The purpose of Amendment 001 is issued to provide responses to questions submitted and extend the due date for quotes to Friday March 29, 2019 by 4:00pm.
Item 1: In the SOW, Section 4.1, CLIN 001, part d) states: “energy source identification labels to be provided and fastened to energy control points where missing.” Does the government have a requirement for the specific material for the labels and mounting devices (i.e. plastic tags and metal zip ties)?
Response 1: The Procedures shall be printed, laminated and hung at each piece of equipment with Serialized Isolation Point Tags.
Item 2: Please provide a list of equipment covered for this project.
Response 2: VAPSHCS Seattle and American Lake (SEA/AML campuses)- Asset Equipment Inventory List.
Item 3: What buildings are covered by this SOW? Building 1, 101, the whole campus?
Response 3: All VAPSHCS-Seattle campus except for building 101 and All VAPSHCS-American Lake campus.
Item 4: What systems are covered by this SOW? (e.g. Elevator, HVAC {boilers, chillers, AHUs?}, Medical gas, Electrical {within what voltage range?}, Plumbing {to what psi minimum?}, Pneumatic {to what psi minimum?}, Fuels/Lubricants, etc.
Response 4: VAPSHCS Asset Equipment Inventory List is available.
Energy sources-electrical(120V-15KVA), mechanical, hydraulic, pneumatic(psi-40-90), chemical, thermal (heat), radiation, hydro (water)(psi-74), fluid-gases, and potential (stored).
Item 5: Has this study been previously completed? If so, when?
Response 5: No, this study has not been previous completed. OSHA 1910.147(c)(4) Energy Control Procedures is required.
Item 6: Are all shutoff devices like valves, switches, circuit breakers, etc. already marked with individual ID’s to reference for LOTO procedures?
Response 6: Partially. The VAPSHCS Asset Equipment Inventory List is available. The vendor will be responsible to complete analysis of the equipment inventory; this may be at the equipment or traced back to the closest isolation point.
Item 7: Have these shutoff devices been line traced back to their power source to ensure individual placards or IDs are accurate?

Item 8: Page 5 of 56 (4.2 CLIN 0002 b.) states the deliverable is to provide a ‘list of equipment’, but it also references supplying consumables like locks. Please specify if and what type of devices are covered.
Response 8: In accordance of Department of Labor Occupational Safety and Health Administration (OSHA) Standard as follows:
1910.147(c)(5) Protective materials and hardware.
1910.147(c)(5)(i) Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources.
1910.147(c)(5)(ii) Lockout devices and tagout devices shall be singularly identified; shall be the only devices(s) used for controlling energy; shall not be used for other purposes; and shall meet the following requirements:
1910.147(c)(5)(ii)(A) Durable.
1910.147(c)(5)(ii)(A)(1) Lockout and tagout devices shall be capable of withstanding the environment to which they are exposed for the maximum period of time that exposure is expected.
1910.147(c)(5)(ii)(A)(2) Tagout devices shall be constructed and printed so that exposure to weather conditions or wet and damp locations will not cause the tag to deteriorate or the message on the tag to become illegible.
1910.147(c)(5)(ii)(A)(3) Tags shall not deteriorate when used in corrosive environments such as areas where acid and alkali chemicals are handled and stored.
1910.147(c)(5)(ii)(B) Standardized. Lockout and tagout devices shall be standardized within the facility in at least one of the following criteria: Color; shape; or size; and additionally, in the case of tagout devices, print and format shall be standardized.
1910.147(c)(5)(ii)(C) Substantial - 1910.147(c)(5)(ii)(C)(1) Lockout devices. Lockout devices shall be substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools.
1910.147(c)(5)(ii)(C)(2) Tagout devices. Tagout devices, including their means of attachment, shall be substantial enough to prevent inadvertent or accidental removal. Tagout device attachment means shall be of a non-reusable type, attachable by hand, self-locking, and non-releasable with a minimum unlocking strength of no less than 50 pounds and having the general design and basic characteristics of being at least equivalent to a one-piece, all environment-tolerant nylon cable tie.
1910.147(c)(5)(ii)(D) Identifiable. Lockout devices and tagout devices shall indicate the identity of the employee applying the device(s).
1910.147(c)(5)(iii) Tagout devices shall warn against hazardous conditions if the machine or equipment is energized and shall include a legend such as the following: Do Not Start. Do Not Open. Do Not Close. Do Not Energize. Do Not Operate.
Item 9: It will be impossible for bidders to include a comprehensive list of LOTO equipment until the study is complete. Therefore, it is recommended the government allocate a block of funds for this CLIN.
Response: Funds are available for all CLINS indicated. The comprehensive list shall be provided once the study is complete. The replacement or installation of equipment will be evaluated and may be included upon modification if deemed within scope of work.
Item 10: When was the last arc flash study completed and will the awardee be given access to this report?
Response: Seattle campus- The latest arc flash study was conducted in April 2017.
American Lake campus- The latest arc flash study is currently in progress.
Item 11: Will the VA provide support personnel for more complicated system shutdowns?
Response: Yes. The VA will provide personnel to facilitate the vendor’s performance surveying the equipment inventory and procedure development.
Item 12: Will systems need to be actually isolated to create procedures. For example, will the boiler LOTO entail actually shutting down and restarting the boiler?
Response 12: Yes. The vendor will determine the specific hazardous energy systems and points of isolation (valve, switch, breaker, or mechanism) that is required to be locked out to bring that equipment to a zero-energy state.
