ATTACHMENT 4

Factor 3: Experience

· Submit a maximum of five (5) contracts in which the Offeror (or JV Partner) was the Prime Contractor that best demonstrates the Offeror's experience on recent relevant contracts that are similar in size, scope, and complexity to this RFP (complete Attachment A spreadsheet in Section D). For purposes of this evaluation, recent is defined as a contract that has been completed within the past 3 years of the proposal issue date for this RFP and/or an ongoing contract where at least 6 months of performance has occurred. The Offeror (or JV Partner) must have been a Prime Contractor. Relevant is defined as projects over $1M annually where the scope of the work was for on-call medical transport via either special mode grounds transportation or ground ambulance services. Experience that is not recent and/or relevant will not be further evaluated.

· A contract is defined as a medical transport service performed under a single task order or contract. For multiple award and indefinite delivery/indefinite quantity type contracts (IDIQ), the contract as a whole shall not be submitted as experience; rather Offerors shall submit the work performed under a task order. If there were multiple task orders awarded under an IDIQ within a year that total over $1M, then those task orders may be submitted as one (1) experience.

· If the Offeror is a Joint Venture (JV) or a participant of Small Business Administration (SBA) Mentor-Protégé Program, recent relevant project experience should be submitted for projects completed by the Joint Venture entity or SBA Mentor-Protégé. If the JV or SBA Mentor-Protégé does not have shared experience, recent relevant projects shall be submitted for each JV partner or for the Mentor and Protégé. Offerors who fail to submit experience for all JV partners or Mentor and Protégé may be rated lower. Offerors are still limited to a total of five (5) projects combined.

Outstanding (O)

Four or more of the offeror’s experience projects is relevant relative to the requirement. Strengths far outweigh any weaknesses. Risk of unsuccessful performance is very low.

Good (G)

Between two to three of the offeror’s experience projects is relevant relative to the requirement. Proposal contains strengths which outweigh any weaknesses. Risk of unsuccessful performance is low.

Acceptable (A)

The offeror has relevant experience for less than two recent and relevant projects.
