now entirely smoke-free.
